RE: PERMIT APPROVAL
Application for: BWPSWII
LANDFILL MAJOR MODIFICATION
POST -CLOSURE MONITORING PLAN
AT: Mansfield Landfill
East St/Rte 1_06
Mansfield, MA
Transmittal Number: 97779
Dear Mr. Dagostino:
The Department of Environmental Protection, Solid Waste
Management Section, (the "Department") has completed its Technical
Review of the Landfill Major Modification permit application
regarding the Post -Closure Monitoring Plan for the Mansfield town
landfill (the "Landfill") located off East Street and determined
that the application is Technically Complete.
The Post Closure Monitoring Plan application (the
"Application") was prepared by Camp, Dresser & McKee ("CDM") on
behalf of the Town of Mansfield (the "Applicant") and submitted to
the Department on November 21, 1995. The Application consisted of
a Transmittal Form #97779, an application form BWP-SW-11, a cover
letter dated October 27, 1995 and a "Plan" entitled:
Town of Mansfield, Massachusetts

Background
On September 21, 1990, the Department issued a Closure Permit
Approval for the Landfill. Based on its review of a November 1991
Engineer's Certification Report, the Department issued a Landfill
Closure Certification for the Landfill on April 12, 1994. This
Certification required that the Town of Mansfield monitor and
maintain the environmental monitoring system in accordance with
Department regulations.
Application Summary
In response to the Department's requirement for environmental
monitoring, the Application was submitted in October 1995 and
proposed actions necessary to develop a monitoring program in
accordance with 310 CMR 19.000. The proposed environmental
monitoring program includes groundwater monitoring, surface water
monitoring, landfill gas monitoring and site inspections.
The Application states that one upgradient groundwater
monitoring well currently exists and three down gradient monitoring
locations have been selected. Each location will include a we11
couplet with one well screened in the top ten feet of the saturated
zone and the bottom screen located just above till or bedrock.
Wells will be installed in accordance with the Department's
Publication #WSC-310-91 entitled "Standard References fer
Monitoring Wells", dated April 1991.
Two surface water sampling locations are proposed to be
incorporated into the environmental monitoring program. One
location is proposed in the Canoe River as it crosses East Street
and is stated to represent conditions upstream of potential
landfill influence. A second location is proposed where the Canoe
River meets an unnamed stream running south of the landfill which
is stated to represent conditions downstream of potential impacts
from the landfill. The precise location of this sampling station is
proposed to be determined during future field investigation.
Surface and groundwater samples are proposed to be collected and
analyzed semi-annually for the parameters listed in 310 CMR
19.132(1)(h) with results reported to the Department within 60
days.
No landfill gas monitoring probes currently exist on site. The
Application proposes to install landfill gas probes every 500 to
1000 feet along the perimeter of the landfill and monitor semi-
annually for concentrations of explosive oases with a portable
field instrument.
The Application states that annual landfill inspections wit-
be performed with the results included in bi-annual reports to the
Department pursuant to 310 CMR 19.142(6).

Landfill Status
On April 13, 1998, the Department and CDM inspected the
Landfill and the surrounding areas to ascertain the existing
conditions. In general the Landfill was maintained with little or
no evidence of erosion and vegetative distress and limited required
maintenance. It is anticipated that a report will be submitted to
the Town of Mansfield and the Department based on the inspection
identifying inspection results and maintenance actions to be taken.
The Department will cite additional concerns, if any, based on
review of the report.
In addition, the Department has noted that the unnamed stream
occasionally yields sufficient flow to be sampled and could be
influenced by stormwater -runoff, leachate outbreaks or contaminated
groundwater discharge from the landfill. This Approval will require
sampling of the unnamed steam at two locations. Inspection of the
unnamed stream revealed that the stream enters a heavily vegetated
area just downstream of the Shell Oil easement and that locating a
viable sampling location downstream of this easement may be
difficult.
Following the preliminary inspection with CDM, additional
inspection revealed a wetlands area approximately 100 yards south
of the unnamed stream and east of the easement which was highly
discolored. This Approval will require that the source of
discoloration be investigated to determine if it is natural in
origin or the result of contamination from the landfill or other
source.
Approval
The Department has reviewed the above -referenced permit
application and hereby approves of the Post Closure Monitoring Plan
subject to the following conditions:
1) Within 30 days of this Approval, a report based on the
April 13, 1998 site inspection should be submitted to the
Town and the Department identifying inspection results,
required actions and an appropriate schedule for required
actions.
2) Existing groundwater monitoring well MW-1 should be
thoroughly inspected to determine that it is in proper
condition to yield representative samples. Should the
well be deemed unsuitable and unrepairable, the Applicant
should review existing background data for compliance
with 310 CMR 19.132(1)(c) and evaluate the need for a
replacement upgradient well. If unsuitable for sampling,
existing monitoring well MW-1 should be decommissioned.
s=
3) Groundwater monitoring wells MW-20A, MW-20B, MW-19A and
MW-19B should be installed at the approximate locations
shown on the Plan. As discussed during the April 13, 1998
site inspection, groundwater monitoring wells MW-18A and
MW-18B should be .located south of the proposed location
in the most southerly accessible location. Every
precaution should be taken to avoid the area of the Shell
Oil Company pipeline. The Plan should be revised to
indicate the final well locations. Wells should be
installed in accordance with the Department's Publication
#WSC-310-91 entitled "Standard References for Monitoring
Wells", dated April 1991. The revised Plan, well boring
logs and a schedule for commencement should be submitted
to the Department for its records.
a) The downstream surface water monitoring location in the
Canoe River should be selected in the field and
identified on a revised Plan. In addition, surface water
sampling locations should be established on the unnamed
stream with one sampling station located at the stream
crossing at Ware Street and the second sampling station
located at an accessible downstream of the Shell Oil
easement. The unnamed stream locations should be sampled
whenever the stream is flowing at both locations during
regularly scheduled sampling events.
5) Inspection of the Landfill area revealed a wetlands area
approximately 100 yards south of the unnamed stream and
east of the easement which was highly discolored. The
source of discoloration should be investigated to
determine if it is natural in origin or the result of
contamination from the landfill or other source. During
the first scheduled sampling round, the Applicant should
thoroughly inspect the area and collect and analyze a
surface water sample. The results should be reported to
the Department with an evaluation of the results,
discussion of potential causes of the discoloration and
recommendations for further action, if appropriate.
Gi The Applicant shall perform environmental monitoring and
maintain the environmental monitoring system in
compliance with 310 CMR 118: Ground Water, Surface Water
and Gas Monitoring Systems, 310 CMR 19.132: Environmental
Monitoring requirements and 310 CMR 19.133: Maintenance
of Environmental Control and Monitoring Systems. Surface
and groundwater monitoring shall be performed semi-
annually.
7; Pursuant to 310 CMR 19.132, all analytical results should
be reported to the Department within GO days after the
scheduled sampling period. Pursuant to 310 CMR
19, 132 (1) (i) , if the concentrations of any parameters
exceed the state or federal drinking water standards or
Maximum Contaminant Levels (MCL's) then the Department
shall be notified within 14 days of the finding and re-
-5-
sampling performed within 60 days of the prior date of
sample collection.
If Secondary MCL's or Massachusetts Drinking Water
Guidelines as listed in "Drinking Water Standards &
Guidelines for Chemicals in Massachusetts Drinking Waters
or Massachusetts Surface Water Standards (314 CMR 4.00)
are exceeded, then the Department shall be notified
within 14 days of the finding. The Department reserves
the right to require re -sampling should exceedances of
the standards or guidelines in this paragraph be
exceeded.
8) The Application proposes to install landfill gas probes
every 500 to 1000 feet along the perimeter of the
landfill. Within 60 days of this Approval, a plan should
be submitted to the Department indicating the aoproximate
proposed locations of gas probes, a probe detail and
surrounding land uses and water bodies, (ie streams,
wetlands, etc.). The evaluation, spacing and construction
guidelines contained in Chapter 4, Section IIIE of the
May 1997, Landfill Technical Guidance Manual should be
followed. Probes should be installed in accordance with
the Department's approval of the plan. Upon approval and
installation of the gas monitoring system, gas monitoring
must be performed quarterly pursuant to 310 CMR
19.132 (40 (b) I until otherwise approved by the Department.
9) The Applicant should submit a report to the Department
every two years summarizing the results of the
environmental monitoring system.
10) The Applicant shall comply with the post closure
requirements in 310 CMR 19.142(5) and the April 12, 1994
Landfill Closure Certification.
The Department reserves the right to require additional
assessment or action, including but not limited to modifying the
scope or frequency of the environmental program and implementing
appropriate corrective measures in order to maintain the
environment free from objectionable nuisance conditions or threats
to public health, safety or the environment.
This Approval has been issued pursuant to M.G.L., Chapter ill,
Section 150A. Any person or persons who are aggrieved by this
action may appeal pursuant to Chapter 30A. An appeal must be
commenced no later than thirty (30) days from the dace of this
Approval. Notice of an action or proceeding to appeal shall be
served on the Department upon the commencement of the Appeal. In
the event of an Appeal, the Department -reserves the righcto reopen
the permit proceedings, supplement the record and modify the
permit. In addition, the Department reserves the right =o require
additional assessment or action as deemed necessary in order to
protect and maintain the environment free from objectionable
nuisances, dangers and/or threats to public health.
