Health Agent explained that this company, GER, started the installation of the septic system
without a permit. Mr. Keenan called Agent for an open hole inspection; at that time, he was told
that he needed a permit. Agent did inspect, however, in the interest of safety. Hole was not dug
properly (according to plan). Mr. Keenan questioned that the plans "must be different ". Mr.
Keenan applied for permit and when references were checked, two communities said that you
"need to watch him ".
With that, the Agent recommended that Mr. Keenan be allow to put just this one system in under
his permit. Board disagreed and the following motion was made.
MOTION: To deny a disposal works installers permit to Matthew Keenan,
GER, Raynham, since he started installation without a permit,
excavation did not conform to plan, and he demonstrated that he
does not have knowledge of Title V.
Fontanini/ Forget. All in favor. VOTED
Agent will send letters to GER and owner of the lot.

5. Public Hearing
At 8:00 PM Chairman Sosman opened public hearing for the Regulation of the Schedule of Fees,
which was posted at the Town Hall. No one attended the public hearing and there were no
questions from the Board. Hearing closed at 8:01 PM.
Chairman did remind the Board that at the last bi- monthly meeting with the Board of Selectmen,
a Selectmen asked the Board to reconsider their fee for the Disposal Works Installers Permit,
which is $25.00. Intent was to align DWIP fee with the Selectmen's new Drainlayer's License
fee, which is $250.00. Also suggested was the possibility of combining the two licenses with
one fee. Concensus of the Board was to do neither.
Agent suggested that there may be a need to increase the Food Establishment Permit fee for
Super Stop and Shop and Rojacks. The next time he inspects, he will keep track of time. It
foresees Stop and Shop taking over a half day.
MOTION: To approve and sign the Regulation of the Schedule of Fees.
White/Fontanini. All in favor. VOTED
6. Health Agent's Report
a. Jameson Farm Subdivision Sewer Project: Sewer installation has been completed.
Variance was conditional upon need, but now there is no need, since the sewer in complete.
First, Mrs. Jameson's attorney will be contacted by phone, followed up by a letter to Mrs.
Jameson.
b. 333 School Street: Waiting for end of month to find out what the Goguens will do
about the failed septic system at 333 School Street. Home continues to be occupied, since
occupant will not leave. System is being pumped according to the Health Agent's order.
c. Title 5 Amendments: Governor has proposed amendments. Certificate of
Compliance valid for two years, times frames lengthened, and standards for failure changed.
d. Hatheway & Patterson: Chairman Sosman and Health Agent walked the site. EPA
has left and DEP will take over. Some areas have been asphalted and some compacted with
gravel to prevent infiltration into groundwater. A significant amount of fencing has also been
installed.
e. Great Woods Sound: Board of Health received no complaints this year. Agent
suggested that this problem is a cultural one, subjective —not objective. Consideration might be
given to a general noise bylaw, not a BOH reg. (Member White did not participate in this
portion of the meeting.)
PAGE 4, BOH MINUTES, SEPTEMBER 7,1995
L Fluoride Update: Waiting for a return call from Engineer Fred Barker, DPH, to find
out where Mansfield is on the list for funding.
g. Community Visiting Nurse Contract for FY96: Agent met with Nurse Chris
Delano and the following was agreed upon for the upcoming contract. Hepatitis clinics will be
paid for by the specific department. Blood sugar/blood pressure clinics will be reduced to once a
month for two hours from every two weeks. Cholesterol clinics will be held twice a year, rather
than every three months. Board will pay separately for any overruns of clinics beyond the two
hours. Mantoux and lead level testing will be continued as needed. Agent suggested that patients
travel to CVNA for service, rather than Nurse Delano taking travel time to go to them. Nurse
agreed providing the patient has transportation. Last year they extended service to about $2,200.
This is not a permanent change; plenty of service will continue and in September, CVNA will
submit a request for additional money for July 1, 1996 to June 30, 1997, in time for the budget
process.
MOTION: To approved CVNA contract for July 1, 1995 to June 20, 1996
in the amount of $16,450.
h. Cobbler's Corner: Proposed subdivision off Willow Street. Developer has been told
that if he is within a reasonable distance to sewer the subdivision, then it must be sewered.
There are two ways to go: forced main and pump (expensive) or gravity main to Spring Street
and Willow Street. A possible cost would be $148,000 ($$7,800/lot, 19 lots). Average system
with those soil conditions could cost about $5,000. Mr. Willis, developer, feels that Agent and
Board have delayed his project. Minutes were reviewed and it was noted that month after month
"no response from Mr. Willis re: Cobblers Corner ".
Two perc applications have been received by a builder (not Mr. Willis) for two street
lots, which are part of the subdivision. Agent is not bound to witness these peres, since this 19-
lot subdivision has been ordered to be severed.
i. Agent's Vacation: Agent and Building Inspector (who covers for inspections) will be
out concurrently. Agent recommended that the Board's policy of August 26, 1992 be enforced
that if an inspection is necessary during this time, a professional engineer or registered sanitarian
may be engaged to certify that all work was performed in compliance with Title V.
j. 340 So. Main St.: Agent believes there is a tenant occupying 340 S. Main St.
Groundwater Technology is conducting testing to ensure safe habitation. A house across the
street has been recently sold and the seller did not disclose to the new owner the hazardous waste
site across the street.
PAGE 4, BOARD OF HEALTH MINUTES, SEPTEMBER 7, 1995
L Future Workshops: Agent would like to continue a workshop a month starting in
October. Since Agent will be on vacation for the Sept. 21st meeting, he sugggested a workshop
with NEMS Tobacco Control. Board agreed.

complying, and commented that the Town `n Country Market is looking good.
7. New Business

among teens lead to discussion of other health related issues. Schools will be contacted to find
