In accordance with 310 CMR 40.1403(3)(a) and on behalf of Mrs. Elizabeth J. Giordani,
Penney Engineering, Inc is hereby providing notification to your offices that over the
next six months we shall be conducting field work related to the implementation of our
Phase IV Remedy Operation Plan at the former Mansfield Motors hazardous waste site
located at 297-301 North Main Street, next to the Fire Station. We shall be periodically
inspecting and sampling the monitoring wells located at the vacant property. The
monitoring wells are sampled every three months.
We may also be applying remedial additives to the monitoring wells and injection wells
to enhance the biological remediation at the remaining oil contamination in the deep soil
and groundwater. The six injection wells are located along North Main Street. During
snow removal activities it appears that the DPW damaged at least one of the stickup -
type injection wells. Could the DPW repair and straighten the damaged wells?
All the fieldwork shall be conducted at Level D personal protection. If the level of
protection is upgraded during the course of the work to Level C or above, we shall orally
notify Mr. Leite of the upgrade as soon as possible.
In accordance with 310 CMR 40.1403(3)(e), you are also hereby notified of the
availability of a report that we have prepared for the site. Every six months we are
required to submit a Phase V Status and Remedial Monitoring Report for the site to the
Department of Environmental Protection ("the DEP"). Our most recent report shall be
electronically submitted on or about February 19, 2010. The report can be viewed on
the DEP website under Release Tracking Number 4-01140.
Local officials may obtain hard copies of our Phase V Status and Remedial Monitoring
Reports by contacting the DEP Southeast Regional Office at (508) 946-2714 and
referring to Release Tracking Number: 4-01140.

Dear Messrs. Riccio and Leite:

On behalf of Mrs. Elizabeth J. Giordani and in accordance with the public involvement
requirements specified in the Massachusetts Contingency Plan, Penney Engineering,
Inc is hereby providing notification to your offices of the availability of a combined
Phase II Comprehensive Site Assessment Report, Phase III Remedial Action Plan,
Phase IV Remedy Implementation Plan and Phase IV Completion Station for her
vacant property at 297-301 North Main Street. Her property is located adjacent to the
Central Fire Station. On February 13, 1992, the Department of Environmental
Protection ("the DEP") issued the first Notice of Responsibility to Mrs. Giordani for the
gasoline -contaminated soil and groundwater discovered by the Town at her property.
Her property became a hazardous waste site.
In April 2003, we excavated and recycled 442 tons of petroleum -contaminated soil from
the property. We applied remedial additives to remediate the contamination that
extended under North Main Street and could not be removed. We monitored the
groundwater on a quarterly basis. The concentrations of the contaminants continued to
decrease. In 2007 we were ready to close the site when the results for only one well
were found to be higher than we ever detected. We suspected a new release near the
well or tampering with the well.
We have confirmed that two private water supply wells are located within 500 feet of
the site and there is a potential for someone to drink the water from the wells. As a
result, the groundwater at the site must meet be remediated to meet the GW-1 cleanup
standards. Our most recent groundwater sampling results exceed the GW-1 cleanup
standards. We shall continue to monitor the groundwater on a quarterly basis and may
apply additional remedial additives. We shall begin to notify both your offices of our
sampling in advance.
Local officials may obtain copies of the combined Phase II Comprehensive Site
Assessment Report, Phase III Remedial Action Plan, Phase IV Remedy
Implementation Plan and Phase IV Completion Statement by contacting the DEP
