HomeMy WebLinkAboutHouse Doctor Services Schools_RFQ Nov 2016Town of Mansfield Request for Qualifications House Doctor Services RECEIVED TOWN CLERKS OFFICE AN OCI -5 P 12* 01 MANSFIELD MASSACHUSETTS The Town of Mansfield invites the submission of qualifications by responsible firms having the ability to perform Architectural, Engineering, Mapping and Surveying Services for Various Municipal and School Buildings and Other Projects on an As Needed Basis (HoHse Doctor Services) in the Town of Mansfield; this is a qualifications -based selection process. The Town may qualify multiple firms to provide these services. All submissions, to be considered, must be prepared in accordance with the requirements specified in this Request for Qualifications and in accordance with applicable provisions of Massachusetts General Laws, Chapter 7. Note if submissions are not in the format as specified in the document they will be considered non-responsive. The fees are to be negotiated with the firm(s) having the highest ranked proposals. The fee proposal is to be placed in a separate scaled envelope bearing the title "Pee Proposal — House Doctor Services" and included with the proposal. This project is being E -Hosted through Projectdog, Inc. Go to www.Projectdog.com and click "Sign Up" for free, or login with your existing account. Enter Project Code 815570 in the project locator box. Select "Acquire Documents" to download bid documents, review a hard copy at Projectdoes physical location, or request a free project CD. It is the sole responsibility of the contractor, subcontractor, vendor, individual and/or corporation to review all ADDENDA prior to bid opening at www.Projectdog.com or at Projectdog's physical location. Eight (8) hard copies of all Submission Requirements must be submitted to: Michael Ahern Town of Mansfield Town Clerk Office 6 Park Row Mansfield, MA 02048 Submissions are due on or before November 3, 2016 at 10:OOam. Any submission received after this time and date will not be considered and will be returned to the Respondent unopened. The clock in the Town Clerks office shall represent the official time for purposes of this determination and all submissions shall be delivered to the Town Clerks office by the above stated date and time. All inquiries regarding the information contained in this Request for Qualifications shall be directed to the Owner's Project Manager, Michael Ahern. All questions must be submitted in writing and sent via certified mail or equivalent. The deadline for questions will be I O:OOAM on October 27, 2016.
