McCarter, Secretary
Attendees: Lincoln Lynch, School Business Manager
Sue Michael Brown, Wallace Floyd Design Group

Paul Letendre, Project Coordinator
Meeting commenced at 7:15 PM.
MANSFIELD HIGH SCHOOL
Gym Locker Room - Sue Michael Brown has issued a bulletin to JTC to proceed with the
minimum amount of work required to meet code requirements in the gym locker room.
Due to the work involved to construct the girl's handicap shower, Paul Letendre will
discuss possible time extension with building inspector. Linc Lynch to verify gender of
any incoming handicap student(s).
Handicap Toilets/Second Floor — Independent Associates apparently expressed opinion to
the Building Inspector that the existing handicap bathrooms on the 2"d floor are too far
away from non -handicap toilets. Sue Michael Brown confirmed that bathrooms must be
handicap accessible to meet building code. Paul Letendre to discuss receiving a waiver
for this particular bathroom with Town Building Inspector. If waiver is denied, added
cost would be incurred by the Town to renovate two existing non -handicap bathrooms on
the 2nd floor. Chairman directed Sue Michael Brown to issue sketch to JTC and request
cost proposal be submitted.
Existing Transformer/Auditorium Stage — Previous directive from local Mechanical
Inspector called for removal of the existing transformer on the Auditorium stage due to
future non-use. Removal of this unit was not included in contract and would have been
an added cost to the Town. Earlier this week TCI issued a letter to the inspector stating
that the existing transformer does meet NEC working space requirements and, therefore,
is not required by code to be removed. The inspector is satisfied with TCI's response and
has agreed that the transformer can remain in place.
Backflow Preventors at Science Labs — Issue raised by Mechanical Inspector re: hoses
attached to faucets in Science rooms having the ability to siphon toxins into the potable
water system. Per Inspector's request, the School Dept. is proceeding with installation
of backflow preventors and vacuum breakers to prevent potential safety hazard and
satisfy inspectors requirements.
- JTC/Application for Payment #20 — Payment Requisition was not submitted to MBC for
w, approval. Sue Michael Brown noted that a pencil requisition was reviewed earlier that
day.
Change Order Proposals — Review/Approval
COP #213 Ref. Power for wheelchair lift - $2,006.00.
Status: Pending. Corrective work is currently under TCI review.
Bulletin 9184 Ref. Handicap Ramp Repairs — $4,133.93.
Status: Pending. Sue Michael Brown noted a discrepancy between
drawings with regard to proper footage required for curb cuts.
WFDG position is that since the Contractor did not issue RFI to
clarify length of cut required, that he should be held responsible to
correct work.
Missing Lockers — Approximately 30 lockers planned for a corridor of the existing
building were not delivered per contract. Chairman asked Linc Lynch to obtain direction
from Superintendent as to whether to pursue credit of $3,500.00 offered by JTC or have
contractor provide lockers per specifications.
Seismic/Gym — Additional seismic work is on hold, pending the outcome of calculations
by Project Coordinator and WFDG. If the amount of renovation work at the high school
is less than 50 percent of overall value of the building, then additional seismic would not
be required by Building Inspector. Wallace Floyd needs to submit letter to Building
Inspector confirming final calculation. Paul Letendre noted that should the additional
work be required, JTC does not anticipate a problem with removal of only one block vs.
three blocks previously discussed.
Emergency Generator - Walter Parker reported existing emergency generator surged
during testing last week. Project Coordinator noted that the FM Generator technician
reported there is too much load on L-1 phase. JRW claims the correction is not contract
work. Chairman asked Sue Michael Brown to contact Bob Wedge at TCI for
recommendation.
Construction Schedule — Project Coordinator reported that work is proceeding well.
Band, Music areas may be complete by the end of this week. JTC will then proceed with
work in the Auditorium. Paul Letendre has reviewed approximately 85% of additional
work completed (since previous punch list was issued) and is working to generate a
second punch list. Mr. Letendre noted that the majority of punch list work was painting,
most of which is now complete. Mechanical punch list is also complete.
Landscaping — School Dept. supplemented JTC's forces prior to opening school by
pulling out weeds etc. MBC members reiterated that crabgrass has taken over lawns. Re-
seeding is needed. Paul Letendre to follow up with Landscape Architect.
- - Exterior Lighting — Rich Feeley questioned Linc Lynch as to whether lights in the rear of
MHS/QMS are on a time clock. Area does not appear to be well lit in the early evening.
Linc Lynch to follow up.
Elevator - Project Coordinator commented that the 30 -day temporary permit expires next
week. Punch List items are now complete. Contractor has called for re -inspection.
QUALTERS MIDDLE SCHOOL
Office Move — Steve Chmura announced that Keyes has moved their Providence Office
to 121 Main Street in Mansfield. New telephone number is (508) 339-9855.
Punch List — Steve Chmura performed walkthrough on Thursday, August 31, 2000.
Michael Callahan was not present as previously planned. JTC forwarded documentation
to Keyes last week contesting a variety of issues i.e. painting auditorium floor, roof
drains etc. MBC is ready to pursue close out of project and move to monetized punch list.
David Young asked Steve Chmura to review JTC's issues and bring two lists for
discussion at next week's MBC meeting; 1) Punch list items contested but owned by
Town per contract and 2) Contested items but possibly negotiable.
Direct Payment Requests — JTC not present to discuss status.
• Construction Consulting — Waiting for Michael Callahan to confirm completion of
punch list work.
• McLaughlin Mechanical — Chairman noted that majority of items identified to be paid
were approved and paid to JTC in Change Order #16. Waiting for Michael Callahan
to confirm payment to subcontractor.
Ramp Light Fixtures — Installation of fixtures around front ramp lights is incomplete.
This item is included on Punch List.
Expansion Joint — Repair of rejected work is incomplete. Project Coordinator
commented that the value for this item is listed on Punch List at $14,000.00.
Lighting/Music Room — Steve Chmura noted that JTC has offered a $2,000.00 credit for
installation of seven lights in the music room owned per contract. Linc Lynch to
investigate and respond to MBC as to whether School Dept. can arrange to install lights.
Issue pending.
Carpet Damage at Elevator — Paul Letendre reported that the damage to carpet at elevator
was caused by sprinkler subcontractor while performing work in this area. Carpet must
be replaced.
Ceiling Speaker in Gym — Contractor left site without installing the gym ceiling speaker
because a lift was not available. Installer is still holding speaker. Members commented
that the Contractor is responsible for installation and coordination of a lift on site. Due to
the difficulty in accessing speaker and to avoid future warranty issues, Rich Feeley
encouraged the School Dept. NOT to accept ownership of the gym speaker or its
installation.
Graphic Panel — JTC has installed the glass panel cover as required per code. Steve
Chmura noted that the existing graphic map needs to be updated per Fire Inspector.
Steve Chmura to provide updated plan prior to issuance of final Occupancy Permit.
Egress to Courtyard — Keyes discussed issue raised by the Fire Dept. objecting to egress
of students from a 2"d floor classroom to courtyard area due to a violation of code. Steve
Chmura noted that this same plan was approved by the Fire Inspector for the opening of
school Sept. 1999. Project Coordinator suggested that doors at courtyard be made to
swing out of building direction. Keyes to review issue further. Keyes needs to send
response to Building Inspector indicating a means of egress that meets code
requirements.
Emergency Lighting/Gym — TCI has issued drawing. Line Lynch noted that Walter
Parker is coordinating purchase and installation of acceptable lighting to meet inspectors
requirements.
Window Conflict — Bob Vogel was not present to provide update. Rich Feeley asked that
Keyes and Cheviot work to have information ready for next Tuesday meeting of MBC
(Sept. 12, 2000).
JTC/Change Order #20 — Still waiting for JTC approval.
AC Unit at Admin — Johnson Controls has replaced damaged coils and indicated that
their contract work is complete. Rich Feeley asked Steve Chmura to verify whether the
unit installed is an equal to what was specified in contract or the exact same unit.
Chairman noted that the unit in specs does not mandate that glycol be used to operate
system. Discussion was that adding glycol to the system would decrease efficiency of the
compressor. Ken Gay commented on personal experience with a similar situation in
Carver, MA having the heat coil too close to the freeze -stat. The unit in Carver was fixed
by relocating the freeze -stat and installing a pressure gage. MBC asked Keyes to verify
operation of unit in Carver as a sufficient reference to pursue Mr. Gay's suggestion.
Suggestion was made to operate unit without AC unit during the interim. Keyes to
review and obtain direction from TCI as to how to proceed. Issue pending.
O&M's —Walter Parker needs Operation and Maintenance Manuals ASAP. Chairman
stated a transmittal must be provided with manuals. Steve Chmura stated that Keyes
would provide information to Mr. Parker by Thursday, September 7, 2000. Steve Chmura
noted that JTC also owes dollar value on various warranties. Keyes to provide summary
to MBC prior to next Tuesday's meeting.
OTHER
Custodial Work — MBC Members shared comments with Linc Lynch regarding the
exceptional work performed by school custodial staff at MHS and QMS prior to opening
of school.
SMI Litigation — MBC received recent correspondence from legal counsel requesting
conference call with Town Manager and Rich Feeley to discuss litigation and settlement
value. Pre-trial was originally set for September 25, 2000, however, it was mutually
agreed by both parties that a motion would be filed to continue the pre-trial to October
30, 2000.
Meeting adjourned at 9:30 PM
Respectfully submitted
Eleanor McCarter, Secretary
