Health authorities now agree that Auoride should not be given to infants. Health
professionals, water districts, and the media need to work cooperatively to deliver
this information to parents and caregivers so they can protect their children.
. On November 9th 2006, based on 0 Notional Research Council
(NRCl report OJ the Americon Dental Association (ADA) warned
that fluoridate water should not be mixed inlo concentrated
formula or foods intended for babies one year and'younger
in order to ovoid moderate dental fluorosis (white, yellow or
brown stains affecting the tooth surface) (2).
. On December 15, 2006, the Centers for Disease Control ((DC)
slated that using fluoridated water for infant formula may
influence 0 child's risk for developing enamel fluorosis (3).
According to the (DC, 32% of Americon children hove dental
fluorosis, 0 signiflcont increase from 23% in the 1980s (4).
. The Acodemy of General Dentistry has cautioned parents to
ovoid fluoridated water when making infant foods (5).
. The Americon Academy of Pediatrics has advised that fluoride

Here also are the revised guideline for fluoride supplementation by the ADA. This
was sent by Chestnut Dental in Franklin.
FLUORIDE GUIDELINES - CHESTNUT DENTAL
ASSOCIATES
In December 2010 the ADA published "Evidence-Based Clinical Recommendations
on the Prescription of Dietary Fluoride Supplements for Caries Prevention". In tllis
widely publicized paper, the authoring panel emphasized the need for caries risk
assessment and judicious prescription of dietaty fluoride supplements.
In addition, we have completed a review of the literature of fluoride use as a dental
decay preventive measure. Based on these reviews, we have revised our guidelines
for the use of fluoride for our cllildren and adolescents patients.
All Children and Adolescents
. Use a fluoride containing toothpaste, regardless of age and town's fluoridation
status
. Swish and spit out excess with minimal rinsing
. Quantity
o Under three years - smear size of paste
o Over three years - pea size of paste
Systemic Fluoride Supplementation ONLY IF
* Living in a non-fluoridated community AND moderate/high risk for decay
which is defined as:
. Mother or primaty caregiver had active decay in past 12 months
. Continual use ofbottle/sippy cup with fluid other than water
. Greater than 3 between meal snack or beverages consisting of
sugars/starch/fennentable carbohydrates
. Special health care needs that increases caries risk
. White spots, decalcifications, decay, previous restorations, visible plaque,
enamel defects
. Saliva reducing factors: medications, medical conditions
* If using strong topical fluoride supplement such as Prevident@,then
discontinue supplement. Those living in a fluoridated community BUT using
well water:

Thank you for responding. I went to the link but it looks like it just tells
how much fluoride Mansfield's water has it does not specifically state not
to use the water to make infant formula. This would probably best be
accomplished by enclosing a notice in the water bills that evelyone has to
open. If you feel that I could have any impact in starting some
discussion on the water fluoridation and the new findings of fluorosis 1
would be glad to speak with people. But if you feel that one person is
unlikely to make a difference ,I am velY busy between my practice and

my three kids, so I do not want to come if you do not think the board is
open to discussion. In any case the flyer in the water bill is important
since I do not think most people realize they should not use the water to
make baby fonnula.
Thanks,
Dianne Pacheco MD

Dear Dr. Pacheco,
The below link has been posted on Mansfield's website since
your previous e-mail. We are occasionally asked what the
fluoride dose in town water is and that is question answered.

Dear Scott,
I have spoken with you in the past about the issue of water
fluoridation in regards to infants. As far as I know the town
has not included warnings for not using town water for the
making Offol111ula. If they have and I missed it please let me
know. I feel not doing so is negligent since the town is putting
a potentially toxic substance in our water which can be
harmful especially to infants.
I have attached a sample flyer. I think it is time we rethink
water fluoridation in general since recently it was found that
40% of children in cOlllillunities that fluoridate their water
have fluorosis. I am not sure why there is not more of an
outrage over this. Recently Chestnut Dentist in Franklin sent
information to my office saying we should no longer be
supplementing fluoride in children anymore before doing a
risk assessment survey first. UnfOliunately those of us with
fluoride in our water do not have a chance to defer the
fluoride.
Please take this under consideration.
Thank You,
Diatme A. Pacheco MD
