WATER TREATMENT FACILITY AND WELLS PROJECT General Update — MBC received a copy of the existing site plan from Woodard and Curran that day. MBC Secretary reported Doug Reed, CDM had visited the MBC Office earlier that day and reported Barry Giorgi (CDM) was working on a floor plan design of the proposed facility. Doug Reed noted plans to begin geo- technical survey work upon completion of the floor layout, possibly sometime next week. Copy to be forwarded to the WC. Vehicle Wash Station — A prior site visit to Norfolk Power Equipment in Wrentham, MA did not meet the Town's needs. MBC was still waiting for W &C to schedule a site visit to a more comparable wash station facility. FISHER- RICHARDSON HOUSE PROJECT Construction Meeting An unscheduled meeting was held in the MBC Office earlier that day (12/17/02) with James Bright Architect, Reza and Marzi Jahedi, ARJ Associates, Rich Feeley, MBC Chairman and Eleanor McCarter, MBC Secretary. Meeting discussion involved a status ( update of ongoing construction and ARJ's first submission of Application for Payment. l Project Updates — MBC Secretary reported the General Contractor had mobilized on -site December 20, 2002 and some work had begun. The MBC Secretary requested James Bright Architect forward a written project status report to the MBC Office for review. The Architect agreed to forward update reports on a weekly basis to the MBC Office for the duration of the Fisher- Richardson House Project. Site Security — MBC was contacted recently by ARJ Associates and notified that items belonging to the Contractor were missing (flashlight and fire extinguisher) from the site. In order to avoid potential security problems in the future, the MBC forwarded a letter to ARJ requesting the Contractor remove 2 existing door padlocks and furnish and install temporary lock(s) that would remain in use for the duration of the construction project. Four keys for temporary locks were to be provided to the MBC (one each for Fire & Police and two for NBC). Existing alarm system is to remain in use and activated when not on site. Per Reza Jahedi, locks were to be changed by Wednesday of that week. Inspection of Work At the 12/17/02 Construction Meeting, ARJ reported meeting with Tom Paske, Preservationist and Eric Gradoia, MASS -HC on site several times to review their progress i Gradoia were pleased with the of construction. Per Reza Jahedi ,both Tom Paske and Eric p Contractor's work to date. Contractor plans to request a site meeting with all parties prior to the installation of structural supports. Schedule Update: (Based upon discussion at the Construction Meeting held earlier that day (Dec. 17, 2002)). • Roofing— During a recent site visit by Eric Gradoia, MASS -HC, ARJ Associates was asked to prepare a sample of the roof shingles system for his approval. Per ARJ the sample would be ready for approval wle December 28, 2002. Reza Jahedi estimated a three day time -frame needed to install wood roof shingles • Material Storage — Wood shingles, paper, nails etc. were delivered on site. Materials were being stored inside the building on the first floor at that time. • Footings/basement — Contractor reported plans to install sonotubes in the basement on Friday, December 20, 2002. Due to potential drop in weather temperature below freezing, Tom Paske, Preservationist had approved use of an acceleration material for the concrete pour of footings. The Architect noted that the ground floor of the basement was not frozen at that time. • Chimney Re pointing —'ARJ was grinding out joints in the brick located above the chimney roofline. ARJ reported Eric Gradoia, MASS -HC had approved that work the week ending December 14, 2002.. Contractor plans to wait for warmer weather temperatures before completing pointing of the brick chimney. • Mortar Mix Tom Paske had satisfactorily responded to ARJ's previous questions regarding the "Town to provide sand" for mortar. James Bright noted that historic mortar is a softer material than modern day mortar, and was composed of sand, lime and concrete. The consistency of modern mortar would be too hard to be used with the softer ( brick found in historic homes. • Roof Turnbuckle — Rich Feeley questioned whether the turnbuckle used to pull the roofline together was installed. Reza Jahedi reported ARJ had recently clarified questions regarding the method of installation with Tom Paske, Preservationist and that the turnbuckle would be installed at floorboard height above second floor in attic space as well as at the ceiling beam. • Project Schedule — MBC questioned status of ARJ's revised Project Schedule. Reza Jahedi responded is was not available at that time. MBC was waiting for ARJ to forward a revised schedule. • Certified Payrolls — MBC Secretary reminded ARJ of need to submit certified payrolls to the MBC Office. Architect advised ARJ that the Town would need these in hand to verify wages prior releasing payment to the Contractor. Marzi Jahedi agreed to forward payroll documentation and a revised schedule to the MBC Office by the end of that week. • Photo Documentation - ARJ reported the construction photos taken to date using a disposable camera would be delivered to the MBC Office for photo processing when the roll was complete. Reza Jahedi commented he would e-mail additional digital photos of progress on site to the MBC Office. APPLICATION FOR PAYMENT — Review /Approval Review comments — Architect raised a previous concern to the MBC Office that the original dollar amount reflected in Contractor's General Conditions appeared to be high. Reza Jahedi (ARJ Assoc.) responded that he had included a number of smaller items in the General Conditions line item rather than using separate line items for each. (i.e. labor cost associated with metal fabrication, insurance, dumpster, toilet etc.). Based upon further clarification by ARJ, and submission of a revised requisition, James Bright Architect signed approval of ARJ's Application for Payment No. 1. MOTION to recommend approval of ARJ Associates /Application for Payment No. 1 i the amount of $36,142.75, for construction related to the Fisher - Richardson House Stabilization Project during the period ending December, 13, 2002. Release of payment to ARJ is pending MBC's receipt of Certified Payroll documentation. DY/WM: Passes 5 -0. JORDAN JACKSON ELEMENTARY SCHOOL Roof Repair Project • Upgraded warranty — Gale Associates had requested Atlas Roofing submit an upgraded warranty per the Roof Settlement Agreement. No further update was received on this issue. • Roof Snow Guards — MBC had not received "Invitation to Bid" information as previously discussed with Gale Associates.' QMS /MHS PROJECT CLOSE -OUT The MBC issued a previous memo to Walter Parker, School Bldg. & Grounds Dept. (dated 11/19/02 -cc: L. Lynch, J. Moretti) requesting he confirm the status of outstanding contract work at MHS and QMS, and also provide invoice back -up for any work completed by the School Dept. in order to back - charge the Contractor. MBC had indicated in that memo a desire to ensure the School Dept. was completely satisfied with the degree of contract work complete ( before releasing final payment to the General Contractor. Based upon the insufficient level of l detail provided by Mr. Parker in his response (memo dated 12/16/02) members unanimously agreed the School Projects could not be closed out at that time. QualtersMiddle School: • Fire Doors - Per Walter Parker's memo "Mae" holders for the fire doors at QMS were scheduled for repair. MBC did not receive a date for the scheduled door repairs or cost back up information on this item. This is a School Department issue. • Visual Alarms/Horns - MBC had agreed to follow up on a Final Occupancy Issue re: synchronization of Visual Alarms/Strobes. (Member Liddick suggested the MBC Secretary contact J &M Brown Co. of Jamaica Plain, MA to schedule a site visit to evaluate that issue.) Mansfield High School: • HVAC —MBC was holding a value of $10,000 for problems related to the HVAC unit located in the kitchen storage area. No response was provided by Mr. Parker on this item. • Electrical — MBC was holding a value of $15,000 for incomplete contract work. Walter Parker forwarded E -mail to Rich Feeley, MBC Chairman (sent 11/14/02) estimating the cost to correct outstanding electrical issues would be approximately $6,000, and to release $9,000 back to the General Contractor. No further information was received about this item. (i.e. itemized list of work completed by School Dept., invoice back -up, 4 etc.) l Direct Payment Claim —Rich Feeley noted a Demand for Direct Payment claim in the amount of $37,262.12, had not yet been withdrawn by JTC's subcontractor (General Mechanical). MBC decided to forward a letter to John Moretti, School Superintendent requesting his immediate assistance to follow up on this matter. Members agreed that if the School Dept. did not provide closure to the MBC Office regarding outstanding HVAC and electrical punch list issues by January 2003, all funds should be released to JTC, per a previous agreement made with the Contractor. School Furnishing Request — MBC received a letter from John Moretti (dated 12/16/02) asking the MBC consider an additional request to purchase school furnishings for QMS and NMS, with an estimated cost of $59,500.00 to be paid from the School Projects Account. The School Superintendent's request was rejected based on a previous discussion with Mr. Moretti (October 8,'2002 MBC Meeting) advising MBC to use funds remaining in the QMS Project to upgrade the current insufficient lighting at the front ramp entrance of the middle school. Cost to install additional lighting was unknown at this time. MBC needs to forward letter of response to School Superintendent. MBC was also waiting for a proposal from Gale Associates for this work.
