WATER TREATMENT FACILITY PROJECT
Punch List Walk -Thru — A final punch list walk -thru was held on Thursday, July 14, 2005.
MBC received comments from the Public Buildings Manager, Mike Ahern as a result of the
walk -thru, however written results from the Project Engineer are not available at this time. Bob
Chapell indicated he is waiting to receive input from W &C's sub - consultants regarding
mechanical, electrical, plumbing and architectural items. Once their information is received,
Bob will consolidate the information into a final punch list for the MBC to review. Earlier that
day, Chairman Liddick requested Bob Chapell to immediately forward comments submitted by
Mike Ahern to Hart Engineering so they could begin addressing those items.
Status of Inspections
• Elec&ical — Final inspection is complete. Permanent Generator still needs to be installed
and inspected.
• Fire Protection — Final inspection is complete. One smoke device needs to be installed in
the front vestibule to meet fire code. This item was not shown on the drawings.
• Mechanical— Sid Greenleaf (AEI) did a partial mechanical inspection, however due to
volume of work incomplete at that time, an additional inspection is required.
• MBC questioned Bob Chapell as to why the HVAC system was not operational at
the time of the walk- through. No reason was provided.
• Mechanical systems (water and air) need to be tested and balanced by the
subcontractor.
• New England Piping needs to submit Final Balancing Reports to AEI for review
before Sid Greenleaf will return to the site to complete the final inspection for
MEP.
• Incontrol needs to test the controls component of the system.
• Lee Azinheira asked Bob Chapell to notify MBC member Ken Gay of the date
when the mechanical inspection is scheduled.
• Outstandinglnspections - Bob Chapell is anticipating the final plumbing, mechanical and
building inspections to be complete by the end of this week.
Substantial Completion — The date of substantial completion for the project has not yet been
determined.
Property Insurance — MBC will pursue insurance coverage with Mark Cuddy of Richardson -
Cuddy Insurance for new facility. Insurance will be taken over by the Town on the same day a
Certificate of Insurance is issued.
Generator Shipping Delay — The contractor provided a temporary back up generator on site as a
condition to obtain a Final signature of approval from the Electrical Inspector. Bob Chapell
noted that the permanent generator is due to be shipped from the manufacturer this week, and is
expected to be delivered on site by next week (w /e July 18 -22, 2005).
Dedication Plaque -As of this date, Bob Chapell had not responded to e-mail sent on May 31,
2005, advising of a few minor corrections needed to the text layout of the building dedication
plaque (copy to Nick Salina at CDM.). MBC continues to question Bob Chapell regarding the
status of the building dedication plaque.
• MBC asked Bob Chapell to provide a proof of the design layout submittal so the MBC
could confirm that the Town's text changes were incorporated correctly.
• Member Feeley urged Bob to follow up on this item quickly since dedication plaques
typically are a long lead -time item.
SCADA Start up schedule — Woodard & Curran is working with the Mansfield Water
Department to coordinate flow tests this week.
Proposed Change Orders — Status Update
COP #22 (Revised) Misc. Electrical /$12,118.85 —Hart Engineering was asked at a recent job site
meeting to re- evaluate the proposed cost, and to identify a more reasonable credit for installation
of single -phase wiring vs. three -phase used for the garage doors openers. Member Feeley
suggested the charges included for wiring were excessive and strongly objects to the proposed
amount billed. The MBC rejected the revised COP amount and directed Bob Chapell to return
the proposal to Hart Engineering.
Credit to delete (1) Gas solenoid valve /$256.32 — The proposed credit is based on the cost of the
solenoid valve plus %2 hour for labor. Based on his personal experience, Member Ken Gay
suggested that a half hour of labor would be insufficient to install a solenoid valve. MBC
requested Bob Chapell revise the proposed cost to reflect 3 hours for labor plus the cost of the
valve. Woodard & Curran needs to assign a COP number to this item.
Mechanical Inspection Fee — N.E. Piping submitted a proposed change order in the amount of
$4,200.00 for mechanical inspection fees. The subcontractor is not required to pay the Town's
portion of permit fees for this project, but is required to pay all "inspection" fees per contract.
MBC rejected COP #27. Bob Chapell needs to send a written response to the Contractor.
OTHER
Mediation Fee — The Town of Mansfield Water Department recently paid fees in the amount of
$1,080.00 to the American Arbitration Association (AAA) to participate in the upcoming
Mediation Hearing with Hart Engineering. Town Accountant was requested by Lee Azinheira to
monitor Town Counsel's monthly charges and transfer any costs related to the Water Treatment
Facility to the Water Department.
Mediation Hearin Strategy trategy Re: COP #3 and COP #11 - (John D'Agostino)
COP #3: Excavation & Removal of RockMaterial — Earth material excavated from the site does
not meet the definition of rock material in the contract documents. Earth material was not
measured properly prior to removal.
COP #11: Sprinkler design — MBC evaluated whether the proposed additional cost would be
valid since the sub - contractor followed W &C's drawing to identify scope of work.
• Bob Chapell stated that a preliminary sprinkler design was provided to the contractor
only to point out the hydraulic system and that the final design for the sprinkler system
per contact is the contractor's responsibility, not that of Woodard & Curran.
• Contractor was provided with the roof pitch dimension and should have calculated the
correct number of sprinkler heads required.
• MBC discussed a suggestion to pay for the cost of sprinkler material only in
consideration of the "value added" to the building, however no action was taken.
Ha "/A for Payment No. 14 — Bob Chapell reported the overall project is 92% complete
at this time. Hart submitted a payment request to the MBC in the amount of $153,324.71 for
construction services through June 30, 2005.
MBC members discussed an ongoing concern regarding the radiant floor tubing warranty:
• Radiant floor tubing was specified to be installed in the vehicle wash station and include
a 30 -year warranty. The manifold, fittings and ancillary items were specified with an 18-
month warranty.
• MBC expressed fiustration regarding Hart Engineering's lack of response to a prior
request to provide a letter from the manufacturer confirming warranty of the splice repair.
MBC wants the manufacturer to confirm the splice of the tubing material is acceptable
and would not void the manufacturer's tubing warranty.
• MBC discussed an holding back $50,000 until they receive a letter of response from the
manufacturer regarding this issue.
• The suggested value of $50,000 is based on the original contract value, plus an estimated
cost to remove and replace the concrete floor should that become necessary.
• Certified Payroll Records and a Project Schedule for the June time - period are on file.
MOTION to recommend a reduced payment to Hart Engineering/Application for Payment
#14 in the amount of $103,324.71 for construction services related to the Water Treatment
Facility through June 30, 2005. Payment is reduced by $50,000 until letter is received
regarding radiant floor tubing warranty. RF/WM: 5 -0 Passes.
Office Furnishings — WB Mason is scheduled to deliver and install a partial order for office
furnishings at the new facility on July 25th and July 26, 2005.
BMF Forms - Woodard & Curran needs to provide completed BMF Forms to the MBC for Town
Manager approval.
Woodard & Curran/Invoice No. 43552 — Water Treatment Facility Project
