On November 16, 1977 Chairman received call. It is originally, James Bieniecky. Chairman took file from Town Hall and went dorm. On 12/15/ Americo Day was issued application for Disposal 'Works Permit. Th's has been sold to Allan Brown. This was originally threeAMT; now two bedroom duplexe. He has recorded all of the monitoring himself and has taken time off from work to do so. His complaint was the house septic system being installed is not going to be four (4) feet above ground water because he had monitored the pipe we had put in and a well that was there beh: -d_ the bushes. Chairman listened to all complaints and showed that the bench mark was for Lots 59 and 46. Chairman explained Bench mark and it appears that everything seems to coincide with the plans the Chairman had with him, but that I would get ahold of our engineer today. I brought Jim Dunn and spent two to three hours there today with L'r. Bieniecky and Mr. Al Fasulo who owns property down there. Allan Brown is the builder. Board of Ilealth Page three November 16, 1977 3. Chairman's Report (cont) The installer is Cary Dethl6fs and David White. On lot 59 we found elyation mark on telephone pole to be about a foot over to the Y y of septic dam. On lot 46 they are using a tree for elevation mark, which RIM engineering also used according to Mr. I,laloon. If this is a true statement, lot 59 is correctly installed. This will need an as built to show that correction. On lot 45 the bench mark was the tree. We found that Mr. Dethlefs, the installer, had gone right by the Plan. Our monitor well on lot 46 is still in the ground. The septic dam on lot 45 is alright, but we found the monitored well water had risen another eleven (11) inches. We felt there would be a difference because from the septic -T&hf to the leeching trenche here was an eight (8) inch difference between the septic Wand trenches This would make it a three (3) inch pitch and pick up the difference. Pick up another five (5) inches in leaching trenches. This was agreeable with the installer, the builder, Vr. Dunn and Chairman. The Chairman will do the final inspection tomorrow and Friday, r'r. Dunn will be submitting a report also. Lot 40 Eaverick Drive, builder Kelton Potter. Letter to I:Ir. Potter advising him that :attached. is a copy of our engineers recommendations. Also a copy of letter we sent to the Conservation Commission regarding #2 of our engineers recommendations. Also attached is a copy of letter from Conservation Commission which requires you will have to file for a Wetlands Protection Hearing on this lot before approval can be granted. 4.
