hearing for the Lakeview Ave. project. M fl. ""
1. Request for Determination of Applicability _ Town of Mansfield
Re: Drainage System Improvements to Lakeview Avenue.
John Sullivan, Town Engineer described the work as a proposed maintenance and improvement project
to the existing storm water system He indicated that flooding occurs on a regular basis since the
existing leaching system cannot handle flows, and freezing takes place in the winter creating a public
safety issue. High ground water and the fact that there is no outlet impact the existing system. The
proposed work will take place in the roadway and on a right-of-way between two houses. New catch
basins with sumps and an oiVwater separator will be installed. The existing storm water goes towards
the lake. The new system will have a discharge pipe to the lake with a riprap and outflow configuration
that will reduce velocity. The existing system creates erosion that runs into the lake and the new system
will reduce this. The proposed system will eliminate 50% of total suspended solids. Abutter Kevin
Boyden urges that some immediate repairs be done to the roadway since the conditions this past winter
have tom up the surface. Paul Boyden asks about the high water elevation. John Sullivan did check this
before the meeting and indicated that the outlet pipe could stop at the current high water mark, which is
probably at its highest level. Motion [Flass/Matthew] to close the hearing; Motion [Smith/Flass] to
issue a Negative Determination citing elevation of outlet at high water mark. Both Motions voted 4-0-0.

Notiee of Intent hearing - Canoe River Campground, Mill Street
Engineer Ralph Maloon and owner Joe Gonsalves presented the plans. Project involves 1.86 miles of
sewer lines, which may be buih in two phases. The shorter phase will connect to the line in Mill St. near
the entrance to the campground. The second phase will serve the majority of the mcility and will go to a
pump station that will run out to Mill St on the north side of the Canoe River bridge. The installation of
the sewer will help with water quality and solve the problem of an old septic system at the campground.
The connections to the individual trailers will be made with a tight fitting approved by the state. The
project will also be reviewed and approved by the Town. The sewer line that runs under the secondary
outlet to the pond, can be done under dry conditions as the flow of water can be controlled by boards.

Notice olInteDt hearing - Home Depot, West St and Route 140
Jack Mette, App1edore Engineering, and Mickey Marcus, New England Environmental presented the
plans. Brian Leahy, representing the landowner, also present. Project involves construction of several
building and associated parking for around 1300 cars. There are two bvw areas near Route 140, and an
isolated area just off the site. This isolated area is presently the subject of an adjudicatory appeal. No
wetland alteration is proposed. No work proposed within 50 feet of any resource area with the exception
of the removal of existing pavement and some invasive plants, and the planting of native species which
will improve the wildlife habitat features. The applicant is looking for the Commission to approve this
work within the 50-foot zone. There is no true stonnwater management system in place. The proposed
system will meet or exceed all DEP policies and practices. All catch basins will have deep sumps, with
water quality swales. These swales will have a plastic turf re-enforcement barrier that allows grasses to
grow through them This helps to prevent eroding of the swales over time. Pre and Post development
flows are balanced. A maintenance program for the basins, swales, parking areas, etc, will be in place
and is outlined in the NO!. Snow storage areas are identified. The shape of Basin # 3 [around 2 acres in
size J may change due to just received information on soil conditions. A revised plan will be presented at
the next hearing. Abutter Bob Jones, Walnut Street, voiced concerns with some of the historical
activities on the site that have impacted his property. A swath was cut near the rear of the proposed
basin # 3, and he is worried with water weeping from the basin onto his land. Also indicates that past
work on the site has backed water onto his land, which in turn helped enlarge this isolated area.
Attorney Bill O'Connell, representing the abutter Royal Oak Trust, outlined the ongoing appeal on the
question of the nature of this isolated area and whether it is in fact a bvw. Mentioned that the EIR is
supposed to address the questions of the filling of ponds on the site years ago. He ask the Commission
continue the hearing so that the EIR can be reviewed and considered. In response to these comments the
applicant's representatives state that corrective action nears Mr. Jones' land will be performed to prevent
any negative impacts. And, they state that the ENF and EIR process is separate and not part of this
Notice review. [Commissioner Flass leaves the meeting]. Motion [Matthew/SmithJ to continue the
hearing until June 4 at 8: 15 p.m when a revised plan for Basin # 3 will be presented; voted 3-0-1
