12. Notice of Intent: George and Lisa Chalifoux 1/1, 49 Jake's Way
Notice of Intent was filed for an addition, 28 ft. x32 ft. and a 30 ft. x 14 ft. deck to
the existing house. The closest distance to the BVW is 68 ft.
Derek House asked if any trees would be removed. Mr. Chalifoux said no.
Silt fence and hay bales will be placed behind the limit of work well before the
tree line. Michele is to inspect the erosion control.

Carole Matthew made the motion to accept the proposed addition and deck for
49 Jake's Way, plan dated February 2, 2009, with silt fence/hay bales behind the
limit of work. Siltation fence and hay bales are to be inspected by Michele before
the start of any work. No work is to be done in the 25 ft. buffer zone. Order of

3. Notice of Intent:. Canoe River Campground Dam Reconstruction@ Mill Street.
,:J#2c~J~0824. ...
John Sullivan, Town Engineer, and Rick Alves, Assistant Engineer were present.
Notice of Intent was filed for repairs and improvements to the Canoe River Camp-
ground Dam, a significant hazard potential (Class II) structure.
John Sullivan told the Commission the proposed repair is required by new laws
and regulations. Inspections of this dam in 2006 identified that significant repairs
were needed to the Canoe River Dam; the dam was found to be in poor
condition with erosion, over-growth of vegetation, unprotected slopes degraded
or failed training walls. Because the dam crest supports, the roadway, the road is
also in need of repair. On February 22, 2008, the state issued an order requiring
the town to repair the dam by November 30,2009. Bidding for the project will
start in April 2009.
Areas to be repaired are:
· Primary Spillway and Training Wall; the existing stone and concrete
primary spillway and training walls will be removed and a new concrete
primary spillway will be constructed within the existing foot print. The
proposed design has a 6 ft. wide fixed crest spillway and two 5 ft. wide
stop log bays.
· Auxilliary Spillway Repairs; the spillway control section will be replaced.
The breach section of the downstream masonary wall will be repaired and
regarded to stabilize the slope.
· Embankment Repairs; the pond embankment at Mill Street will be cleared
and graded to stabilize the slope. To protect the slope, riprap will be
added. The man-made downstream diversion channel will be graded to
stabilize the new slope walls and direct runoff to the primary channel. A
portion of the embankment between the main pond and an existing
campground duck pond will be repaired in a small area, 3 ft. by 100 ft., to
raise the embankment of the main pond. The embankment will be raise
approximately 1 ft. to meet the required design elevation.
· Roadway Improvements: related improvements will be made to Mill Street;
regrading of the roadway embankment slopes and installation of curbs
and drainage swales for stormwater management. John Sullivan told the
Commission the regrading will improve roadway drainage.
Erosion control, hay bales and siltation fence, will be installed at the limit of work.
Michele is to be notified when the hay bales/siltation control are in place for
inspection; no work is to be done before the erosion control has been inspected;
sediment controls must be inspected bi-monthly by the Conservation Agent.
All disturbed areas will be loamed and seeded when work is completed.
Water control and diversion shall be completed in two phases, to maintain the
flow throughout construction.
Temporary cofferdams will be provided to dewater areas to allow for work to be
completed in dry conditions.
The entire project is located within the 200 ft. Riverfront Area of the Canoe River;
because the project consists of the maintenance of an existing structure it is
exempt from the requirements of the Riverfront Act. The project is not subject to
the current Stormwater Management Standards; the roadway improvements
proposed, however, qualify as a redevelopment under the Stormwater
Management Standards.
The majority of the proposed work will occur within the 100 ft. buffer and the
bank of bordering vegetated wetlands and the 25 ft. and 50 ft. no disturb zones.
Disturbances in these areas will include clearing, embankment grading, slope
stabilization, spillway repairs, erosion control, equipment access and impacts
associated with the downstream training wall repair and replacement.
John Sullivan told the Commission the estimated cost of the repairs is
approximately $400,000.
Carole Matthew noted the project meets the requirements of the Town of
Mansfield Wetland Protection Bylaw, Section 2: Jurisdiction, 2.1, 2.2 & 2.5.
Paul Boyden made the motion to accept the plan, dated February 4, 2009, for the
repairs and improvements to the Canoe River Campground Dam with the
following conditions: hay bales and siltation fence will be installed at the limits of
work; Michele Grenier is to be notified when the hay bales/siltation control are in
place for inspection-no work to begin until inspected; hay bales and siltation
control are to remain in place until the project is completed; sediment controls
must be inspected bi-monthly by the Conservation Agent.
Linda Falk seconded.
Passed 5-0. Derek House abstained.
Hearing closed at 7:55 pm.
13. · Discus.sion: Phase IV SeWer Infiltration Rehapilitation Project-John Sullivan.
John Sullivan and Rick Alves.
The project was funded by Article 6 of the November 18, 2008 Special Town
Meeting.
John Sullivan told the Commission the project will be to repair the town's sewer
system by re-lining some of the existing sewer pipes that are leaking and sealing
(grouting) some sewer manholes. Most of the work is within existing roadways.
Most of the work will be done internally, without roadway excavation.
He told the Commission some of the work to be done includes repairs from inside
some of the manholes on the Plymouth Street cross-country sewer line that goes
through the woods behind the Plymouth Street Recreation Area. A NOI (DEP
#211-0783) was filed September 26,2006 for the construction of an 8 ft. wide
gravel access pathway within the existing 30 ft. wide sewer easement for
maintenance of the sewer main between King Arthur Way and Plymouth Street.
This access path has been completed in accordance with the Order of Conditions
issued October 25, 2006, allowing for the repairs necessary in this area. All the
work in this area will be from inside the sewer manholes and there will be no
impact on the wetlands in the area.
Discussion ended at 8:05 pm.
I. .5.. Old Business:
The Commission wanted an up-date on 23 Riverside Drive. Mr. lei wanted to use
the displaced dirt from the construction of an addition to fill in and level out his
property (the addition did not require a NOI). The property abuts Cabot Pond
and the Commission was concerned about Mr. lei doing any work without input
from conservation. Michele has spoken to Mr. lei; the project has been put on
hold.
