Representative Gary D'Ambra, Water Operations Manager Kurt Gaffney, DPW Director Lee Azinheira, and representatives from Hart Engineering, to review concern raised by MBC Member Ken Gay about the current grade level of the transformer and pad already installed. Bob Chapell reported that based on the assessment made on -site by the meeting participants noted above, all parties are satisfied with the installation of the transformer at the current grade level. MBC members noted a continued concern that if the storm drain is not maintained properly, a potential occurrence of overflows could result in damage to the transformer in the future. • Finish grading is not yet complete. Pipe Sleeves — Chairman Liddick opened discussion about continuous pipe sleeves required per specifications, which are not installed throughout the building. • The MBC originally brought this issue to Hart's attention at the Job Site Meeting held on March 9, 2005, whereby Hart Engineering acknowledged the sleeves were missing and indicated plans to install split pipe sleeves to correct the situation. • Chairman Liddick suggested a need for Hart Engineering to provide the MBC with more detail regarding their intention to install split pipe sleeves. Woodard & Curran agreed to follow up with the Contractor on this item and obtain this information. COP #11 Re: Additional Sprinklers • Hart Engineering submitted a formal claim to Woodard & Curran in the amount of $17,472.00 to furnish additional sprinklers in the attic in order to meet NFPA No. 13 code requirements (letter December 22, 2004). • Based on a letter to Hart Engineering from Bristol Fire Protection (dated Dec. 21, 2004), required sprinkler heads were not shown on the bid drawings, therefore not included in the Subcontractor's bid. Woodard & Curran's original response letter to Hart (dated Nov. 29, 2004) does not recommend additional payment for sprinklers due to the following reasons; • The Contractor is responsible for providing a sprinkler system that meets the requirements of NFPA No. 13. • There have been no changes to the bid documents that would warrant a change in the number of attic or other sprinklers. i • The shop drawings received from the Hart on November 1, 2004 complied with NFPA No. 13. • The Bid Documents indicated information such as the roof pitch, (5 in 12), building square footage, materials of construction and hazard group designations that allowed Hart to develop a design and corresponding bid price for meeting the requirements of NFPA No. 13 at the time of the Bid. Woodard & Curran confirmed that the Contractor filed the sprinkler claim within the 30 -day period allowed to do so per contract. It was also noted that Hart Engineering and /or Bristol Fire Protection did not submit a Request for Information to clarify concerns about the scope of work for this item prior to bidding on the Project. COP #14 Re: Additional Bollards • MBC received a copy of a letter to Hart Engineering from Woodard & Curran (dated March 25, 2005) confirming discussion with Hart at a job site meeting (held March 9, 2005) whereby Hart agrees to furnish and install five pipe bollards per a W &C sketch with no added cost to the project. COP #16 Re: Oil/Gas Interceptor • W &C needs to provide a copy of their original response to Hart Engineering rejecting this item. COP #18 Re: Request for Extension of Contract Time/Weather • MBC received a copy of a letter to Hart Engineering from Bob Chapell (dated March 25, 2005) indicating, based on W &C's review of the claim, Hart has not provided any supporting data with the claim to make a determination about the request. Based on review of the current schedule, Bob Chapell and the MBC agreed this claim has no merit because completion of the project is on schedule at this time. COP #19 Re: Furnish and install 500 Linear Feet of Permanent Fence. • Hart submitted a proposal in the amount of $17,535.00 to provide additional fencing at the project site. Hart's proposal matched a fence quote obtained previously by the Water Operations Division for this scope of work. W &C recommends approval of the cost proposal. COP #20 Re: Request for Extension of Contract Time/Emergency Generator • Hart Engineering submitted a Change Order Proposal to extend the length of the contract time with no associated cost, due to documentation from Fall River Electrical reporting a shipment delay of the Emergency Generator. • W &C recommends that the time extension only be approved for the work associated with installation of the standby generator and that the original substantial and final completion dates be maintained for the remainder of the work. • The MBC suggested to Kurt Gaffney that he prepare a letter to Hart Engineering stating the Contractor would be held responsible for all costs incurred to operate the facility in the event of an electrical failure. COP #21 Re: Add Radiant Heat Unit in Room A- 103/$1,330.44. • During a recent site visit, Kurt Gaffney and Sid Greenleaf identified a need to add a radiant heat unit to an exterior wall located near a front window in the administration area. • W &C recommends approval of this cost proposal. • Hart proceeded to install the pipe material to add the heat unit last week, based on the direction given to them on site. • W &C needs to contact Sid Greenleaf (AEI) and request an immediate inspection to ensure the pipe is installed properly. InControl may be required to provide an additional wire and control point for this unit. Motion to recommend approval of COP #21 in the amount of $1,330.44 to furnish and install a radiant heat unit in the administration area. Approval of the proposed cost is pending AEI's acceptance of the installation. R. Feeley/K. Gaffney; 5 -0 Passes. Approval is pending a recommendation from W &C and availability of funds once priority needs are satisfied. • Furnishings Allowance —Water Department anticipates incurring a cost of approximately $30,000 for office furnishings for the new building, which is currently not included in the project budget. The MBC discussed appropriating funds toward this item, once priority needs are satisfied. Hart Enaineerin"A plication for Payment No. 11— Review /Approval Pencil Requisition — The MBC received Hart's pencil requisition the day after the regular MBC meeting. Building Start up — With respect to a substantial completion date of May 15, 2005, Chairman Liddick emphasized a critical need for Hart to have a concise start up schedule. • Bob Chapell reported Woodard & Curran would require a one -week period to startup the Water Treatment Facility and complete testing. • Chairman Liddick expressed concern that an anticipated delay in filter piping may affect Hart's critical path of construction. MBC suggested Woodard & Curran follow up with Hart to ensure sufficient time is allowed to start up the unit. • W &C needs to obtain a start up schedule from Hart. Close -out Documents — Kurt Gaffney questioned when the Water Department could expect to receive the Project Operations Manual. Bob Chapell advised that the final Operation Manual would be provided when the project is officially closed out, however Equipment Manuals will be provided earlier at the time of building start up. Certificate of Occupancy (CFO) — Kurt Gaffney questioned at what point a CFO is issued. Bob Chapell responded that the CFO would be received pending final inspection approvals at the date of Substantial Completion May 15, 2005. Chairman Liddick stressed that in order to receive the CFO by this date that Hart Engineering needs to focus closely on the project schedule to ensure all critical path items are on track for completion. SCADA — Per Bob Chapell reported that system programming of the new SCADA computers is on track. Due to the insufficiency of existing computers, Bob Chapell requested permission from the MBC to all the new SCADA computers to be moved to Cate Springs this week for an interim period. W &C will move and set up the new computers in the new facility when construction of the building is complete. Mr. Chapell advised that setting up the computers now would activate the equipment's warranty period a couple months sooner than planned.
