Ramp Li hg tine — Keyes was questioned as to the status of lighting at front ramp. Issue re: Opening for light fixtures at ramp are too large for fixtures. Keyes to issue directive to JTC based on options to fill in or put in flange. Question was raised as to whether the grade of ramp would require handicap landings. Steve Chmura to follow-up. Generator — Project Coordinator stated that according to Michael Callahan, a 60KV generator unit was ordered by JTC. Committee requested that Steve Chmura obtain written confirmation from JTC that generator has been ordered. Toilets — Re: handicap issue, the MBC suggested that unit ventilators be retro -fit similar to other bathrooms at Qualters. Water Fountains- Height of (2) drinking fountain locations do not comply with handicap access code. Modifications are necessary to meet code requirements. Response to RFPs — Chairman questioned correspondence from JTC regarding Keyes' non- response to RFPs. Paul Letendre's stated opinion that JTC's communication to Keyes was unjustified. Project Coordinator explained that a meeting was scheduled to discuss electrical and mechanical issues. Sometime after the meeting was planned, JTC raised objection that the architect (Keyes) should be present to respond to RFI's. (Keyes was not asked to attend.) Mr. Letendre will forward written response to JTC. Construction Consulting - Steve MacCaffrie expressed concern that his payment was not included for installation of doors/hardware in COP#171 ref. Acceleration. Chairman noted that the invoice for Construction Consulting was included in COP 171A and already paid to J.T. Callahan. Lock sets — Several issues outstanding. Mr. Chmura raised concern that various lock sets may have been installed in the wrong doors. Doors - Mr. MacCaffrie indicated that several doors are not reusable due to their interior rotted condition. Steve Chmura responded that Keyes has sent a letter to JTC requesting cost proposal for new doors. Mr. Chmura to follow up regarding status. Keyes to generate letter to JTC that they will be held responsible for added costs incurred should JTC fail to act promptly with regard to ordering new doors. David Young asked the Project Coordinator to generate a punch list to include an inventory of door closure conditions throughout the school. Heat Trace — Manufacturer's representative will be arriving in our area on Nov. 5, 1999. Project Coordinator to notify MBC when representative will be on site. Construction Schedule — Ongoing discussion that J.T. Callahan has not provided accurate up to date construction schedules, yet they continue to reference delays incurred to the project. Previous discrepancies in JTC's schedule have included number of days to complete project longer than contract, omission of site work, performing phased work out of sequence. Committee questioned Keyes as to the status of the written construction schedule evaluation that was requested in an MBC letter to Keyes dated 9/27/99. Keyes not finished with review yet. David Young commented that the evaluation must be reviewed by Tuesday. MBC needs written documentation immediately to respond to JTC's correspondence in a timely fashion. Manpower - Project Coordinator noted that JTC's manpower at both job sites has been very limited. Chairman asked that Paul Letendre document daily manpower in a memo, and send notification to JTC any day that the job is not staffed properly. Library-- Project Coordinator commented that progress on the library is slow due to limited manpower working in this area. David Young commented that the exterior entrance must be addressed before occupancy permit will be granted. Grading — Issue discovered during excavation regarding not enough coverage on the water main. Keyes to provide sketch to JTC. Steve Chmura to follow up. Gym — Project Coordinator stated that all life safety issues have been addressed in the gym. Waiting for stamped affidavit to be issued by Keyes Associates before any use of the gym. Locker rooms are not available as yet. Ceiling grid is installed in vestibule outside of gym. Tile to be installed. Gym Floor Credit — Walter Parker asked for clarification re: conditional credit for gym floor. Chairman clarified that the credit was not conditional. The MBC approved the School Department's previous request to delete gym flooring in the new gym addition as long as the School Dept. provides and installs alternative flooring material. It was agreed that J.T. Callahan would still be required to do prep work as provided by contract. Keyes has directed JTC to provide credit. MBC will ensure that a reasonable credit is accepted. Underground Oil Line — Bill Callahan stated that while excavating for exterior lighting, one of JTC's operator's hit "blue plastic" material. Ken Gay noted that the "blue plastic" being referred to is part of the oil leak detection system to the underground oil line. The blue material protects the oil filled pipe therefore contamination outside the pipe is unlikely. Walter Parker obtained a quote from TMC Services Inc. to repair and reinstall the line. Keyes Associates to follow up. JTC to be back charged. F Elevator — Phones have been installed in cabs. Question was raised as to whether alarm rings directly to police station. Paul Letendre will follow up. Issue of Fire Protection within the elevator is not resolved. Keyes to issue letter to JTC directing them to finalize and complete necessary requirements and call for inspection. Roof— Paul Letendre has requested original warranty. FAX copy of warranty indicated no punch list. Concerns were raised by Walter Parker re: an incomplete area that called for tapered insulation, roof drain plugged and flashing bent. Keyes to issue letter to JTC rejecting the roof warranty due to the roof drain not working properly. Auditorium — JTC has been directed to pursue ordering auditorium seating. JTC/Application for Payment — Steve Chmura commented that JTC provided the back up included with the application for payment to him only a couple hours prior to tonight's MBC Meeting. In a quick review, Mr. Chmura objected to the complete billing for carpentry at 100% and discrepancy in the total amount included for COP #171A/B ref. Acceleration. MBC Chairman returned JTC's October requisition and asked that Keyes thoroughly review and resubmit for approval at a later date. Site Lighting — Chairman indicated that site lighting must be in by November 15, 1999 Special Town Meeting to be held at MHS. Paul Letendre noted that ship date is expected this week. Waiting for fixtures to be delivered. Wood Shop — Abatement work is scheduled for tonight. Project Coordinator commented that APEX is not performing duct work. Occupancy is expected by November 22, 1999. Keyes Outstanding Invoices — Keyes questioned as to status of payment. Project Coordinator noted conversation with Universal Engineering expressing reluctance to send hygienist to site until payment is received from Keyes. MANSFIELD HIGH SCHOOL JTC/Application for Payment — The JTC Application for Payment #10 was reviewed by Wallace Floyd and submitted to MBC for approval. Jim May noted that Release of Liens and Change Order #2 are included. Committee questioned status of construction schedule evaluation requested from WFDG on 9/27/99. Mr. May noted that he is waiting for JTC to provide an up to date schedule. MOTION to recommend payment to JTC in the amount of $641,225.59 for work performed at the Mansfield High School for the month of October, 1999. Amount includes Change Order #2 previously approved. DY/JL: Passes 5-0. Ball field Credit — Waiting for credit from JTC. Jim May noted that ball field credit will be reflected in Change Order #3. Estimated credit for gravel is $15.00 per yard. Change Order Proposal #40 Ref. 3-100Amp, 2Phase, 4Wr, 120Panel. Jim May presented proposal for wiring in the amount of $26,784.00. Ken Gay commented that pricing seemed reasonable. Question was raised as to ability to use existing service within the building. Suggestion to obtain conditional approval. MOTION to recommend approval of Change Order Proposal 940 in the amount of $26,784.00 contingent upon Paul Letendre and Jim May contacting Bob Wedge TCI to explore retrofit of existing electric heat breaker panel. Paul Letendre and Jim May are authorized to move forward with recommended change. Amendment to include Paul Letendre and Jim May negotiating installation of only the breakers needed. DY/KG: Passes 5-0. Elevator — Variance verbally denied. Elevator truss needs to be modified to create 2 hr. rated ceiling. Keyes will do structural calculations. Stamped affidavit from Architect is required. South Addition — Temporary Occupancy Permit has been issued. Paul Letendre confirmed with JTC that occupancy would not occur until punch list items are addressed. Jim May to send follow-up letter to JTC. Regarding punch list items, the School Dept. was asked to compile a list of any outstanding invoices owed by JTC. Roof Credit — In response to unit costs for gypsum deck removal and metal deck replacement, JTC claimed substantial error was made in pricing (more than one year after close of general bid). Roofer's work remaining includes metal work at 2 additions and auditorium (79%). JTC has been paid 100% of roof work based on their numbers. MBC questioned whether to continue paying roofer. Jim May to obtain opinion from Attorney General's Office and respond to MBC. Landscape — Committee expressed concern regarding quality and warranty if landscaping is installed this Fall. Project Coordinator's opinion was that landscaping would not occur until Spring, 2000. Request was made to install a plastic air curtain ($225.00) Issues to be forwarded to School Dept. for review. Salad bar - Shop drawings indicated sliding doors on front serving side. Apparently Crabtree McGrath deleted sliding doors leaving open access to thermostat controls. (Finished apron is on back side.) Suggestion was made to order sliding doors for front of unit. Jim May to verify intentions from Bob McGrath. Damaged Library Books — Original cost estimate to replace books damaged by JTC was $30,000. Estimate returned from the Insurance Company is $14,677.57 plus 23 hours. Linc Lynch noted that a meeting is scheduled on site Wednesday at 1:00 PM to discuss value of damaged books.
