MANSFIELD HIGH SCHOOL Infill Stairway #6 — Previous issue raised regarding structural integrity of Stairway #6 infill located at the South Addition. Walter Parker reported that JTC was on site recently and did some welding in this area to provide additional support. Structural Engineer needs to review and verify repair is acceptable or issue detail to correct as needed. To date Bruce Davey (Davey Associates) has refused to review this issue until his firm receives confirmation of additional compensation to be paid for their service. MBC members agreed to send a letter to Davey Associates authorizing payment of up to $500.00 for engineering services necessary. Per Walter Parker, extra floor tile is in stock. Contractor is scheduled to repair cracked floor tiles during December school vacation. Drain issue at Girl's Locker Room — Inadequate As -Built Drawings contributed to problems in locating the drain cleanout in this area. Walter Parker coordinated exposing drain clean out to find New England Flooring had poured excess floor material in shower drain which caused a blockage. Blocked drain problem is resolved. New England floor agreed to be responsible for cost to replace floor top coat. MBC agreed that JTC should cover cost of time incurred by School Building and Grounds Manager to resolve this issue. Electrical Issues — Walter Parker noted concern regarding several electrical issues that remain incomplete by the subcontractor (JRW). Rich Feeley referred to list provided by Building Inspector that noted only a limited amount of work remained. This list was issued to JTC as part of the Final Inspection Report. As of July 31, 2001, Punch List value for electrical was $750.00. MBC advised Walter Parker to forward letter to JTC directing work be completed by Friday, December 7, 2001. Doors/Hardware Girls Locker Room — Walter Parker reported having problems with door locks. MBC advised School Dept. hire someone to fix locks vs. waiting for contractor. Door 13 111 — Walter Parker reported the right side door in Girl's team locker room is missing. JTC needs to provide. Door 131E also located in Girl's Team Locker Room, was not provided. This is included in Change Order #26 in the amount.of $2,437.00 not yet approved. MBC asked Walter Parker to notify JTC to install this week. COP #24 /Seismic Credit — Value of credit due to the Town remains in dispute. ( Pump Station — Walter Parker reported outstanding issue regarding poor ventilation in pump station that needs to be corrected. Walter Parker to coordinate necessary work to correct issue. Generator — School Dept. continues to rent temporary unit. MBC is working with School Dept. to replace existing generator from School Project Account. TCI provided bid documents earlier this week for MBC review. Scope of work includes purchase of one new Generator, wiring, transfer switch and new panel. Discussion was as follows: Coordination - David Young noted several items show no coordination between trades within the contract documents. TCI should note coordination required in contract documents. Fuel - MBC questioned TCI's choice to use diesel fuel. Existing genset is gas. MBC recommends use of natural gas, not diesel, Location — David Young noted that the new generator location recommended by TCI near auditorium entrance is directly under an outside air vent (vent location was not shown on original drawings). MBC recommends new generator be located near loading dock where temporary genset is now located. Existing generator located inside building is to remain in place at this time • Alternates — MBC recommends that existing Alternate 1 be only option. • Transfer Switch - Needs new transfer switch. 1 SOM • Exterior Site Work —MBC states exterior site work needs to be shown, including underground oil tank. • Enclosure - Drywall enclosure needs to be done ivith proper fire rating. Four bollards should be shown. • Filed Sub- Bidders —Question was raised as to why six Filed Sub - bidders are listed on documents. MBC suggested documents are boiler plate and six "Filed" sub- bidders would not be needed. Electrical trade is listed to be General Contractor. • Bid Documents — TCI to include Copies of Plans and Specifications available for pick up at MBC Office at no cost or sent to requestor by mail for a fee of $25.00. • Proposed Bid Schedule Documents to be made available by December 18, 2001 Pre -Bid Walk through to be held Thursday, December 27, 2001 Filed Sub Bids due either Tuesday, January, 15th or 22, 2002 MBC to make 12 copies of bid documents. • MBC needs to check with Town Counsel regarding wording about waiving fees for various permits. Weight Room —Walter Parker advised MBC that the current Storage Room in the Locker Room may be used as weight room in the future. MBC noted this would be a separate project from Locker room renovations and a separate building permit would be needed to convert storage room to weight room. WFDG/Remaining Scope of Work - At this time, MBC agreed Wallace Floyd would not need to attend any future MBC meetings regarding the school projects. The following scope of work needs to be completed by WFDG: High School. 1. Architectural Record Drawings — WFDG has in possession. Documents need to be reviewed, approved and returned to Owner. 2. O &M Manuals — JTC needs to revise and resubmit to WFDG for approval. 3. Delay Claim Schedule — WFDG received schedule from JTC on November 30, 2001. Jim Liddick suggested MBC get copy of this schedule and review it prior to WFDG sending any response to the General Contractor. 4. WFDG and TCI need to provide Final Affidavit for Locker room area. 5. Punch List — Unless JTC hires WFDG to re- inspect additional work completed, the value of the punch list will remain the same as submitted in Revised Change Order Proposal #26 in the amount of ($36,911.00). Quniters Middle School. 1. COP #322 — WFDG needs to process Change Order #322 for Fire Protection in Cafeteria performed on T &M. 2. Final Requisition — Based on advice from Town's Legal Counsel (Sue Jacobs), no further payment should be made to JTC until the pending litigation regarding the window conflict at QMS is resolved. WFDG needs to review final requisition only so MBC would have accurate records for future payment. Requisition should not be signed by Architect at this time. QUALTERS MIDDLE SCHOOL Window Conflict — MBC previously sent RFP's to Gale Associate and Wallace Floyd to provide a cost estimate to remove and replace newly installed windows in conflict with walls and /or partitions. Gale Associates has expressed interest. Wallace Floyd has not responded to date. David Young asked if Mr. Stewart agreed to be an expert witness for the Town. This question was not asked in original request for cost proposal. David Young suggested Ed Stewart (Gale Assoc.) contact Sue Jacobs to determine scope of work needed. Unit Ventilators #13, #14 #15 #16 — Town continues to hold money for this work until units are tested Spring, 2002 and accepted by the Town. Handicap Station at Home Economics — No change. MBC asked Walter Parker to hire someone to do this work. This needs to be complete to obtain Final Occupancy. MHS TRACK AND FIELD Project Budget - Walter Parker reported that the School Business Manager would provide an up- to -date budget summary to MBC next week. Track - David Young noted track is uneven on opposite right side. Contractor may have run out of asphalt at that point or possibly truck traffic delivering temporary bleachers. Mr. Young suggested Walter Parker review issue. Synthetic Field - Approximately 100 lbs. of additional rubber pellets remain on site and will be added to the synthetic field in Spring, 2002. Initial installation of rubber needs compacting before it would be able to accommodate remaining pellets.
