Change Order Proposals — Jim May presented Change Order Proposals for MBC
review/approval.
COP #39 Ref. Deleted Wood Doors — Timeframe was too late to cancel the door order.
Richard Feeley asked that door frames and hardware from Eagle Supplies be delivered to
Walter Parker, Buildings & Grounds Dept. to store for future use. Credit in the amount of
$1,071.00. Mr. Feeley noted that the Town is due an additional credit to reflect the cost of
installation. Jim May to follow-up.
• COP #40 Ref. Additional Electrical Service — Panels were originally overlooked by TCI. Jim
May reported that three new panels have been installed to accommodate need for the existing
service. TCI supports the installation of service. Richard Feeley questioned pricing
indicated from subcontractor. Jim May to request further breakdown.
Existing Bldg./Phase 3A - Jim May reported that a meeting was held with the Contractor and
School Principal to discuss science rooms that could be turned over to J.T. Callahan. Jim May to
send a memo to JTC specifying the rooms agreed upon by the School Dept.
South Wing — Jim May stated that the South Wing classrooms would be turned over October. 18,
1999.
Construction Schedule — Project Coordinator commented that very little work has been
completed in the cafeteria and library since Labor Day. Jim May expressed frustration in JTC
pushing back completion dates for occupancy in cafeteria from Sept. 14 to Sept. 18, 1999. As of
Friday (Sept. 24"), the completion date for the cafeteria and library is now Oct. 11, 1999.
Library — Occupancy anticipated on October 11, 1999.
Acceleration — Discussion opened Re: C.O.P. #171 ref. Acceleration, that JTC has not completed
the areas promised in their original acceleration proposal. Richard Feeley reiterated that without
the kitchen equipment, JTC put the Town at risk to receive SBAB funding. JTC to forward a
written response to the MBC substantiating the $15,000.00 acceleration expense.
Schedule Review — Chairman requested that Jim May perform a comprehensive review of the
MHS construction schedule. Project Coordinator cited examples of deficiencies, i.e. elevator
installation = 1 day work, site lighting included on schedule, etc. Michael Callahan commented
that an updated schedule would be provided to Jim May. Mr. May will review and provide a
written evaluation to the MBC.
Completion Work — Walter Parker requested that JTC be required to finish areas completely
prior to turning over to School Dept. Discussion was that JTC has unfinished work in several
areas that are now occupied. Concern is that the work will not be completed. MBC agreed.
Project Coordinator to monitor.
Kitchen — Inspection passed. Kitchen was turned over to School Dept. today.
Transition strip is needed from kitchen to cafeteria.
Elevator — Issue re: height of truss in elevator - Based on response from Thyssen Elevator and
Jim May's experience, a waiver was not applicable. The Structural Engineer is looking into how
to modify the truss. To prevent a potential delay of issuing the temporary occupancy permit,
Linc Lynch will forward letter from School Principal confirming that there are no wheelchair
students enrolled at MHS.
Doors - Delivery of hollow metal doors is still pending. Temporary doors that were installed are
missing hardware.
Site Li hg ting — Walter Parker reported that the School is receiving calls from parents, with safety
concerns, due to the lack of site lighting in parking lots. Light poles arrived today. Still waiting
for fixtures. Chairman asked for confirmation that count includes fourteen fixtures, in addition
to the two fixtures added by addendum. Michael Callahan to confirm. MBC questioned why
trenching and sono tube installation had not yet begun. Response was that JTC was working in
other areas. JTC agreed to install temporary lighting on Wednesday, Sept. 29, 1999.
Painting — JTC still waiting for a bond from painting contractor.
Abatement — Asbestos abatement completed based on a cost per square foot. If there is
additional removal required, an additional cost would be submitted in a Change Order Proposal.
Roofing Issue — An allowance was carried in General Bids to include removal and installation of
roof decking at MHS. Upon investigation, the work was found to be unnecessary. JTC was
asked to provide credit for roof work. Michael Callahan commented that JTC made an error in
their General Bid in the value of unit pricing listed for roof work. Mr. Callahan stated that the
$12.00 per sq. ft. indicated for installation of decking should have been listed at $2.00 per square
foot. The $14.00 listed for removal of gypsum decking should have been $4.00 per sq. ft. Jim
May stated that both numbers quoted by Michael Callahan were less than the two other general
bids received for the same work. Chairman Feeley referred to past history whereby the Town
was required to pay JTC without question a substantial cost based on the unit price amount
written in specifications. Jim May requested to provide memo summarizing other bids. Issue
pending.
Damaged Library Books — Chairman questioned JTC as to status of the Insurance Claim for
$30,000 damage to library books caused by JTC. Marc Jaoude commented that paperwork was
forwarded to the insurance company. Michael Callahan to contact insurance company and
follow-up. Linc Lynch commented that the delay of this claim is starting to impact school
programming.
Parking lot Paving/Stripping — David Young questioned status of completion work. Mr. Young
commented that at existing walkways, the line work needs to be replaced.
Fire Hydrants — Project Coordinator noted comment from the Fire Chief that two fire hydrants on
site near East St. are graded too low. Fire Department must be able to rotate valve completely
around in the event of an emergency. JTC to follow-up.
Fencin — Walter Parker noted that he had stopped contractors installing the chain link fence at
the rear of track (near pole vault pit area). Mr. Parker informed them that a vehicle gate was
needed at that location. A 12 -foot gate is required at field hockey field as well. Project
Coordinator noted that the wrong gage pipe was installed initially. Galvanized pipe has been
inserted in the center of each pole for reinforcement.
Field Committee — Line Lynch reported that a movement was established to improve and expand
athletic facilities in Town of Mansfield. Keyes Associates was asked to submit schematics,
working drawings, and project management for the job. A meeting will be scheduled with
Selectmen, Fields Committee and MBC. Hope is that SBAB will include field expansion
expenses as part of the School Reimbursement. Mr. Lynch noted that the burden is on the Town
to prove that areas are critical to athletics.
MOTION to authorize Keyes Associates to release typography re: field areas.
JL/KG Passes 5-0
JTC/Change Order #2 — Jim May commented that Change Order No. 2 would be resubmitted to
include credits for asbestos removal, painting and previously approved COP'S. Jim May noted
that he would be on vacation for the next two weeks. Mr. May will review Application for
Payment this Friday. Provided that all paperwork is in order, Jim Burson will sign and forward
to JTC for signature.
QUALTERS MIDDLE SCHOOL
Landscapin¢ — David Young pointed out that the original landscape plan indicates a heavy
concentration of landscaping at front entrance of QMS (where new generator is to be installed).
David Young requested that Wallace Floyd Associates re-evaluate the landscaping plan. Mr.
Young also suggested that the four trees recently planted at the front bus loop be moved from
under existing trees to a more baron area where old entrance road was located. Question was
raised re: status of trees to be installed on islands in parking lot (MHS). Response was that fall is
not the best time to dig and replant the types of trees specified.
Guidance/Nurses Area — Contractor is currently finishing up work. Inspections to be scheduled
at the end of this week. Walter Parker will arrange for final cleaning over the week -end.
Shop —Completion of wood shop area is pending. Project Coordinator expects that the area
would not be available for occupancy for at least another 2 weeks.
Gym — Sanding of floor is complete. Floor to be sealed tomorrow. Issue raised by School Dept.
re: markings planned for gym floor. Specifications indicate that a shop drawing submittal is to
be submitted. Submittal has not been provided. Project Coordinator to follow-up.
Gym Floor Addition — Linc Lynch commented that the School Dept. would prefer that the wood
flooring in the new gym addition not be installed. The School Dept. has agreed to provide and
install a non-skid flooring material in place of wood. Keyes Assoc. will request credit from JTC
for wood flooring (approximately 54 ft. x 25 ft.) for an estimated credit of $10,000. Mr. Parker
commented that the prep work would still be required by JTC. Concrete grinding needs to be
done in some areas.
Gradin Ig ssue — Issue raised Re: installation of unit ventilator near new Sped Room (to the left of
basement stairs). Height to cut louver would be below grade 3ft. David Young asked to review
proposed fix. Keyes Assoc. to investigate further.
Home Economics — Project Coordinator noted little progress to date. Painting to begin today or
tomorrow. Ceiling grid is in. Washer/dryer hook-up is complete. MBC noted that this was the
first room turned over for demo. Richard Feeley asked Paul Letendre to push progress of work
in this area.
Paintin - Project Coordinator commented that the painter had offered to paint walls in locker
room if MBC paid for paint. Committee members agreed.
Heat Trace — TCI visited QMS site on Saturday, Sept. 25, 1999. Test was run with water leaving
boiler room at 120 degrees to evaluate what was attainable at end of line. Result was that water
temperature reached 115 degrees, but not consistently. TCI's opinion was that the heat trace was
not working properly. In order for the heat trace to activate properly, temperature leaving boiler
room should be set at 125 degrees. This will allow the faucet at lavatory to reach 110 degrees
and the lab sink to reach 120 degrees.
Certified Payrolls Keyes to verify certified payroll for sheet metal contractor.
Change Order #13 — The following COP's were presented for review/approval to the MBC as

Vogel, to evaluate the most current construction schedule and provide a written comprehensive
