Motion: [Hanlon/Boyer] to accept the October 21, 1996 Meeting Minutes as
presented.
Vote: 3 -0 -1
(Jack Smith abstained.)
Certificates of Compliance
SE211 -310 - Treetop Subdivision - Deerpath Lane
Two buildable lots remain in this subdivision. Mr. Lewis recommended a Full
Certificate of Compliance be issued.
Motion: [Hanlon/Boyer] to issue a Ikill Certificate of Compliance.
Vote: 4-0 -0
SE211 -484 - Cobblers Corner (Off Willow Street)
This was a roadway drainage project. Mr. Lewis inspected the area which is
stabilized and there is no sign of siltation coming from the subdivision. Individual
lots may come before the Commission later from separate Notices.
Motion: [Boyer/Hanlon] to issue a 14ill Certificate of Compliance.
Vote: 4-0-0
SE211 -483 - Lot 3. York Road
This is a single family lot in Forest Park. An Order of Conditions was issued in
January, 1994. This is now in satisfactory condition. Mr. Lewis recommended the

Motion: [Hanlon/Smith] issue a Partial Certificate of Compliance.

Public Hearings
Massachusetts Highway Department - Request for Determination of
Applicability
Re: South Main Street - Resurfacing of South Main Street to Route
495
7:45 p.m. -- Mr. Tom Maguire, Department of Environmental Engineering,
Department 5, and Charles Susi, Highway Maintenance Section, are putting
together a contract package for the resurfacing of South Main Street to Route 495.
They intend to put clown a new riding surface, two (2) inches off, two (2) inches on.
There will be no road widening and no new drainage. They will, however, make
repairs to existing areas as required (catch basins, new lining). The asphalt will be
disposed of properly.
Mr. Lewis recommended the Commission issue a Negative 3 Determination.
Motion: [Hanlon/Boyer] issue a Negative 3 Determination.

Folkman & Zola - Notice of Intent
Re: Lot 9, Keli Green
8:00 p.m. - Construction of a house with associated grading within 100 feet of a
wetland resource area. Paul Folkman noted that this is a very straight forward
proposal.
A site walk will take place on Saturday, November 23, 1996, at 7:30 a.m. Abutters
were invited to attend. The corner of the house will be staked out. There is no file
number for this work.

Motion: [Hanlon /Smith] to continue this Hearing until October 9, 1996 at 8:00
p.m. with a site which will take place on Saturday, November 23,
1996, at 7:30 a.m.

Angle Tree Development Corporation - Reauest for Application of
Applicability
Re: Mayhew Farms, School Street
Review and Verification of Wetland Boundaries
Robert Colloran has already had a preliminary meeting with the Planning Board
and he needs to have the wetland line verified by the Commission so that they can
be aware of any discrepancies and reach agreement concerning areas that have been
flagged. Ms Sandy Vokes, abutter, raised concern regarding water runoff. Mr.
Lewis will meet with Ms Vokes and walk the site this week.
A site walk will take place on Saturday, November 23, 1996, at 8:15 a.m.
Motion: [Hanlon /Smith] to continue this Hearing until December 9, 1996 at
7:50 p.m, and a site walk will take place on Saturday, November 23,
1996 at 8:15 p.m.

Old/Now Business
Next Meetint: The next meeting of the Conservation Commission will take place
at 7:30 p.m, on Monday, December 9, 1996.
Enforcement Order - Treannie/West Street
RE: Grading and Filling Activities
Mr. Lewis reviewed the November 6, 1996 Enforcement Order sent to R. Treannie
regarding grading and filling activities. Hay bales are to be used in the winter and
Mr. Lewis will follow -up in early spring regarding planting. There will be no
extension of anything into the brook.
Wetlands Protection Act Workshop
There will be a workshop on Tuesday, December 3, 1996, from 2:00 p.m. to 4:30 p.m.
and from 6:30 p.m. to 9:00 p.m. in the Regional Office in Lakeville regarding the
Rivers Act.

Boy Scouts Community Project: Re: Great Woods Trails Signage. Mr. Lewis
will review issues of maps and required tasks for trail work.
