QUALTERS MIDDLE SCHOOL
Inspection Status — QMS received a 30 day Temporary Occupancy Permit from the
Building Dept. signed by Nick Riccio dated Sept. 7, 1999. J.T. Callahan will need to
send a written request to the Building Dept. near the end of the 30 day period to extend
the Temporary Occupancy Permit.
Site Fencing — Discussion was that J.T. Callahan would install necessary fencing on site
to secure work areas and landscaping. David Young commented that the loam area is not
fenced. Project Coordinator to follow up.
Additional Walk — Discussion opened Re: David Young's previous suggestion that an
additional walk be installed in the side parking lot at handicap ramp. Adding a walk that
follows the logical foot traffic path, would reduce tracking mud and grass into the school
as well as damage to the seeded area. Waiting for Keyes Associates to provide sketch
from the new handicap ramp area to the existing walkway being centered on the nearby
manhole.
Additional Manhole/drain — Walter Parker commented Re: conversation with Building
Inspectors suggesting to add a drain basin at the rear loading dock near elevator pit to
catch water overflow. Project Coordinator commented that a basin already exists in that
location. David Young will visit site to review.
Egress Plan — Project Coordinator reported, that the Fire Dept. needs to review the egress
path of travel with the school principal. Lt. Sarro is unavailable to meet this week.
Discussion was that path of travel is logical for most areas in the building with the
exception of administration which has two exits within a close distance.
Phones — Chris Sullivan reported having a few minor problems with installation. Phones
are currently working.
Elevators — Directive was issued by Keyes Associates to install a heat detector. Project
Coordinator commented that there is no heat detector in elevator shaft as yet. It is the
responsibility of the Contractor to request the elevator inspection. Elevator phones will
need to be connected through the Town's service following inspection.
Shop Window — MBC questioned the status of window installation. Steve Chmura
commented that the shop window is incomplete. Cheviot was working to finish voids in
classrooms today. Mr. Chmura estimated completion of shop room windows in 3 weeks.
Library — Chairman expressed concern to the Project Coordinator regarding completing
work in the library in a timely fashion. Chris Sullivan will follow up with JTC.
Trailer Rental — Walter Parker informed the committee of an agreement made with Eagle
Leasing to take over use of a storage trailer rental currently on site. Rental will be paid
for out of the MBC school budget account.
Sills — Chairman questioned status of the sills that were removed. Mr. Feeley asked that
Chris Sullivan follow up with subcontractor to determine when the window system can
be put in place.
Door Entry/Administration — Fix is needed to fill void over doorway. Keyes Associates
to follow-up.
Generator — The installation of the temporary generator at QMS was approved by the
electrical inspector. Chairman commented that rental fee is very high ($1700 per month).
Arrangements for a permanent set-up should be coordinated immediately. TCI is
currently working on specs. Walter Parker and Steve Chmura to review best location to
install permanent generator. Committee members recommended that the generator be
installed in the rear of building at QMS.
Heat Detector/Crawl Space — The Fire Dept. needs to review a plan of the crawl space.
Cafeteria -Expansion joint is installed.
Thanks — John Moretti, Superintendent, offered thanks to the MBC for their "Herculean"
efforts in coordinating the essential construction work to open QMS & MHS on time for
September.
Windows — Issue re: water penetration noted in the South Wing. Ken Gay questioned
completion of caulking of new windows. David Young stated observation of ceiling tiles
soaked approx. 6" around each tile from outside cleaning company. Chris Sullivan to
follow up.
Heat Trace — Question was raised as to the cause of varying water temperatures.
Installation was correct. Discussion was that problem could be caused by bad heat trace
or a cross connection. Plumbing Inspector issued only a 14 -day permit. Issue must be
addressed A.S.A.P. to avoid a withdrawal of the Temporary Occupancy Permit.
SMI/Notice to Proceed — Chris Sullivan requested a copy of SMI's Notice to Proceed.
Bill Moore's notes indicated that the MBC asked that Town Manager issue Notice to
Proceed. Copy should be forwarded to Attorney Conn's office for litigation.
Locker room walls — Project Coordinator questioned whether paint contractor should
paint locker room walls. Decision was to paint walls. MBC agreed to cover cost.
Freezer Ramp — Chris Sullivan noted that specs do not indicate ramp. Issue to be
reviewed by Crabtree McGrath. Chris Sullivan to follow up.
( Lockers — Linc Lynch informed the MBC that a number of lockers (approx. 20) were
removed by the School Dept. and moved to Robinson Elementary School. Lockers were
installed in the Extension to D terminal.
Project Coordinator Responsibilities — MBC Chairman, Rich Feeley commented re: a
brief discussion with Paul Letendre regarding assuming responsibilities of Project
Coordinator at QMS due to resignation of Christina Sullivan. Work hours could be
extended for 4 month period (or end of QMS contract) to include working week days
until 5:30 PM as well as some Saturdays. The MBC will discuss additional
responsibilities and compensation for QMS with Paul Letendre at the next MBC Meeting.
Chairman will discuss issue with Town Manager prior to meeting with Mr. Letendre.
MBC to send letter to Town Manager recommending an amendment be made to Mr.
Letendre's contract to include the additional responsibilities.
MANSFIELD HIGH SCHOOL

missing. New ones will be ordered and paid for by JTC.
Site Lighting — David Young stressed the need to install as soon as possible even if
temporary lighting is needed.
