QUALTERS MIDDLE SCHOOL
Window Conflict — All members present walked through approximately 20 classrooms to review
conditions of improper window installation. MBC had hoped to visit every room and reach an
agreement with Keyes Assoc. and Cheviot Corp. as to how each window conflict would be
resolved. Keyes had originally stated that they would cover the cost to correct all windows
impeded by columns or end walls. During the walk through it was found that the total number of
windows impeded by columns or end walls, far exceeded the number of units Keyes had
anticipated correcting. The Municipal Building Committee rejected the limited fix proposed by
Keyes/Cheviot. MBC proposed that all lower windows be made operable and upper window
panels left locked or fixed in place. Keyes to verify uniformity throughout school. Bob Vogel
and Larry Wilcox to review conditions in remaining classrooms and prepare response to MBC
with a plan to correct conflict issue.
Administration/Guidance — Bill Maher noted concern that the heating units were drained by
McLaughlin Mechanical this week. Principal wants to ensure the units are functional again in
the fall when cool weather returns.
AC Unit — AC has not been operable since installation May, 2000. Contractor has several issues
with McLaughlin Mechanical with regard to incomplete contract work and punch list work. Jim
Liddick questioned proper protocol for the Town to supplement forces.
Completion Schedule - Bob Vogel stated that a 7 -day notice was issued (via certified mail) to
Michael Callahan. MBC commented that JTC's response to 7 -day notice is not acceptable,
noting several outstanding issues i.e. AC at Administration, Auditorium floor finish, Expansion
joint at Cafeteria, UV -13, work to relocate Kiln to second floor and Paving ($60K line item) etc.
MBC directed Bob Vogel to issue letter to John T. Callahan, III (certified mail) rejecting JTC's
response to the 7 -day notice previously issued. David Young asked Mr. Vogel to also follow up
issue via phone after letter is sent. Chairman commented that no schedule has been provided by
JTC showing when the required work would be performed i.e. Handicap ramp to parking on
West side to be reformed and poured etc.
Punch List — MBC noted numerous old issues that JTC has still not addressed
Emergency Lighting at Gym — School Dept. raised issue regarding lack of emergency lighting in
the gym. Emergency lighting is required by code for any existing place of assembly. MBC
Chairman asked Bob Vogel to follow up with TCI as to what is required. This work needs to be
completed prior to start of school in September.
Handicap access — Project Coordinator commented that the current grading at the curb is
incorrect to meet ADA requirements. Sidewalks need to be corrected per Building Inspector
prior to issuance of an occupancy permit.
Application for Payment — Steve Chmura reported meeting with Michael Callahan this week.
Mr. Chmura noted that Michael Callahan has refused to sign Revised CO #20 (including COP
numbers 117R, 218, 229B, 249, 273, 309 and 313 - total amount of $7,375.00). Mr. Callahan
requests that the MBC reconsider the original close out agreement ($25,000.00) made to exclude
a few Change Order Proposals specified by him (i.e. roof drain issue and patch of ceiling paint at
gym). MBC unanimously rejected reconsideration of the close out agreement made with
Michael Callahan on June 6, 2000.
MBC directed Steve Chmura not to process any payment requisition until CO#20 is included.
Montle Plumbine — Steve Chmura noted outstanding work issues with Montle Plumbing. Mr.
Chmura stated that Montle Plumbing has not received response regarding a pending direct
payment claim. MBC to check for receipt of direct payment claim.
MANSFIELD HIGH SCHOOL
Locker Room Change Proposal — Revised preliminary cost estimate for proposed changes to
locker room is now up to $230,173.23. David Young suggested that work be re -bid next spring.
MBC Chairman to meet with School Superintendent and Town Manager. Issue was raised that
ADA and life safety (sprinkler) requirements must be met prior to opening of school. Based on
discussion, suggestion was made to have Project Coordinator advise Wallace Floyd to direct
Contractor to proceed with a minimum scope of contract work.
Lockers —New gym lockers are currently being stored on site. Discussion was raised as to
whether to hold off installation in anticipation of future renovations or have Contractor install
lockers in boy's locker room per contract. Based on the timeframe (over one year), future
installation could be a problem with missing parts or assembly. School Dept. and MBC agreed
to have Contractor be directed to install units now per contract.
Non-Performance/Johnson Controls — Critical issue raised regarding Johnson Control's lack of
performance on site. Project Coordinator commented that representatives from Johnson Controls
have been non-responsive to numerous phone calls requesting them on site to perform work.
Paul Letendre stated that the HVAC work must be done prior to opening of school.
Seismic at Gym — Pending issue regarding TCI's interpretation of seismic work required per
code. Paul Letendre noted that Nick Riccio would review issue with the State Inspector and
verify if a suggestion to deduct value of new addition is possible to reduce the amount of seismic
work required.
Construction Schedule —Project Coordinator commented that turnover of rooms at the high
school is proceeding well.
Painting Lockers — Contractor disputes work. MBC asked Paul Letendre to verify that WFDG
issue notice to Commercial Painting directing them to paint lockers per spec.
Handicap Access — Project Coordinator noted conversation with Building Inspector that
condition of handicap access ramp near Auditorium would be accepted with variance.
Safety Concerns — Rich Feeley raised concern re: construction trailers creating a safety hazard in
obstructing view of children running from rear field in between trailers to parking lot. Trailer
storage area needs to be fenced. Paul Letendre to issue notice and follow-up with JTC.
Landscaping — David Young asked Paul Letendre to request the Landscape Architect to address
the condition of crabgrass on site and tree plantings on parking lot islands.
Elevator Inspection — Project Coordinator reported that a 30 -day temporary permit was issued
this week noting only a few minor issues that need to be addressed.
