Meeting commenced at 6:30 PM.
WATER TREATMENT FACILITY PROJECT
MBC members met at the new Water Treatment Facility located at 500B East Street in
Mansfield. The purpose of meeting on site was to review the current completion status of the
project.
Punch List — Bob Chapell distributed copies original punch lists based on the results of recent
final inspections performed mid -July 2005: Woodard & Curran (structural & sanitary); AEI
(mechanical, electrical, & plumbing inspection); CDM (architectural inspection); and a general
inspection by Public Buildings Manager Mike Ahern.
Woodard & Curran also generated a consolidated final punch list (dated August 2, 2005) and
distributed this list to the MBC for review and comment.
• Acting Chairman Feeley requested that Bob Chapell modify the consolidated Punch List
into two distinct lists to easier identify incomplete contract work from punch list work.
• Contract work is defined as work that should be completed in the original scope of work
within the timeframe allowed. Punch List work is defined as work complete, but not
accepted.
• Lee Azinheira requested that Bob Chapell verify the completion status of each item noted
on Mike Ahern's list, and to make sure W &C includes any outstanding items in the final
punch list issued to Hart.
Mechanical
• Ongoing problems with the mechanical system are not resolved. The system needs to be
tested and balanced.
• Certified Balancing Reports (water /air) need to be submitted to AEI for review.
• Member Ken Gay commented that the air - handling unit installed in the Mechanical room
does not appear elevated high enough off the floor to allow the condensate drain to
operate properly. All agreed this issue is critical and needs to be corrected. AEI needs to
respond to this issue.
Project Completion — MBC requested that Woodard & Curran send an official letter to Hart
Engineering putting them on notice regarding potential penalties, which may be imposed for the
incomplete contract work still outstanding past their contract end date of June 16, 2005. All
outstanding work must be completed immediately.
Painting — A significant amount of touch -up painting is still required.
Interior Building Signage - MBC received an update from Architect Nick Safina (CDM)
indicating that he received a re- submittal for building signage from Hart Engineering earlier that
day. The submittal is not yet reviewed or approved.
Inspections and Permits - The follow permits are approved: Electrical, Mechanical,
Architectural, Structural, and Sanitary.
Substantial Completion — The MBC does not accept the Water Treatment Facility project to be
substantially complete at this time.
As -Built Drawings — As -Built Drawings in paper and digital format are included in Hart's
contract. Hart needs to provide As -Built Drawings in paper format to Woodard & Curran. Based
on the paper As -Built Drawings provided by Hart, Woodard & Curran would then provide As-
Built Drawings in digital format and provide As -Built Drawings in both formats to the Owner.
Radiant Floor Tubing Warranty — In response to a concern regarding warranty, members
discussed requiring that Hart Engineering provide to a bond in the amount of $50K for a 5 -year
period to cover potential future repairs. Lee Azinheira suggested the MBC also require Hart to
modify two pipe elbows in order reduce potential problems with flow and heat loss. Resolution
of this matter remains under discussion.
Woodard & Curran — Engineer needs to follow up on the following responsibilities:
• Provide Architect's Affidavits to the Building Inspector.
• Provide As -Built Drawings in Digital Format to owner.
• Obtain Certificate of Compliance from Mansfield Conservation Commission a required for
final Certificate of Occupancy
• Transfer SCADA system to new facility.
• Complete start up training for electrical, mechanical, chemical analyzer, and submersible
pumps scheduled for later this week.
Provide Operation & Maintenance manuals to owner. Bob Chapell continues to receive
information from Hart.
Office Furnishings /WB Mason
• The MBC authorized spending up to $15,000 for office furnishings from the project budget
to date. Water Operations Manager Kurt Gaffney is handling the purchase and installation of
office furnishings needed.
• Kurt Gaffney reported that W.B. Mason damaged a small amount of floor tiles outside the
Chief Operator's Office while installing workstations in the administration area. The floor
subcontractor has since replaced the damaged tiles, but did not seal coat the tiles.
• W.B. Mason agreed they would pay for the cost of repair.
• Owner Kurt Gaffney is not satisfied with the installation of a workstation countertop and
intends to modify the furnishings order. Mr. Gaffney requested the MBC not process
payment to W.B. Mason for Invoice No. 294102/Furnishings; $12,327.30 and Invoice No.
294184/Installation fee: $2,672.70 until workmanship issues are resolved.
Meeting adjourned at 8:00 PM.
