Mr. Stephen Provazza manager from Honey Dew Donuts approached the podium and addressed
to the board that he has re-trained his employees, and put up new signs in the facility. Risk
category will be moved from a category 2 to a 3.
Ms. Karla Hernandez, District Manager for Dollar General, approached the podium and addressed
the Board regarding the recent concerns at the store. She explained that there is a new Store
Manager and that employees will be retrained on proper procedures.
Ms. Hernandez stated that the store recently underwent a remodel and believes the third -party
contractor responsible for restocking the shelves may not have been properly trained on the store's
procedures. She explained that all items should be checked weekly for damaged or expired
products, with an electronic checklist used to document the inspections.
Ms. Hernandez also stated that Dollar General will be conducting a deep cleaning of the store the
following week. During the cleaning, staff will pay particular attention to identifying and removing
any dented, damaged, or expired cans and products.
Ms. Donovan -Palmer stated that she will send an email to the responsible party and allow one
week for the requested documentation to be provided. If no response is received, or if it is
determined that there has been negligence, the Board will consider issuing a fee of $50 per day.
It was also discussed that the technology installed when the building was originally constructed is
now outdated. Because the technology is no longer available, repairs have become difficult. The
issue now is determining how the existing technology can be upgraded or replaced.
Ms. Donovan -Palmer clarified that the matter remains a housing code violation and that the
outdated technology must ultimately be replaced to bring the property into compliance. The
meeting minutes did state that the owner was to attend the September loth meeting.
The occupant is no longer residing in the unit. Ms. Donovan -Palmer has scheduled an inspection
with the owner for September 22nd to verify that all required corrections have been completed.

This unit is located within West Village Apartments. Ms. Donovan -Palmer visited the building for
several housing inspections and observed that the call box was not functioning. Ms. Donovan -
Palmer contacted management and informed them of the violation.
Management requested an extension to allow them time to obtain quotes from contractors to
address the issue. They anticipate that the repairs will be completed in 2027, with no specific date
communicated to the Board for completion.
Ms. Donovan -Palmer recommended that management attend the October Board of Health meeting
to provide an update on the status of the issue and the steps that have been taken to correct the
violation.
