Room last week. Issue raised that unit needs to be vented. Keyes to investigate what is

Handrails — Handrails as installed do not meet Fire code. No more that 1/4" gap is allowed
between walls and railing. Chairman informed Michael Callahan that an Occupancy
Permit would not be issued by the Building Inspector until the railings are corrected.
Electrical Closet — Project Coordinator reported a doorframe found in electrical closet
was padded out on room side. In location where sinks were removed, floor penetrations
need to be filled in. There is no floor the is in place where cabinet unit heaters were
removed.
Surplus Equipment — Michael Callahan noted a request from school staff that left over
equipment be given to the School Dept.
Gymnasium —Question was raised regarding the status of emergency lighting in the gym.
Steve Chmura commented that some emergency lighting existed previously.
Kitchen — Steve Chmura noted six outlets not yet installed. Regarding the leaking drains
from vat, issue seems to be resolved.
Tree Stump — David Young questioned the status of tree stump removal. Michael
Callahan claimed that the Town cut the tree down to install a light, therefore JTC does
not own stump removal.
Original Punch List — Steve Chmura commented that JTC has not completed original
punch list items. Mr. Chmura estimated Contractor to be about 75% complete with
original list.
Final Amended Punch List — Chairman expressed frustration that Keyes has not
completed and/or provided an amended final monetized punch list to JTC based on the
May 27, 2000 walk through. Steve Chmura commented that Keyes needs to verify some
issues. List is expected to be complete by Wednesday, June 7, 2000. Chairman
reiterated that JTC could not complete the outstanding work without this list.
Aluminum Storefront Doors — Doors have not yet been turned over to Town. Michael
Callahan noted that doors are on site at JTC. JTC will turn over to the Town.
Drainage Issues — Regarding ongoing drainage problems near Library, Keyes noted that
an additional catch basin is to be installed.
Swale at Parking Lot — Paul commented that JTC needs to remove two hay bales at
retention area.
Auditorium Lift Hand Rails — Installation is incomplete. Michael Callahan estimated that
handrails would not be ready until close to June 17, 2000.
Gym Floor lines — Keyes confirmed that the crooked lines on gym floor pointed out
during MBC walkthrough was removable adhesive tape and not painted.
Scoreboard — Installation is incomplete. Two power sources needed for operation.
Vented Base Cove — Installation is incomplete. Keyes received submittal from JTC
(COP#313 Ref. Installation of Cove Base - $2,100.00). JTC proposed credit of $600 for
maple trim and $200 labor. MBC disputed cost of credit offered by JTC stating that the
cost of 1" x 4" maple trim far exceeds cost of rubber cove base.
Recessed Mats - School Dept. requested JTC provide a submittal for recessed mats on
site. Michael Callahan to follow up.
Ramp Li h ting - Lens covers are installed. Lights need to be filled to be weather tight.
Discussion was for JTC to place silicone on 3 sides and install stainless steel frames.
Concrete Walk — Walkway into Library is incomplete.

release of $125,000.00 to keep job moving. MBC expressed concern about releasing
additional funds without complete punch list.
MOTION to recommend payment to JTC in the amount of $125,036.58 for work
performed during the month of April. Retainage reduced to 1% complete. Includes
Change Order Number 19. Chairman noted that CO#18 should be reflected in
Application for Payment. JUDY: Passes 5-0.
Operation & Maintenance Manuals — Steve Chmura commented that some are in. Walter
Parker requested that all manuals be turned over to the school as one complete package.
Window Conflict — Work rejected by owner. Still waiting for Keyes to follow up with
Cheviot. Previous option to correct issue is not possible. Window could not be removed.
Mr. Vogel indicated that a revised mock up would be ready in 2 weeks (June 20, 2000).
Committee expressed concern that the current installation creates a critical maintenance
issue if a window is broken. School Dept. will not be able to replace window. Chairman
stressed priority to have issue resolved.
Keyes Manpower — Chairman commented that the level of manpower provided by Keyes
is insufficient. Manpower has remained unchanged even after repeated requests by MBC
to increase man-hours. Chairman again asked Bob Vogel for Keyes Associates to assign
additional staff and/or man-hours to complete the QMS Project.
Expansion Joint/Cafeteria — Installation is unacceptable. Walter Parker commented that
there should be enough material on site for JTC to properly repair expansion joint.
