MBC Member/Water Operations Manager Lee Azinheira, DPW Director Robert Chapell, Woodard & Curran Meeting commenced at 7:30 P.M. WATER TREATMENT FACILITY PROJECT Status Update - MBC members met with Bob Chapell to confirm the status of outstanding warranty items at the new water treatment facility located at 500B East Street. (Warranty lists dated June 29, 2006, revised July 20, 2006 and revised August 3, 2006). Hart Engineering was notified to address these issues within the one -year warranty period. Jim Ramos of Hart Engineering declined an invitation to attend this evening due to a prior commitment. A status update was received from Hart Engineering via electronic mail prior to this meeting. Outstanding Warranty Items: 1. Air Scour/Wash Blower trips dining backwash (warranty list 6- 29 -06) Hart Engineering (Response Aug. 8, 2006): Fall River Electric confirmed to Jim Ramos that the combination starter's thermal overload was set too low as shipped from the factory, as they did not take into account motor's service factor. This was adjusted accordingly. DPW/Water Dent: System backwashes are performed on a weekly basis. There are no recurring problems at this time. Status: Closed item. 2. IR rinse valves failed to open during "Auto" regeneration (warranty list 6- 29 -06) • Hart Engineering: (Response Aug. 15, 2006) Valve failure is determined via limit switch feed back. Limit switch's can come out of adjustment due to flow /mechanical causes. Limit switch adjustment is standard maintenance. Switches were tested and verified during start up. • DPW /Water Dent.: Water Treatment Foreman indicates that this issue is still a problem that needs to be resolved by Hart Engineering and /or Roberts Filter. • Woodard & Curran: Bob Chapell disagrees with Hart's statement that the limit switches for the IR rinse valves came out of adjustment due to flow /mechanical causes for these reasons; • The IR rinse valves have not been used since startup in August of 2005, and questions how the valves could come out of adjustment when not being used. • During a test of the intermittent regeneration program in May 2006, Woodard & Curran observed that all of the IR rinse valves failed to open under "Auto" operation mode. In order to confirm that the problem was not a "sticky" valve, at that time, each of the IR rinse valves were manually exercised. When exercised manually all of the valves moved to their correct position without any alarms. W &C then completed a follow -up test of the IR rinse valves in "Auto" operation, which resulted in the same failure of the valves. • Status: Open item. Roberts Filter needs to address this issue. 3. Provide tttah* to owner suntntarizing steps and valves that should be opened or closed during "Auto "regeneration (warranty list 6- 29 -06) • Hart Engineering: Jim Ramos requested Roberts Filter to provide a matrix per contract. • DPW /Water Dept.: Water Treatment Foreman indicates that the valves have never opened during "Auto" regeneration. • Status: Open item. Still waiting to receive matrix from Roberts Filter. Hart needs to forward the matrix to Woodard & Curran upon arrival. 4. Filter Cotatrol Pattel program shotld riot allows perntartgaitate to enter the filter until after receiving pertttissfve front Main Conh -ol Panel that vatdt valves are moved to con ect position (warrano, list 6- 29 -06). • MBC Discussion: MBC asked Woodard & Curran to confirm whether this is a change order item. • Hart En ing eering (Response Aug. 15, 2006): Per Roberts Filter, Roberts has no control of the vault valves. The subcontractor indicates that programming modifications can be made to incorporate this request, but would require Hart to provide a written request and Purchase Order to accomplish this task. • DPW /Water Dept. Comment: Water Treatment Foreman indicates that this issue is still a problem that needs to be resolved. • Woodard & Curran: Bob Chapell is aware that the Filter Control Panel (FCP) does not have direct control of the vault valves. W &C does not require the FCP to control the valves. • W &C's wants Roberts Filter confirm that the FCP waits for the permissive from the Main Control Panel (MCP) before proceeding with the IR sequence. This step is necessary to ensure that permanganate waste is directed to the equalization tank. • Bob Chapell reiterated that W &C is not requesting that the FCP be modified to control the vault valves. • Status: Open item. Roberts Filter needs to address this issue. S. Provide repairs1modications to NaOCIMetering Pump No. 1(warranty list 6- 29 -06) • MBC Discussion: The center chlorine metering pump has been leaking at fitting and tube below counter. • Hart En ineering (Response Aug. 15, 2006): Hart Engineering hired LMI to furnish and install fittings and has since returned to correct any leaks found. • DPW /Water Dept.: A representative from Northeast Pump was on site 8 -14 -06 and replaced a fitting on the center NaOCI metering pump with the same style as two other chlorine pumps on either side. A spare fitting was also provided for future use at that time. Hart believes item is resolved to everyone's satisfaction. • Woodard & Curran: Bob Chapell noted that the manufacturer had changed the design of the fittings during the course of the project, and had since reverted back to the original design which they found performs more efficiently. • Status: Closed item. 6 Repair KOH leak at the KOH motorized valve (fvarranty list 6- 29 -06) • MBC Discussion: MBC reported to Hart that the recent repair by Hart on 8 -7 -06 did not fix the ongoing KOH leak issue. Chemical leaks have been an ongoing issue since substantial completion of the facility. Jim Ramos was notified that Hart is responsible to fix all leaks until its no longer an issue, even if this time extends beyond the one -year warranty period. KOH material continues to leak as of this date. • Hart Response: Hart's personnel was on site 8/7/06 to correct any visible leaks. MBC reported a continued leaking situation near the motorized valve to Jim Ramos on 8/9/06. Hart ordered new "O" rings and will return to the site to correct accordingly. • DPW/Water Dent.: Water Treatment Foreman indicates that this problem still needs to be resolved. • Woodard & Curran: Bob Chapell concurred that a small leak remains at the threaded fitting near motorized valve. • Status: Open item. Hart Engineering to return and install "O" rings. Repair work needs to be coordinated with Water Treatment Foreman, Kevin O'Donnell. 7. Repair leak near the flange on the KOH transfer pump (warranty list 6- 29 -06) • MBC Discussion: Chemical leaks have been an ongoing issue since the project was substantially complete. Hart is responsible to fix all leaks until its no longer an issue, even if this time extends beyond the one -year warranty period. • Hart Engineering: Hart has ordered new "O" rings and will return to correct accordingly. • Status: Open item. Hart Engineering to return and install "O" rings. MBC Secretary will notify Jim Ramos to coordinate leak repair work with Water Treatment Foreman, Kevin O'Donnell. 8. Investigate and repair Filter flow meters (warranty list 6- 29 -06) • Hart Engineering: Flow indication forward and backwash are correct and set up by the manufacturer. Step 7 is a transition step therefore flow indication will be effected. A programming modification can be made by Roberts Filter to ignore the indication during this step if desired. Roberts Filter would request a written change order for this work if the modification is requested. • DPW /Water Dept.: Flow meters indicate a fluctuating flow rate and /or zero during backwash (Step 7). Water Treatment Foreman, Kevin O'Donnell indicates that this problem needs to be resolved. • Woodard & Curran: Bob Chapell explained that Step 7 is designated by Robert Filter as a "Pre -low flow backwash" with an adjustable preset for the step duration currently set at 5 minutes. o The statement by Roberts Filter of this being a "transition" step where operators are to ignore the flow readings for the duration of the step is confusing. W &C questioned the point of the low flow rate for 5 minutes, if the staff cannot verify that flow rate. Also, the valves typically take 10 to 30 seconds to move to their required position. • Bob Chapell questions whether Roberts Filter response is stating that no function other than valve positioning is happening during Step 7 for 5 minutes? If so, W &C wants confirmation. • Woodard & Curran does not need the Filter Control Panel modified to ignore this indication. • Status: Open item: Roberts Filter needs to respond to Woodard & Curran regarding this issue. Note: Bob Cltapell referenced having recent conversation with Harlan Bartlett of Roberts Filter about issues at the Water Treatment Facility. Mr. Bartlett assured Woodard & Curran that Roberts would return to resolve outstanding issues with the treatment system. 9. Landscaper to rentol,e and replace (4) fbur dead shrubs (warranty list 6- 29 -06) • Status: Pending. See Item No. 13 below. 10. Repair /replace Weil McLain Boiler controller (warranty list 6- 29 -06) • MBC Discussion: Ken Gay suggested that DPW request the HVAC contractor (InControi) to verify repair /replacement when they perform seasonal changeover from cooling to heat this fall. • Hart Engineering: Mark Cordery (NEP) informed Jim Ramos that this work was completed earlier in the year. Hart requested, but has not received written confirmation from his subcontractor. • Woodard & Curran: Bob Chapell is unable to verify. • Status: Closed item. Town will request InControi to confirm change. IL Sensor for boiler water supply temp relocated to supply header )Varranty list 6- 29 -06) • Hart Response: Mark Cordery (NEP) informed Jim Ramos that this work was completed earlier this year. Hart requested, but has not received written confirmation from his subcontractor. Hart believes work is done and recommends owner field verify. • Woodard & Curran Response: Bob Chapell stated that as of this date the sensor remains in the wrong location. • Status: Open item. New England Pipe needs to respond. 12. Correct space gap in exhaust louver in northwest corner of Vehicle Storage Roont (warranty 6- 29 -06) • Hart Engineerintr (8- 15 -06): Mark Cordery (NEP) informed Jim Ramos that this work was complete. Hart personnel was on site 817106 and adjusted the damper to close further. • MBC Discussion: MBC previously notified Hart that the louver installed needs to close tight with no air gaps, in order to avoid drafts complaints. • Hart Engineering (8- 22 -06): NEP and Hart adjusted this damper several times and believe it is fine. Jim Ramos responded that the damper in question is an intake.damper, which simply has aluminum blades that close against each other. There is no gasket for a completely weather -tight seal therefore a slight gap should be expected when in the closed position. • DPW /Water Dept. Comment: Water Operations Manager Kurt Gaffney reported that prior adjustments to repair the damper are unsatisfactory. • Louver panel is still bent. • Space gaps between the louvers on the upper tier of the damper approximately f - inch wide must be corrected. • Kurt Gaffney noted that the louvers on a second damper installed in the opposite side of the same garage do close tight. • Woodard & Curran: Both dampers should close tight to prevent heat loss. • Status: Open item. Hart and/or Phoenix Sheet Metal need to address this item. 13. Revised; Remove `ALL" dead shiwbs and replace ivith new plantings. (Revised warranty list 7- 20 -06) • MBC Discussion: Owner requests all dead shrubs be removed and new plantings be installed during planting season Sept. 2006. 1 -year warranty is expected for all new plantings. • Hart Engineering: MON Landscaping is scheduled to replace shrubs. Subcontractor visited the site and agreed it would be best to wait to replace "ALL" dead shrubs during fall planting season. • Status: Pending completion of Item 9 and Item13: 14. Adjust belt for indoor fan in air handling unit (Revised warranty list 7- 20 -06) • Hart Response: NEP was contacted and did not response. Belt was adjusted accordingly. • Status: Closed item. 15. Replace filters in air handling unit with proper size (revised warranty list 7- 20 -06) • Hart Engineering: Correct size filters are installed. Owner needs to replace filters on occasion as part of routine maintenance. • Status: Closed item. 16. Lights on hwnsfer pump boxes do not illuminate in i7in and close positions • Hart Response: Hart verified power is active lights on pump panel. • DPW/Water Dent.: Water Division to replace bulbs as necessary. • Status: Closed item. 17. Adjust door weather stripping to close center- gap between doors in wafer treatment area (warranty list 7- 20 -06) • Hart Response: Adjustments to weather stripping is as of 7 -7 -06. • DPW /Water Dept, Comment: Work appears to be satisfactory. • Status: Closed item. 18. Investigate trouble signal and reset Fire Alarm System (wan•anty list 8 -3 -06) • Hart Response: Alarm system was checked completely and reset by Fall River Electric on August 7, 2006. . I • DPW/Water Dept Comment: Fire Dept. was unable to reset system. The problem may have been associated with fire hydrant testing that was going on outside of the building. • Status: Closed item. 19. Continued problem reported with leaking at motorized valve (8 -3 -06) • Hart Response: New O -rings should be delivered this week. Hart will return to change accordingly. • Status: Open item. Hart needs to address ongoing leak issues. Related Discussion: UtMwid brnoice/Door Systems, Inc. — The Water Division received an invoice in the amount of $345.00 for overhead door repairs that were completed within the warranty period.
