When you applied for your 2015 Town of Mansfield Tobacco Permit
you submitted a Temporary Voucher from the MA Department of
Revenue Cigarette and Tobacco Excise Unit. You should have
received your permanent Form CT-3T by this time.
Please be advised that this office requires a copy of your 2014-2016
Mass. Dept. of Revenue Form CT-3T when you receive it. This form
can be entailed, faxed or mailed to this office. The email, phone and
mailing address is at the top of this memo.
