1. In order for the Board of Health to take action on the application, you must
get numbered address and map and parcel number from the Town Assessors office.
No application will be accepted without this information.
2. Fee is $100.00. Application and fee must be filed 10 days before testing
date.
3. On -site sewage testing for in season water table testing; December 1 to
April 30.
4. Percolation testing may be done all year. All seasonal testing applications
MUST be received by March 31.
5. You must know before the Town's Agent comes out to your lot, as to where
you wish to have on -site sewage testing done.
6. Requirements needed at time of testing:
a. your own engineer
b. back -hoe or similar equipment, which would be able to dig 10-12
feet for pits or galley trenches
c. enough water for on -site sewage testing
d. sufficient 4" pipe for a monitor well
7. Permit will become invalid if percolation test is not performed within
the current percolation season, provided that the season has not been
suspended by the Board of Health, or if an out -of -season approval is
granted. Fee is non-refundable.
8. All septic system plans and DWCP applications must show soil testing
number, street number, and lot and plot number.

Gentlemen:
Enclosed are soil examination reports and a preliminary Plan
showing; the proposed location of the house and leach Meld on a
lot owned by Earle P. and Marie M. Zecher.
The :first soil test was done January 23, 1979 with consultant
James M. Dunn, his recommendation was to comas back in the dry sea-
son For a Percolation. test. The second soil examination was with
the Town. Engineer* Anthony G. Stanzione, he has .approved the soil
but is referring, us to your board. The lot would requi e up to
five feet of fill for a depth of about 120 feet from the street.

3/30 Perc test at 200 Otis Street, Zecher, could not be performed because
of the high water table,
9/4 Checked Sanitary Landfill. I spoke to the bulldozer operator.
He is beginning westward into Phase III as per plan.
9/5 Visited 16 Coral Street, ,lames Bell. Existing cesspool
seems to be filled. He is going to have Roto Roter and a
sanitary pumper re -pump the system. This will be the second
time he has had it pumped. A Repair Permit was given to him
and lie was advised of the proper proceedure to follow. He has
owned the property for one month. Mr. Bell felt this problem
should not have occured so soon. The previous owner should be
node liable for any repairs from this date forward.
