3. Communications
a. DEP to Mr. Frank O'Kom, Plant Mgr of ADM Cocoa - re; Notice of Inspection. Dated April 2, 2004
b. Health Care Options Report for the Town of Mansfield. March 2004
c. Bristol County Mosquito Control Project to Board of Health re-Notification of mosquito adulticide
applications. Dated May 1, 2004
4. Health Agent's Report
a. Household Hazardous Waste Collection
a. Event - Held April3rd. spent approximately $14,000
b. Consortium - Focus on public education.
b. Emergency Preparedness - several meetings. No money available yet. Health Agent spoke
with Fire Chief and recommended they choose the equipment we should have and store it at
the fire station.
c. 2 Brookside Drive - Homeowner is putting on an addition. Architect did not take into account
the septic system on the plans. Addition is too close to the leaching fields. Health Agent
stated an enforcement letter will be issued.

d. 262 Pllain St. - House is on the market. Owners found an installer to install new system.
Health Agent is meeting with the installer on Monday.
e. Predatory Business Practices regarding Septic Systems - Health Agents suggested putting a
statement on the website warning residents of unethical business practices used by some
businesses regarding their septic systems. Also residents can contact him with any questions
regarding their septic systems.
f. Budget - Nursing services were not cut.
