MANSFIELD HIGH SCHOOL
Seven Day Notice - MBC previously asked that WFDG put together a "complete" monetized
Punch List and issue a Seven -day Notice to J.T. Callahan. Seven -day Notice has not been sent
due to all punch lists not yet fully monetized by WFDG and /or TCI.
Monetized Punch List — Total value of remaining work including retainage is about $225,000.00.
Sue Michael Brown provided monetized punch lists of several areas to the MBC for review
amounts. Ms. Brown indicated that she would not issue the final Punch List to JTC until the
MBC confirms that amounts assigned to each line item are acceptable. Overall Punch List value
could exceed the $225,000 value
Existing Building — Architectural Punch List was provided by WFDG. Total estimated value
is $24,250.00. This amount does not include value of MEP Punch List. Ms. Brown noted
that TCI needs to re- inspect work and provide up -to -date monetized punch list to WFDG.
JTC is refusing to provide letter to TCI verifying that work is complete. MBC advised Sue
Michael Brown not to wait any longer and to move forward and monetize TCI's "original"
Punch List for the Seven -day Notice to JTC.
• South Addition — Architectural Punch List provided was generated by Project Coordinator
and is monetized by WFDG. Total estimated value is $2,000.00.
• West Addition —Architectural Punch List provided was generated by Project Coordinator
and is monetized by WFDG. Total estimated value is $7,300.00.
• Locker Room — Architectural Punch List was provided by WFDG. Total estimated value of
architectural items is $11,565.00. The MEP Punch List (provided by TCI) is monetized.
Total dollar value of MEP Punch List was not available. Copy of this list to be given to
Walter Parker for comment.
• Auditorium, Band, Music, Toilet — Architectural Punch List was provided by WFDG. Total
estimated value of items included is $3,000.00.
• Landscaping — Sue Michael Brown advised the MBC that the Landscape Architect (WFDG)
is waiting until Spring to follow up on status of work. Vegetation needs to be re- inspected.
MBC asked Sue Michael Brown to monetize original Punch List prepared by Tamar
Zimmerman (WFDG) and include in Seven -day Notice package to JTC. The MBC pointed
out that the first day of Spring was March 20'x', assuming we were both talking about this
year.
• Exterior Site/Paving —Punch List is incomplete. Sue Michael Brown asked permission from
MBC to utilize services of Town Project Coordinator to review status of site work and
generate Exterior Punch List, (includes walks, curb cuts etc.). MBC agreed to this request.
Sue Michael Brown to contact Paul Letendre to coordinate site visit.
• Kitchen (Crabtree McGrath) —Kitchen Punch List was handled independently through
Crabtree McGrath Assoc. Status of Punch List items is unknown. Rich Feeley asked Sue
Michael Brown to monetize the original Punch List provided by Crabtree McGrath and
include Kitchen Punch List in Seven -day Notice package to JTC.
Change Order Proposal - Review /Approval
COP #280 Ref. Structural infill of CMU to underside of truss — $1,520.00. Work was
performed based on T &M. Walter Parker verified that work is complete.
Status: MOTION to approve COP 9280 as recommended by WFDG.
JL/WM: Passes 3 -0.
Locker Room Update — Sue Michael Brown reported that a walk - through of the locker room was
held this week with TCI, School Dept. and Wallace Floyd. Bid Documents are currently on
schedule for distribution in late April/Early May.
Guardrail at rear Parking Lot —MBC asked Sue Michael Brown to issue directive to JTC to
install guardrail per contract, whether or not access to new road is blocked. Issue was raised
regarding construction related debris in this area also needing to be removed.
Drinking Fountain Installation — David Young reported that the current installation of (3)
handicap drinking fountains in the gym is unacceptable. Walter Parker agreed that the masonry
around each fountain is loose fitting. Sue Michael Brown to send letter to JTC rejecting work.
Paving — MBC asked Sue Michael Brown send letter to JTC asking they follow up and respond
regarding previous paving concerns. JTC needs to check and verify grades at rear parking lot are
appropriate. MBC previously sent letter to JTC (dated 2- 28 -01) reporting that binder thickness
appears insufficient. JTC has not responded as to how this would be corrected. Contractor still
needs to provide some site signage. Chairman asked Ms. Brown to also obtain a date from JTC
as to when final paving would be scheduled.
QUALTERS MIDDLE SCHOOL
Change Order #21 — Includes single Change Order Proposal #321 for increased size of lintel
materials. Sue Michael Brown generated Change Order Proposal in the amount of $10,300.00
(excludes JTC mark up as previously agreed by Michael Callahan). Ron Furr needs to pick up
Change Order #21 from MBC Office to obtain JTC signature.
Existing Floor Grout — Previous issue reported that the level of tile grout in kitchen is too low
causing potential safety hazard as well as chipping and cracking of some floor tile. RLM
Flooring was to correct during February vacation. Sue Michael Brown reported that
representatives from RLM attempted to re -grout tile but determined that level of grout meets
industry standard. Sue Michael Brown to review grout work and ask JTC to forward a response
from Dal Tile, verifying whether level of grout meets specifications.
Unit Ventilators - McLaughlin Mechanical scheduled to start and test unit ventilators #13, #14,
15 and #16 during school vacation next week (April 13, 2001 to April 20, 2001). JTC needs to
follow up to see that this is done. Note: Buildings and Grounds Director will be on vacation
during this time.
LOCKER ROOM MODIFICATION PROJECT
Project Coordinator Responsibilities — School Building and Grounds Manager expressed interest
in fulfilling duties to oversee construction of locker room modification. He proposed to be
compensated for a weekly lump sum fee of $300.00. Chairman questioned whether there would
be enough funds to authorize the stipend proposed. This would rely on the cost of bids received
for this work. MBC asked Mr. Parker to review with School Department.
WATER TREATMENT PROJECT
Discussion was that the MBC would oversee Project (i.e. periodic updates by Water Dept.,
review change order approvals, and requisitions). Water Operations Department would work
with water consultants day -to -day to oversee actual construction of the facility.
FDMW FEASIBILITY STUDY — MBC Office to forward letter to all members requesting
final comments to be sent to CSS for inclusion in the Feasibility Study Report.
