On behalf of the 79 Rumford Avenue Realty Trust and in general accordance with the
public involvement requirements of the Massachusetts Contingency Plan (310 CMR
40.1400) Penney Engineering is providing notice to you of the availability of our first
Immediate Response Action (IRA) Status Report for the heating oil release at 79
Rumford Avenue. On December 9, 2015, Grant Brothers Associates of Mansfield was
removing an underground heating tank from the residential property. Lt. Mark Goyette
of the Mansfield Fire Department was on -site. The tank was removed and heating oil
was observed in the soil at the bottom of the tank grave. The Department of
Environmental Protection ("the DEP") was notified and assigned Release Tracking
Number (RTN):4-25914. On December 10, 2015, approximately 50 cubic yards of
contaminated soil was excavated from the tank grave and stockpiled on -site under my
direction according to an oral IRA Plan. The heating oil contamination was found to
extend under the rear section of the house and could not be removed. On February 15,
2016, we submitted a written IRA Plan to the DEP.
On March 8, 2016, we excavated a test pit at the opposite side of the house and
installed an observation well. We only found residual evidence of heating oil at the
groundwater table. The analysis results of a soil sample were all below the DEP
cleanup standards. We also sampled the groundwater in the tank grave and found it to
be significantly contaminated with heating oil. On April 14 & 15, 2016, the stockpiled
soil was transported to the Aggregate Industries asphalt facility in Stoughton to be
recycled.

In accordance with 310 CMR 40.1403(3)(a), I am notifying you that over the next six
months Penney Engineering shall be conducting field work at 79 Rumford Avenue. The
fieldwork shall include installing monitoring wells, collecting samples, and applying
remedial additives. All the fieldwork shall be conducted at Level D personal protection.

On behalf of the 79 Rumford Avenue Realty Trust and in accordance with the public
involvement requirements of the Massachusetts Contingency Plan (310 CMR 40.1400),
Penney Engineering is providing notice to you of the availability of our Immediate
Response Action (IRA) Completion Report and Permanent Solution Statement for the
heating oil release at 79 Rumford Avenue, On December 9, 2015, an out -of -service
underground storage tank, last used in the 1970s, was removed from the rear of the
residential property. Lt. Mark Goyette of the Mansfield Fire Department was on -site to
witness the removal. The old steel tank was found to have leaked. I was contacted and
informed of the situation. I immediately notified the Department of Environmental
Protection ("the DEP"). Mr. Jaime Goncalves assigned Release Tracking Number
(RTN):4-25914 and approved my oral IRA Plan.
We subsequently removed 58.81 tons of contaminated soil from the tank grave and
later recycled it off -site. We sampled the soil from the excavated tank grave. We found
that a layer of heating oil -contaminated soil remained at the groundwater table, at a
depth of 10 feet. The remaining heating oil primarily extended to the south, under the
rear section of the house, and could not readily be removed. Only relatively low
concentrations of heating oil -related contaminants were later detected in the
groundwater in the excavated tank grave. We excavated a test pit on the opposite side
of the house. We encountered a layer of heating oil -stained soil at the groundwater
table which confirmed our assumption that the heating oil had migrated on the
groundwater to the south towards Fulton Pond. The stained soil was sampled and
analyzed. No heating oil -related contaminants were detected in the stained soil
indicating that the very old heating oil had been adequately bioremediated. It is my
professional opinion that the small amount of heating oil remaining 10 feet below grade
at the rear of the property, does not pose a significant risk of harm to human health or
public welfare. It is expected to also be adequately bioremediated in time.
