MANSFIELD HIGH SCHOOL
Hay Bales — Jim May reported that J.T. Callahan was issued directives from Wallace
Floyd re: Revisions to the location and extension of hay bale placement adjacent to
playing field and expanded parking along East St. Hay bales are in place as directed.
Data Cabling —Bob Gurek reported that the School Department is currently investigating
` the possibility of Time Warner Cable providing computer cabling for all schools. If this
is a possibility, the School Dept. will not pursue contract negotiations with JCI as
planned.
TV Studio at MHS —Discussion was opened as to whether or not Mansfield Cable
Access would be subject to public bid laws in paying to build the Studio at Mansfield
High School. Mr. May stated that it is his understanding from Cable Access, that since
their private funds are paying to build the building, that the TV Studio Project would not
be subject to the public bid law. Bob Gurek strongly disagreed. Mr. Feeley responded
that since the TV Studio is to be built on school property and attached to a public
building (Mansfield High School), it would be considered a public building, and therefore
most likely be subject to public bidding laws. A suggestion was made by David Young
to schedule a meeting with John D'Agostino, Town Manager, School Dept. and
Mansfield Cable Access to clarify the proper bidding procedures required. Rich Feeley
added that coordination of all construction activity on site will be essential to both
projects.
JTC Management Team Concerns — Rich Feeley opened discussion with the Committee
regarding the job performance of J.T. Callahan's MHS Site Supervisor. Mr. Messina's
general uncooperativeness and attitude has negatively impacted working relationships
with the Project Coordinator, Building Inspectors, and MBC some members to date.
Mr. Feeley cited the following examples as reasons for concern:
• Work schedule — Job Supervisor is not on site full time.
• Job Supervisor does not leave job trailer when on site.
Job Supervisor allowed J.T. Callahan to perform construction activity in a
wetland area without a permit from the Conservation Commission.
• Job Supervisor allowed J.T. Callahan to perform demolition activity within the
courtyard area of the high school without a demo. permit from the Building Dept.
• Job Supervisor has not enforced on-site hard hat and security tag system.
• Job Supervisor has scheduled and held project site meetings inconvenient and/or
without notice for owner to attend.
It was noted that the issues regarding permitting have been addressed with J.T. Callahan.
Demolition and Conservation and Foundation Permits have since been issued. Paul
Letendre to follow up with J.T. Callahan regarding enforcement of hard hats on site and
security tags on site. Rich Feeley requested that Paul Letendre document dates and times
of Mr. Messina's time schedule in the daily reports. Jim May to check references on
Tony Messina from previous jobs. The Committee agreed that any future issues of
concern would be addressed with John T. Callahan III and would support a reason to
request that Mr. Messina be removed from the Project if necessary.
Site Meeting Cancelled — Paul Letendre reported that the site meeting, scheduled for
Friday, February 19, 1999, was cancelled by J.T. Callahan due to the Site Supervisor
(Tony Messina) taking vacation.
Emergency Numbers — Mr. Letendre reported that emergency telephone numbers have
been posted in trailers.
Egress Plan/South Addition — Jim May noted that an egress plan has not yet been
submitted to Fire Dept. for the South Addition. Work in this area will block an existing
entry. Paul Letendre to follow up with Fire Department.
Fencing — Site fencing has been reviewed with Lt. Sarro. No problems were indicated.
Site Lighting — Questions were raised as to the adequacy of site lighting planned. David
Young questioned why light poles were not indicated near the small front parking lot.
Jim May agreed that lighting was planned for that area. Mr. May to follow up. The
committee also questioned the number of poles scheduled for installation in the main
parking area. The Committee expressed concerns as to whether the new site lighting will
be able to sufficiently light the additional parking spaces. Mr. May commented that a
high intensity fixture is planned. Issue pending further review. Request was made by
Bob Gurek to explore the possibility of individual light controls.
Electrical Service Needs —Walter Parker indicated to the MBC that no provisions were
made to feed electrical service to the maintenance building or pump shed at MHS. Both
buildings are currently being fed service off of light poles that are scheduled for removal
under the construction project. The MBC requested that Wallace Floyd prepare plans for
underground feeds to the pump shed at MHS and the maintenance shed. Jim May to
follow up.
Soils — Paul Letendre was asked to monitor soil compaction and de -watering near
foundation work. David Young noticed a large puddle of water near west foundation.
Ball field — The committee asked for clarification that the existing ball field would be
excavated completely and replaced with 4" sand and 6" loam. Discussion was that Jim
May will make notation in changes to plan to remove existing in -field.
Bleachers — Walter Parker questioned whether the contract would still provide portable
bleachers for ball field. Mr. Parker stated field hockey would like to have portable
bleachers on site that could be relocated each year when they change the direction of the
playing field. It was discussed that bleachers could be purchased under the original
contract as planned. J.T. Callahan would not be asked to install. Jim May to follow up.
Dug Out — Discussion was opened re: the feasibility of moving or donating the existing
dug outs at the ball field. Rich Feeley noted that the existing dug outs are scheduled for
demolition under the construction contract. It was discussed that the work required to
move the existing dug outs would be extensive and would also require that award of the
dug outs go through a public bid process. On behalf of the School Dept., Mr. Gurek
agreed that the dug outs should be left for demo as planned.
Pole Vault pits — Rich Feeley questioned Walter Parker as to the status of relocating the
pole vault pits behind the high school. Mr. Parker had indicated to the MBC at the time
/ revisions were being made to the parking plan that he would find a new location for the
Y pits. Mr. Parker reported that he has not yet coordinated this effort.
Rebar — Jim May provided an update re: submittals previously provided by J.T. Callahan
for rebar that were rejected. Submittals have since been revised, resubmitted and
approved.
Courtyard — Paul Letendre reported that the slab is completely removed in courtyard.
Excavation only is planned for this week. No rebar is yet in hand. Mr. Letendre noted
that the ground is being covered each evening for protection.
Steel drawings — J.T. Callahan has not submitted any steel drawings as yet. Jim May
noted that Wallace Floyd is up to date with all RFI's, with the exception of one relating to
steel.
Payment Requisition #1/J.T. Callahan — Mansfield High School Project
The committee reviewed Application for Payment #1 with Jim May, Wallace Floyd.
Rich Feeley requested confirmation from Mr. May that all required documentation, i.e.
insurance, schedule etc. was provided by J.T. Callahan. Mr. May identified all the
documents that were received. Mr. Feeley requested that Mr. May notify J.T. Callahan
regarding construction photos that are also required, but were not provided. Jim May to
follow up.
MOTION to recommend payment to J.T. Callahan as recommended by Wallace
Floyd Associates in the amount of $106,993.75 DY/JL Passes 3-1-0

anyone to date. The suggestion was made to contact Albin Pilblad, Structural Engineer to
prepare a proposal for a Peer Review of the Structural Documents for MHS as required.
MBC Secretary will follow up.
Asbestos Abatement - Jim May reported that J.T. Callahan has sent 3 proposals for
abatement work at MHS to Ammar Dieb at Universal Engineering for review.
Payment Requisition/Paul Letendre — The Committee discussed recommendation of
payment of one personal day indicated on Mr. Letendre's first scheduled day of
employment. Mr. Letendre is allowed 8 personal days per his contract. It was agreed to
that since Mr. Letendre had worked time prior to his official start date, the personal day
should be recommended for approval.
MOTION was made to recommend payment to Paul Letendre, MHS Project
Coordinator in the amount $2,400.00 for weeks ending February 6t1' and February
13t11, 1999. JL/DY Passes 4-0.
ROBINSON ELEMENTARY SCHOOL
OutstandingIssues -
• Jersey Barrier — Rich Feeley questioned Walter Parker re: Status of the placement of a
Jersey barrier to be placed in front of the gas meter at Robinson. Mr. Parker
responded that he was having difficulty locating one. Mr. Feeley suggested that Mr.
Parker contact the Boro Sand and Gravel Co. to arrange purchase. MBC will cover
cost of purchase under the School Project account. A barrier must be placed to meet
code requirements.
• Painting of Exterior Gas Pipe —Walter Parker confirmed that his staff has painted the
pipe gray as directed by code from the Building Dept.
QUALTERS MIDDLE SCHOOL
Knox Box — Rich Feeley questioned Walter Parker as to the status of a December 4°i
memo requesting that he order a knox box for QMS. Mr. Parker responded that he had
asked his electrician to order the unit. Mr. Parker will follow up.
Water Coolers — A letter was sent to Walter Parker from Chris Sullivan identifying (5)
broken water coolers at QMS. Mr. Parker was asked to confirm any other bubblers that
might also be broken.
Damage to Ramp Wall — David Young reported that one of the concrete walls at the front
entrance ramp of Qualters has been scored along one side. Mr. Young noted that the
markings were fairly deep suggesting that something heavy caused the marking. Actual
cause of damage is unknown at this time.
Payment Requisition/Christina Sullivan — The Committee reviewed for approval.
MOTION to recommend payment to Christina Sullivan, QMS Project Coordinator
in the amount of $2,160.00 for weeks ending February 61h and February 13, 1999.
Invoiced amount includes deduction for one sick day pay. JLIKG Passes 4-0.
Drainage Swale -Clean out — It was discussed by the MBC that cleaning the swale is a
maintenance issue and is not covered under the realm of the school construction project.
The Committee agreed that J.T. Callahan has completed the work to the swale required
by their contract. If the debris in the swale is to be cleaned out, it is up to the Town to
make that decision, not the MBC. It was suggested by David Young to send a memo
from the MBC to the School Department and Town Manager with cc: to Dick Lewis,
Conservation Commission to determine how this issue will be addressed.
