DEP to Mr. Victor Alfonso & Mr. Robert Nault - re; Notice of Audit Finding, Audit Inspection, 261 Chauncy St. Dated April 10, 2003 On April 4, 2003, an audit inspection at CVS was done by the Massachusetts Department of Environmental Protection. The Department did not identify any violations. c. DEP to David Doran, President and Samuel Andrews, Chief Financial Officer of Doran & Bagnall & Company, Inc.- re; Notice of Responsibility Former Sweet Manufacturing Co. Dated April 28, 2003 Inspection report was received from the EPA dated January 13, 2003 containing sampling data results indicating hazardous material has occurred at the location referenced and as owners they are the responsible parties. d. DEP to Bristol South Management - re; Notice of Responsibility at 23 Erick Road Parking Lot for Building #23. Dated April 30, 2003 An undetermined quantity of diesel fuel was released from a motor vehicle fuel line. The release impacted a paved parking lot. $750 fine imposed. e. DEP to Mr. Kenneth F. Hackett, Town of Mansfield - re; Residuals Approval of Suitability. Dated Apri130,2003 Approval to sell compost produced at the Town of Mansfield Sludge Composting Facility in Mansfield. f. Bristol County Mosquito Control Project to Board of Health - re; Notification of Mosquito adulticide applications. Dated April 30, 2003 Public notification of mosquito adulticide applications r 4. Health Agent's Report a. CDM - Update on Sewer Master Plan Corrected - should be Wastewater Management Plan. Health Agent is meeting with Camp Dresser McKee to work on a colored keyed map of soil types in town and standardizing basic descriptive soil types and geologic terms. b. 340 South Main St. - Gorham Silver In the late 1980's, chemicals were being dumped into the ground and water plumes were traveling, therefore, the BOH ordered wells in the area not to be used. These wells are periodically tested. Reminder letters were recently sent out by ENSR reminding residents to have their wells tested. c. Lyme Disease Seminar Health Agent stated the seminar can be held the second Thursday in June. He suggested holding the BOH monthly meeting prior to the seminar in the 31d floor meeting room.
