The Town of Mansfield invites the submission of qualifications by responsible firms having the
ability to perform Construction Testing & Inspection Services for the Town of Mansfield; this is a
qualifications -based selection process. The Town may qualify multiple firms to provide these
services.
All submissions, to be considered, must be prepared in accordance with the requirements specified
in this Request for Qualifications and in accordance with applicable provisions of Massachusetts
General Laws, Chapter 7. Note if submissions are not in the format as specified in the document
they will be considered non-responsive.
The fees are to be negotiated with the firm(s) having the highest ranked proposals. The fee proposal
is to be placed in a separate sealed envelope bearing the title "Pee Proposal — Construction Testing_
& Inspection Services" and included with the proposal.

Up" for free, or login with your existing account. Enter Project Code 815951 in the project locator
box. Select "Acquire Documents" to download bid documents, review a hard copy at Projectdoes
physical location, or request a free project CD. It is the sole responsibility of the contractor,
subcontractor, vendor, individual and/or corporation to review all ADDENDA prior to bid
opening at www.Projectdog.com or at Projectdog's physical location. Questions regarding the

Eight (8) hard copies of all Submission Requirements must be submitted to

Submissions are due on or before November 10, 2016 at 10:OOam. Any submission received after
this time and date will not be considered and will be returned to the Respondent unopened. The
clock in the Town Clerks office shall represent the official time for purposes of this determination
and all submissions shall be delivered to the Town Clerks office by the above stated date and time.
All inquiries regarding the information contained in this Request for Qualifications shall be directed
to the Owner's Project Manager, Michael Ahern. All questions must be submitted in writing and
sent via certified mail or equivalent. The deadline for questions will be 10:OOAM on November 4,
2016.
The Town of Mansfield reserves the right to waive any informality in the submissions, to reject any
or all submissions, or to accept any submission which it deems to be in the best interest of the Town
of Mansfield.
Submissions are to be sealed and properly identified on the outer envelope as Qualifications for
Construction Testing and Inspection Services. Rate proposals -are to be in a separate sealed
envelope, appropriately marked.
