MANSFIELD HIGH SCHOOL
Manpower/Johnson Control — Issue re: Johnson Controls not staffing job to perform necessary
contract work. As a result, of numerous phone calls made to Johnson Controls (over the past
several weeks) requesting manpower be provided, Paul Letendre noted that three workers were
working on site that day. Johnson Controls' lack of performance has delayed resolving AC unit
issues in the Administration area which has been inoperable since May, 2000.
Handicap Access Issues:
• Drinking Fountains — Independent Associates raised issues this week that drinking fountains
at existing location must be made handicap accessible per code. Sue Michael Brown stated
that this item was not included in the drawings and noted that the cost to make adjustments to
every station could be substantial. Ms. Brown proposed alternative options to reduce added
costs by installing fountains in only critical areas (i.e. gymnasium and auditorium) and have
remaining fountains eliminated or pursued for a variance to meet code. WFDG opinion was
that a variance would not likely be granted because the issue does not create a hardship. Sue
Michael Brown to issue bulletin to JTC to remove existing recessed drinking fountains in
gym and surface mounted fountains in corridors. All fountains to be replaced with new
handicap accessible units at the same locations and patching of CMU wall. Rich Feeley
asked Michael Callahan to prepare cost proposal using unit pricing to make the necessary
adjustments at each fountain location. Only replacement of gym fountains are definite.
• Toilet/Gang Bath — Michel Callahan noted that a variance is needed due to lack of space
availability.
• Toilets/Second floor — Current conditions in west wing have acceptable handicap toilet
access however the existing building area does not. WFDG to investigate further.
Seismic Issue — Project Coordinator reported that the Building Inspector has not yet reviewed
Bruce Davies' interpretation of the code with the State Inspector. Limited timeframe remains to
perform seismic work in the gym prior to opening of school. JTC needs a response as to how to
proceed as soon as possible. Phil Ricci plans to test immediately whether a clean removal of
only one block (located at the bottom flange of the beam) is possible to install seismic clip. Rich
Feeley commented that the Auditorium ceiling is has limited access to perform work. Chairman
will contact Bob Vogel to review possible relief of added seismic work in gym and auditorium.
A substantial added cost would be incurred by the Town for additional demo., if seismic is
required in the gym and/or Auditorium. Auditorium work is owned in contract.
Paint Dispute — Sue Michael Brown reported that the paint arbitrator (Dick Coughlin of North
Scituate) is currently gathering information from Wallace Floyd and JTC with regard to work
items in dispute. Commercial Painting is performing work on site.
Transformer on Auditorium Stage - TCI previously stated that the transformer at Auditorium
stage had to be removed. TCI was to issue drawing to remove unit per School Dept. request.
This was not done. Sue Michael Brown commented that after further review, TCI now says that
the unit has proper clearance and is acceptable to remain in place. Project Coordinator noted
comment from the Building Inspector, saying the transformer must be removed anyway.
Waiting for written response from TCI.
Specialty Handicap Toilet — Due to space clearance, a custom door panel installation is needed to
allow handicap access. David Young questioned Project Coordinator about status of work. Paul
Letendre reported that the cost of the partition panel is $540.00. The cost of remaining necessary
door hardware was priced under $200.00. Paul Letendre plans to perform installation on
Saturday, August 26, 2000.
MOTION to proceed with hardware purchase and installation required for handicap toilet.
Work to be performed by Letendre Company.
JL/WM: Passes 4-0.
Punch List — Project Coordinator walked MHS site on Saturday and has prepared a partial punch
list. Chairman recognized Project Coordinator's efforts noting later that he unknowingly
supplanted Wallace Floyd's responsibility to verify Contractor's work.
Locker Room Changes — A meeting was held with Town Manager, School Superintendent,
Building Inspector, Fire Dept and Project Coordinator. Rich Feeley stated that the immediate
goal is to have JTC perform minimum work required to obtain occupancy. Committee members
agreed with Wallace Floyd's suggestion to proceed with all life safety work per original design.
Sue Michael Brown will provide JTC with a plan of existing locker room and issue bulletin to
proceed with sprinkler work using larger size pipe to address future needs. Project Coordinator
commented that limited work would be needed to address ADA requirements. JTC needs to
issue credit for labor.
Gym Lockers - Sue Michael Brown to clarify discrepancy in the number of gym lockers ordered
and/on site. MBC directed Wallace Floyd to issue bulletin to JTC that all lockers on site be
assembled and installed per contract. It was agreed that any gym lockers not being installed
should still be assembled by the contractor and turned over to the School Dept.
Metal Plaque — Size of dedication plaque reduced so $3,400.00 cost should be reduced to
$1,200.00. Credit Change Order to be issued.
Construction Schedule — Michael Callahan commented that the hold on work in locker room area
and issue with Science Room sinks has caused delays to the project. MBC Chairman and David
Young responded based on the work performed to date, excluding issues in the locker room and
Science Labs, JTC would not be even close to the completion by the Contract. JTC requested to
supply two-week schedules at this point.
Wheelchair Lift — Power to new lift in Auditorium not shown on drawings. About a $2,000.00
Change Order.
Temporary Occupancy — Issue raised that emergency lighting in gym could hold up occupancy
permit. Project Coordinator to follow-up with Tom Heron at TCI Re: status of drawings.
Curb cuts on East Entrance — Phil Ricci commented re: area near loading dock in student parking
area that JTC waiting for direction from Keyes regarding handicap assess requirements.
Handicap access is required per code but not shown on plans. Project Coordinator to follow up.
Immediate response is needed.
Backflow Preventor — Walter Parker reported that Inspector Connie Lajoie, is requiring that
backflow preventors be installed in the Science Labs. Concern was raised with hoses connected
to faucet creating siphoning of toxins that could get into potable water system. Mr. Parker noted
that some of the new classrooms and science rooms already have backflow preventors. Vacuum
breakers could also be used to prevent problems. John Moretti to discuss planning to discuss
issue with MBC Chairman. Rich Feeley asked Sue Michael Brown to ask TCI to review and
respond with interpretation of code.

QUALTERS MIDDLE SCHOOL
Project Completion - Rich Feeley expressed the Committee's frustration that JTC has not met
project schedule. Jim Liddick suggested a date be set to officially close out the QMS Project.
JTC needs to submit a final list of COP'S to Keyes' for review. David Young requested that
Michael Callahan review completion status of Punch List, initial each item done and respond to
MBC.
AC at Administration — AC Unit still inoperable to date. Walter Parker read letter to
McLaughlin Mechanical from Project Manager, Keith Korell (Johnson Controls). Mr. Korell
referenced a manufacturer's recommendation to use glycol in the system that would prevent coils
from freezing. Mr. Korell claims that the coils broke because glycol was not used. Johnson
Controls requested submittals from McLaughlin for review. Rich Feeley commented that a
"recommendation" does not alleviate Johnson Controls' responsibility for the unit being
inoperable. School Dept. has been more than patient with the lack of AC and disruption in work
space over the summer months. Michael Callahan agreed to follow up with McLaughlin
Mechanical. A $1,400.00 School Dept. laser printer was damaged by the leaking AC Unit.
Change Order #20 — Rich Feeley questioned Michael Callahan as to the status of approval. JTC
responded that revised Change Order #20 in the amount of $7,375.00 has not been signed due to
other outstanding JTC issues in dispute.
f - Direct Payment Request —
• Construction Consulting — Rich Feeley questioned Michael Callahan as to the status of the
outstanding claim submitted by Construction Consulting. Michael Callahan agreed to follow
up with completion status of his punch list work and respond to Steven MacCaffrie who has
threatened to take the Town to small -claims court.
Montle Plumbing — Michael Callahan commented that Montle Plumbing is suing JTC over
cost of roof drains. Chairman stated that roof drains were required per contract. A no cost
change was made in the field to use materials supplied by Premier Roofing rather those to be
supplied by Montle. Montle Plumbing performed installation only. Mr. Callahan implied
that Montle's cost of materials was incurred and should be charged to Owner. Location of
any roof drain materials from Montle was unknown. Chairman responded that if Montle
continues to pursue this cost change, the Town's position would remain that Christina
Sullivan was not authorized to negotiate any cost changes for this item.
