MHS LOCKER ROOM RENOVATION PROJECT General Update -Sue Michael Brown introduced Connie Lai (WFDG Co- worker) to members of the MBC. Ms. Lai is working to complete the locker room plans with Ms. Brown. Structural Engineering - Sue Michael Brown advised MBC that the cost for structural engineering services was not included in the existing WFDG contract (dated April 9, 2001). Ms. Brown noted that WFDG would be generating structural plans with input from Davies Engineering. MBC supported additional compensation in the amount of $1,400.00 be paid to Davies Engineering for structural services as recommended by WFDG. Schedule - MBC asked Sue Michael Brown for a status update regarding the bid schedule for locker room renovation work. • Advertising - MBC suggested WFDG advertise 2 weeks for subcontractors and 1 additional week for General Contractor. Discussion was to advertise Filed Sub bids in the Central Register on May 16th and May 23rd, and General Contractor bids on June 6, 2001 in order to meet a September completion schedule. • Sue Michael Brown reported that bid plans should be ready to distribute by the beginning of June. • Sub Bid Opening - Bid opening is tentatively scheduled for June 6, 2001. Sue Michael Brown noted at this time both Mechanical and Electrical are expected to be filed sub bidders (having cost exceed $10,000). • General Contractor Bid - GC Bid opening is tentatively schedule for June 15, 2001. • Notice to Proceed - Anticipated date to issue Notice to Proceed is June 25, 2001. • Contract End Date - Per Sue Michael Brown the estimated date to complete locker room work is by the middle to the end of August, 2001. Wall Finish - Sue Michael Brown noted that the existing walls in locker rooms have glazed CMU and asked Committee members preference of wall finish for new walls. MBC advised to have walls painted with epoxy paint to lower cost. Metal Lockers — JTC has not installed the original locker room lockers per contract. School Department and MBC agreed to include 60 metal football lockers as an add alternate to the locker room bid. Final color of lockers is to be chosen by owner. Per School Dept. WFDG should not specify any additional lockers for locker room. MBC questioned what items (i.e. lockers, 8 doors or floor) could have a long lead -time for delivery. Walter Parker to check on lead -times from local vendors. Sue Michael Brown stated that there is to be no rubber base applied on locker bases in all team locker rooms per plans. Interior Signag —Walter Parker has reviewed signage plan with Sue Michael Brown as suggested by MBC. Discussion was to include the missing signage within locker room project bid. Grout/Kitchen — School Dept. previously reported that level of tile grout appears too low. MBC asked Sue Michael Brown to review the installation of ceramic tile grout located in the kitchen. WFDG opinion is that the level of grout meets standard installation requirements. MBC asked Sue Michael Brown to obtain written verification from Dal Tile. Monetized Punch List — JTC subcontractors have received monetized punch list. Subcontractors need to sign off on completed work prior to final inspection. Value exceeds amount remaining in contract. Seven Day Notice — WFDG to issue notice to JTC to complete all contract and punch list work by May 31, 2001. MBC asked Sue Michael Brown to check status of J.T. Callalian's plan to complete all remaining work. Payine — MBC to issue letter to JTC rejecting recent paving work at rear parking lot and demand a response regarding installation of insufficient paving binder. Suggestion was made by Mike Ahern, Special Projects Manager Town of Mansfield, that if JTC does not provide response, Town should contact an independent contractor to do 8 core samples of binder course. Committee did not feel doing core samples would be necessary since the areas that are broken up are obviously thinner than specified per contract. MBC has physical samples showing binder thickness installed was approximately 1/2 inch to 3/4 inch. Boys Locker Room — New doors installed. Still waiting for JTC to install automatic openers. Site Work — JTC proposing additional cost of $5,000 to move handicap curbs, replace 2 curb cuts, and replace sidewalk due to conflict in details shown. MBC maintains contractor owns work and does not authorize further compensation of any concrete work. ADARequirements — Walkthrough held Friday, May 20, 2001. Waiting for final report from Independent Living Associates and Building Inspector. Guardrail Installation — Installation of guardrail at rear parking lot is complete. Length of guardrail installed is insufficient. Guardrail was installed to left of light -pole. Per drawings, length should be 125 feet, which is located East of existing light -pole. Suggestion was made to have JTC add two lengths of guardrail to protect light -pole and protect steep drop -off at asphalt. Full sweeping and installation of pavement tack coat is required. Per Mike Ahern, DPW will sweep parking lot. Drinking Fountains — Masonry work around fountains located in gym was rejected. Sue Michael Brown reported another drinking fountain near Classroom No. 115 is also unacceptable. WFDG to issue letter to JTC stating that based upon further inspection by WFDG, a number of corridor fountains are rejected and with an assigned a dollar value for unacceptable work. JTC /Change Order #25 - Sue Michael Brown presented JTC Change Order #25 to the MBC for review and approval. Change Order includes previously approved COP #280 Ref. QUALTERS MIDDLE SCHOOL Occupancy Permit — Rich Feeley reported that Mike Ahern paved the handicap ramp needed at the gym entrance of QMS to meet handicap requirements. Per previous conversation with Building Inspector, this was the final requirement necessary to obtain a Final Occupancy Permit. WFDG to ask JTC to request inspection for Final Occupancy Permit. Unit Ventilators — Air Conditioning units are still not working properly. McLaughlin Mechanical is apparently refusing to perform work needed due to non - payment issues with JTC. Per Walter Parker's conversation with Dick Carney (TCI), there is no freezestat for cooling side of both units. Controls are included in specifications. JTC /Change Order #21— Change Order is not signed by J.T. Callahan. COP #314R. Additional cost for Lintel material $10,300.00. JTC resubmitted COP to include mark -up increasing total to $11,824.00.
