Problem Definition (use multiple pages if needed) The purpose of this Quality Assurance Project Plan (QAPjP) is to summarize the proposed sampling and analysis strategy, sampling procedures, analytical procedures and other procedures that will be followed to assure that quality data will be collected during the assessment of oil and hazardous material contamination at the Hatheway and Patterson former wood treating facility, and the Clemmey Tank Site located in Mansfield, Massachusetts. Facility operations at the Hatheway and Patterson facility have resulted in the contamination of soil, sediments, surface water, and groundwater in overburden soils and potentially bedrock groundwater. Contaminants of the various media include pentachlorophenol (PCP), fluorochrome-aresenate-phenol (FCAP), fuel oil [polynuclear aromatic hydrocarbons, (PAH)], creosote, and dioxins. Underground and aboveground storage tank construction and coating operations at the Clemmey Tank Site have resulted in contamination of sediments and soil. Groundwater has not been assessed at the Clemmey Tank Site. PAH is the only known contaminate at the Clemmey Tank Site. The purpose of the proposed Site Assessment is to: • delineate the vertical and horizontal extent of contamination to the extent possible, • quantify the concentrations of contamination in the various media, • identify the nature of contaminant migration at the site including potential pathways related to soil, groundwater (overburden and bedrock), surface water sediments and air, • determine if firther clean-up of site contamination is necessary to protect human health, welfare and the environment, or if construction of specific barriers or impediments would sufficiently reduce the risk to human health, welfare and the environment and, • determine the potential for future development of the site as an access route for the Town of Mansfield commuter rail station. The Hatheway and Patterson site is located in a mixed residentiaV'mdustrial area. Operations at the site included wood preserving and the manufacture of wood sheeting, planking, piling, poles, and other wood products. According to historical information, Hatheway and Patterson began operations at the site in 1953 utilizing PCP in fuel oil and creosote to treat lumber. From 1958 to 1974, solutions of PCP in fuel oil and FCAP salts in water were employed. From 1960 to 1984, PCP in mineral spirits was also used. From 1974 through 1984, Hatheway and Patterson utilized solutions of chromated copper arsenate (CCA) salts in water along with PCP in fuel oil. Beginning in 1984, and continuing until the facility closed in April 1993, solutions of CCA salts in water and PCP in water were used at the site. The company filed for bankruptcy in February of 1993. The site is currently inactive and access is restricted by chain link and barbed wire fence and padlocked gates around the property. Title: QAPjP Revision Number: 1 Site Name: Hatheway & Patterson Facility Revision Date: 1/4/99 Site Location: County Street, Mansfield, MA Page: 4 of 39 In October 1989, a Phase II Site Investigation and Preliminary Health and Environmental Assessment Report was prepared by Keystone Environmental Resources, Inc. of Monroeville, Pennsylvania for Hatheway and Patterson Company, Inc. The purpose of the investigation was to delineate the areal extent of soil, sediment, surface water and groundwater contamination at the Hatheway and Patterson facility. The following is a summary of the conclusions reached by the Keystone Environmental Resources, Inc report: "Based on the information generated during this and the 1987 investigations of soil, surface sediments, ground and surface water at the Hatheway & Patterson site, environmental impact from plant operation has been defined to the north, by MW-1 and MW--11, and to the west by MW-4. Deep wells MW-7B and MW-9B in the central area south of the railroad right-of-way indicate vertical migration of the plume. These two well nests lie directly in the direction of groundwater flow from the treatment areas than nests MW-5 and MW-8. Due to the site condition, well MW-10 could not be located as far south as anticipated, so the extent of the plum in this direction is still in question. Well MW-10 does show a lesser degree of impact than wells north of this location (closer to the treatment area). Surface water samples collected from the Rumford River backwash showed slightly elevated levels of PCP, but these levels are below proposed EPA standard drinking water levels. The assumption can be made from the analytical data obtained that the extent of the plume would be within the backwash area to the south of this well. Shallow well MW-8A exhibits concentrations similar to those found in MW-10. Boring B- 16 and B-17 to the east/southeast along the drainage ditch showed little to no contamination. This data would indicate that effects from the old drainage ditch did not reach the groundwater from this area and that the concentrations observed in MW-8A analysis could be from the treatment area, which is diverted to the easilsoutheast by the bedrock outcrop. Due to the efforts that Hatheway & Patterson have taken to preclude the introduction of wood treating constituents from entering the environment, there should be little potential for further impact on soil and groundwater quality by the plant operations. The improvement of the environmental conditions of the Rumford River are attributable to the remedial actions taken by Hatheway & Patterson since 1972. All of these efforts have been performed with the MA DER It In December 1994, a Final Report - Soil Sampling and Field Screening was prepared by the U.S. Environmental Protection Agency (EPA)/Enviromnental Response Team (ERT). The report summarized the results of a surface soil extent -of -contamination survey for the presence of CCA metals using field portable X-ray fluorescence analysis and for PAH and PCP using field screening kits and laboratory analysis. Title: QAPjP Revision Number: 1 Site Name: Hatheway & Patterson Facility Revision Date: 1/4/99 Site Location: County Street, Mansfield, MA Page: 5 of 39 The following is a summary of the EPA assessment results. Area and sample locations are illustrated in the EPA final report. • Areas 3 and 4 had PCP values greater than 5 ppm for both immunoassay test kits and GUMS confirmation. In Area I two grid cells reported values greater than 5 ppm (HPI-N5 and HPI-05) and all samples collected in Areas 2 and 5 had values less than 5 ppm. • Area 1 had three samples that had total PAH concentrations greater than IOppm; two results were from Millipore data (HPI-B5, HPI M5) and one result from GUMS data (HPI N5). One location in Area 2 reported higher than 10 ppm PAHs (HP2 A). In Area 3, two out of three GUMS confirmatory samples were higher than 10 ppm PAHs (HP3 B5, HP3-05). In Area 4, five out of eight sample locations reported values greater than 10 ppm, all sample locations in Area 5 reported values less than 10 ppm. • All samples in Areas 1, 2, 3 and 4 had arsenic values greater than 30 ppm. In Area 5, eight of thirteen samples had arsenic values greater than 30 ppm. • All locations in Area 3 and 4 had chromium values greater than 1, 000 ppm. All locations in Areas 1, 2, and 5 had Cr values of less than 1000 ppm. • All locations in Area 3 and one location in Area 4 had copper values greater than 1000 ppm. All other sample locations yielded values less than 100 ppm for copper. • Lead results of greater than 300 ppm were observed at two locations in each of the following areas: I, 3, 4, and 5; and in three locations in Area 1. In Area 2, all locations reported a value of less than 300 ppm for lead These samples were collected at HP-H5, HPI M5, HP3-B5, HP3-05, HP4-A, HP4-B, HP4-D, HP5-J, 5T-G, C6, C6-3. Statistical analysis of the results and are not included in the QAPjP. In April of 1997, assessment activities were conducted at the Clemmey Tank Site. Soil and sediment samples were submitted for laboratory analysis of RCRA 8 Metals, Total Petroleum Hydrocarbons (TPH), and asbestos. Groundwater samples were not collected. Analytical results of the Clemmey tank soil samples are summarized in the table attached to this appendix. Soil sample locations are illustrated on the Clemmey Tank site plan that is attached to the QAPjP. On June 11 and 25, 1998, the DEP collected groundwater samples from monitoring wells MW-3, MW-4, MW-5A, MW-5B, MW-7A, MWBB, MW-9B, and MW-10. Surface water samples were collected from the area of the County Street bridge upstream of the site, from the Rumford River downstream of the site, from the interceptor trench located by the seep in the Rumford River, and from the Rumford River backwash. Soil samples were collected from the soil near MW--9, the interceptor trench, and a soil sample near the NAPL breakout located in the wetland in the area of MW-12. The samples were submitted for laboratory analysis of EPH, VPH, AcidBase Neutral Extractables (EPA Method 8270), semi -volatile organic compounds, and RCRA 8 Title: QAPjP Revision Number: 1 Site Name: Hatheway & Patterson Facility Revision Date: 1/4/99 Site Location: County Street, Mansfield, MA Page: 6 of 39 Metals. The following samples, MW--S (groundwater), MW-12 (oil), MW-9 (surface soil in the vicinity), Rumford River Downstream (surface water), interceptor trench (solid), and breakout area (solid in vicinity of MW-12), were submitted for dioxin furan laboratory analysis. The DEP has not generated a report for the June 1998 assessment. However, analytical reports and data summary tables will be reviewed as part of the assessment. In addition to previous assessment work, the DEP and U.S. EPA are conducting site assessment activities which include collection of soil samples, river sediment samples, surface water samples, and fish samples. DEP is requesting that EPA conduct additional site assessment activities such as excavation of test pits on the Hatheway and Patterson Facility. If necessary, Resource Controls will adjust the proposed scope of work to prevent redundant assessment activities at the site. Contaminants of concern that have been identified in the soil and groundwater at the site include copper, chrome and arsenic metals, volatile and semi -volatile organic compounds, and polynuclear aromatic hydrocarbons. The individual compounds will be tabulated in the assessment report and compared to Massachusetts Contingency Plan 310 CMR 40.0000 Method 1 groundwater and soil standards. The groundwater and soil standards will be used to characterize the risk of harm posed by oil and hazardous materials at the site. Title:QAPjP Revision Number:1 Site Name: Hatheway & Patterson Facility Revision Date:1/4/99 Site Location: County Street, Mansfield, MA Page: 7 of 39 Appendix D 4. Project Description (use multiple pages if necessary): 4.1 Investigation Summary The purpose of conducting a Site Assessment of the Hatheway and Patterson Facility and the Clemmey Tank Site is to define the vertical and horizontal distribution of contamination in sediments, soil, overburden and bedrock groundwater. Field screening and laboratory analytical data will be collected from the various media to quantify concentrations of pentachlorophenol (PCP), fluorochrome-aresenate-phenol (FCAP), fuel oil (PAH), and creosote. Proposed investigation methodologies to complete the Site Assessment of the Hatheway and Patterson Facility include, but are not limited to, the following: • Collection of sediment and soil samples using the Geoprobe soil sampling technique and hollow stem auger drilling and split spoon soil sampling technique. • Collection of groundwater samples from proposed overburden monitoring wells. • Collection of groundwater samples from the bedrock aquifer from proposed bedrock monitoring wells. • Review of Massachusetts Department of Environmental Protection (DEP) Geographical Information Systems (GIS) Maps to identify areas of critical environmental concern including, but not limited to, endangered species or critical habitats for flora and fauna in the vicinity of the site. • Review of historical and proposed assessment data to evaluate the potential for contaminant migration. Assessment of the Clemmey Tank Site will not include collection of Geoprobe soil samples or installation of bedrock groundwater monitoring wells. To improve the efficiency of the Hatheway and Patterson Facility Site Assessment and data review, the site will be divided into four quadrants based upon proximity to the Rumford River and the railroad tracks. Quadrant one is the northeast area, quadrant two is the southeast area, quadrant three is the southwest area, and quadrant four is the northwest area of the site. Sample locations will be determined based upon information provided in previous site investigation reports and site conditions observed during assessment activities. Massachusetts DEP and U.S. EPA are currently conducting assessment activities to delineate dioxin contamination at the site. Assessment has included collection of surface water, sediment and fish samples from the Rumford River and Fulton Pond for laboratory analysis of dioxins, polychlorinated biphenyls (PCB), volatile organic compounds (VOCs), semi-VOCs, Title:QAPjP Revision Number:0 Site Name: Hatheway & Patterson Facility Revision Date:114/99 Site Location: County Street, Mansfield, MA Page: 8 of 39 pentachlorophenol, pesticides, and RCRA 8 Metals. Because of the comprehensive dioxin assessment being conducted by DEP and EPA, Resource Controls will not collect samples for dioxin analysis. 4.2 Geoprobe Soil and Groundwater Sampling On the Hatheway and Patterson Facility, approximately 50 locations will be identified for collection of soil samples using the Geoprobe soil sampling technique. The proposed Geoprobe soil sample locations are illustrated on the attached site plan. Sample locations will be adjusted as field conditions and field screening results dictate. The Geoprobe soil sampling method provides continuous soil samples from ground surface to the depth of the boring. The soil samples are collected inside acetate liners inside steel sampling tubes which are driven to depth using a vibration/percussion hammer. Each Geoprobe soil sample location will include collection of soil samples in four foot increments from the ground surface to the water table. The water table interface is anticipated to range from 8 feet to 12- feet below ground surface. Each soil sample will be screened for the presence of volatile organic vapors using a PID and the jar-headspace technique. Each Geoprobe location will have a minimum of one soil sample submitted for laboratory analysis of Extractable Petroleum Hydrocarbons with PAH using DEP Method 1.0, one sample submitted for Copper (Cu), Chromium (Cr), and Arsenic (As) metals, and one sample submitted for semi-VOCs using EPA Method 8270. The Geoprobe soil sample which is submitted for laboratory analysis will be determined by the highest concentration of volatile organic vapors detected using the PID. If no organic vapors are detecteg the sample will be taken from stained soil, soil which exhibits odor, or the sample collected from immediately above the water table. Several Geoprobe sample locations in Quadrant 2 will have two samples collected from the location. One sample will be collected approximately half way between ground surface and groundwater, and one sample will be collected from immediately above the water table interface. The analytical results will be compared with previous surface soil assessment information to determine if the soil between the surface and the water table interface is contaminated Sample locations that will have two samples collected are identified on the plan by the #2. 4.3 Test Pit Excavation Due to financial limitations and potential EPA assessment activities, test pit excavation will not be conducted during the assessment. Title:QAPjP Revision Number:1 Site Name: Hatheway & Patterson Facility Revision Date:1/4/99 Site Location: County Street, Mansfield, MA Page: 9 of 39 4.4 Overburden Monitoring Well Installation Approximately nine overburden monitoring wells will be installed on the Hatheway and Patterson Facility. Additional monitoring wells may be installed at the Hatheway and Patterson Facility based upon site conditions. Four monitoring wells will be installed on the Clemmey Tank Site. Approximately four of the monitoring wells installed on the Hatheway and Patterson Facility will be excavated to the depth of bedrock to determine bedrock topography. The boreholes will be grouted within two feet below of the bottom of the monitoring well to prevent vertical migration within the aquifer. Monitoring well depth will be dependent upon depth to bedrock, drilling conditions and field screening for the presence of contamination. Soil boring equipment will be steam cleaned between borings. Split spoon soil samples will be collected continuously from ground surface to 10-feet below ground surface and at five foot intervals thereafter. Each soil sample will be placed in a clean eight -ounce glass jar for headspace analysis. The jar will be filled half way and tightly covered with aluminum foil. Each soil sample will be screened for the presence of volatile organic vapors using a Photovac Photoionization Detector (PID) with a 10.6 eV lamp calibrated to a isobutylene standard of 100 parts per million per volume (ppmv). Soil lithology, field screening results, and monitoring well construction details will be logged in the field by a Resource Controls field technician. Boring logs will be submitted with the final Site Assessment Report. The monitoring wells will be constructed of two-inch diameter, thread -coupled, Schedule 40 PVC materials with 10-foot lengths of machine cut, 0.010-inch slotted well screen placed across the water table interface. Several wells will be screened at depths below the water table interface to provide groundwater samples at depth in the overburden aquifer and to monitor vertical groundwater migration. 4.5 Bedrock Monitoring Well Installation Two monitoring wells will be installed into bedrock at the Hatheway and Patterson Facility to provide bedrock aquifer monitoring and sampling points. Each boring will be advanced to the overburden/bedrock boundary using the hollow stem auger drilling method. The borings will be advanced into bedrock using percussion air hammer drilling method through fractured bedrock (approximately five to ten feet) until competent bedrock is encountered. Four -inch diameter carbon steel casing will be inserted to the bottom of the boring. A three-foot bentonite seal will be grouted inside of the casing. The annulus of the casing will be tremmie-grouted with bentonite from the bottom of the casing to five feet below ground surface to provide a seal between the bedrock and overburden aquifers and to prevent vertical migration in the annulus in the overburden aquifer. The remaining five feet of annulus will be cemented to form a seal from surface water runoff. After allowing the grout and cement seals to set overnight, the monitoring Title:QAPjP Revision Number:0 Site Name: Hatheway & Patterson Facility Revision Date:1/4199 Site Location: County Street, Mansfield, MA Page: 10 of 39 wells will be advanced approximately ten -feet into competent bedrock using the coring drilling method. Depending upon bedrock competency, the wells may either be left as open hole monitoring wells or completed with two-inch diameter thread -coupled, Schedule 40 PVC materials with 10-foot lengths of machine cut, 0.010-inch slotted well screen and #1 well gravel in the annulus. No bedrock monitoring wells are proposed for the Clemmey Tank Site. 4.6 Monitoring Well Development Upon completion of installation, each monitoring well will be developed using bailing and surging. Approximately five well volumes will be bailed from each well to ensure hydraulic connection between the monitoring well and the aquifer. 4.7 Groundwater Elevation Survey Subsequent to installation of monitoring wells on the Hatheway and Patterson Facility and the Clemmey Tank Site, existing and proposed monitoring well top -of -casing elevations will be surveyed to an arbitrary datum of 100 feet. Monitoring well locations will be surveyed using stadia/angle reference to known site features. Subsequent to the survey, each monitoring well will be gauged using an oil/water interface probe to measure depth to groundwater and non -aqueous phase liquid (NAPL) thickness. Groundwater elevation data will be plotted on a site plan and lines of equal piezometric head elevation will be plotted to produce a Water Table Contour Plan illustrating groundwater flow direction and hydraulic gradient. Product thicknesses will be plotted on a site plan and lines of equal product thickness will be plotted to produce a NAPL Isoconcentration Plan. 4.8 Soil Sampling During soil boring activities, soil samples will be collected by either split spoon soil samplers or macro -core direct push samplers using standard ASTM penetration test methods. The depth and location of all soil samples will be noted on soil boring logs. Soil samples will be screened for the presence of volatile organic vapors using a PID and the jar- headspace technique. Soil samples exhibiting the greatest concentration of organic vapors, staining, or odors which may indicate the presence of contamination, will be submitted for laboratory analysis. Laboratory analyses for various soil samples will be based upon soil sample locations and conditions. Proposed analytical methods that will be utilized during the Site Assessment are summarized in Table 1. Title:QAPjP Revision Number:1 Site Name: Hatheway & Patterson Facility Revision Date:1/4/99 Site Location: County Street, Mansfield, MA Page: 11 of 39 4.9 Groundwater Sampling Subsequent to monitoring well elevation survey and gauging, groundwater samples will be collected from the proposed monitoring wells for laboratory analysis. Groundwater samples will not be collected from monitoring wells which contain a measurable thickness of NAPL. Prior to sample collection, groundwater conductivity, temperature and pH will be monitored. Each well will be purged using clean, dedicated polypropylene bailers. Groundwater will be monitored periodically during purging until groundwater conductivity, temperature and pH stabilize to within 10% of the original monitoring results, or a minimum of three well volumes have been evacuated. Purge water will be discharged pursuant to MCP 310 CMR 40.0045. The samples will be collected using the polypropylene purge bailers. Proposed analytical methods that will be utilized during the Site Assessment are summarized in Table 1. On June 11 and 25, 1998, the DEP collected soil, river sediment, surface water, groundwater, and NAPL samples for laboratory analysis. The analytical reports are discussed in Appendix C. Because the DEP sampling event took place recently, Resource Controls will be referring to the DEP data and will not collect additional samples from the existing groundwater monitoring wells. 4.10 Sample Handling Protocol Samples will be collected in containers provided by the laboratory for specific analytical parameters. Preservatives for aqueous and non -aqueous samples will be added by the laboratory performing the analysis. Aqueous and non -aqueous samples will be collected in accordance with the procedures required for the specific analytical methodology. A chain -of -custody form will be filled out completely and signed by the sampling personnel in the field. The original form will accompany the sample containers at all times and must be signed and dated each time the sample is relinquished to another person until the sample reaches the laboratory and the analysis is completed. The sample containers will be labeled with the following information: Project Name (or number) and address Date and time of sample collection Sample Location Analysis to be performed Sample identification Title:QAPjP Revision Number:0 Site Name: Hatheway & Patterson Facility Revision Date:1/4/99 Site Location: County Street, Mansfield, MA Page: 12 of 39 The chain -of -custody form shall contain, at a minimum, the following information: Project name, address and number Date and time sample is collected Sample location identification Analysis requested and preservative (if required) Number of containers Analytical results turn -around time frame Deliverables package requested Sample type Signatures of sampler and other personnel that acquire/relinquish custody of the samples 4.11 Identification of Areas of Critical Environmental Concern Resource Controls will submit a request to the DEP for an Massachusetts Geographical Information Systems (Mass GIS) Map illustrating areas of critical environmental concern. In addition, during site assessment activities, Resource Controls personnel will observe site conditions to confirm the results of the Mass GIS information. 4.12 Reporting Upon completion of the field investigation, receipt of analytical reports and Mass GIS information, Resource Controls will prepare a Site Assessment Report which summarizes field screening results, sampling methodologies, sample results, areas of critical environmental concern, and an evaluation of the potential for contaminant migration. The Site Assessment Report will provide a summary of field screening and sample collection protocols. Field observations will be presented in the tent of the report. Laboratory analytical results and field screening data will be tabulated in the text of the report and laboratory reports attached as appendices. The report will include a Site Locus Map indicating the location of the Site, nearby geographical features and surface drainage; a Water Table Contour Map illustrating water table elevations of equal piezometric head elevation; and a NAPL Isoconcentration Plan illustrating the distribution of NAPL using lines of equal product thickness. 4.13 Risk Characterization Upon completion of the Site Assessment and Site Assessment Report, characterization of the risk to health, safety, public welfare and the environment will be conducted pursuant to the MCP 310 CMR 40.0900. Risk characterization is used to establish whether a level of no significant risk exists or has been achieved at a disposal site. The criteria used in this determination are described in 310 CMR 40.0900. The two basic approaches are, a chemical specific approach which Title:QAPjP Revision Number:1 Site Name: Hatheway & Patterson Facility Revision Date:1/4/99 Site Location: County Street, Mansfield, MA Page: 13 of 39 compares site concentrations to standards in soil and groundwater or, a cumulative risk approach which compares site specific information to a cumulative cancer risk limit of an excess lifetime cancer risk of one -in -one hundred thousand. The results of the Risk Characterization shall be included as an appendix to the final Site Assessment Report. Sampling Design (use multiple pages if needed) Historical use of the site as a wood treating facility has resulted in widespread contamination of sediments, soil, groundwater and surface water. The Site Assessment will investigate the vertical and horizontal distribution of contamination, to the extent possible, in the following media in the four Quadrants (Site Plan) of the Site: • Sediments • Soil • Overburden groundwater • Bedrock groundwater Data collected during Geoprobe assessment will assist in the placement of additional soil borings and monitoring wells. Soil screening and laboratory analysis conducted during soil boring activities will provide information regarding the vertical extent of soil contamination from immediately below ground surface to bedrock. Installation of monitoring wells in the overburden and bedrock aquifers will provide hydrogeological monitoring information regarding the presence of NAPL, horizontal and vertical groundwater flow, hydraulic communication between the bedrock and overburden aquifers, and hydraulic gradient. Groundwater samples will provide laboratory analytical data concerning dissolved phase contamination in the overburden and bedrock aquifers. Analytical procedures chosen for the Site Assessment were determined based upon historical use of the site and previous assessment activities. Should the need for additional analytical procedures be observed, a determination will be made by the Resource Controls Management Team to collect additional samples for specific laboratory analysis. ooLc ir bx•aUCiGaymdQNte.zQUyn±ykOt d .F.v'pa•.•CUib;_UCp?ayd.3i o`: 'bo"0 oti.aqo-r Uxx'.a-3U 1t--di o IT00 v 0A0o.H3U0V2 0 0 MzU U Uj Z e o to Cc > a 0002 ^ 0cl VN 00 N 00 m O N N N N N N-4td NpnU U U V] CdmN N m V aM NN MN N 'O 0 CA o C N `-' N V N Ca4 00 ti U ZI C via W > RUC] W >UC1 & § 0 k 72 © 0010 cl ]§§> 2% ])3A)ou» /\E\]§/ o=§,to \o00 ƒƒ IfJ/b \G)\\\E 2\\§\6 .\ .p a ae q a « \ \4 )) \ cq } ) \/ 0 ƒ / \ \ \ \ \ ) §2 4 4 7 \ § K a §2 \\ \ ]\ _ \7\\ \ -» ® B\/U.0 u I met G§eek 2j(to \2E%« )=f _-.. § m§]( rL Mae»ƒ ]/2GQ §13kG r. 40. \)\/ e 3/ U ƒ o 2a)& gcn /23}\W Title: QAPjP Revision Number: 1 Site Name: Hatheway & Patterson Facility Revision Date: 1/4/99 Site Location: County Street, Mansfield, MA Page: 18 of 39 Appendix G 6. Sample Handling and Custody Requirements (use multiple pages if needed) All samples collected for laboratory analysis will be traced from collection, to shipment, to laboratory receipt under Chain -of -Custody protocol. The original form of the Chain -of -Custody is to accompany the sample containers at all times and must be signed and dated each time the sample is relinquished to another person until the sample reaches the laboratory and the analysis is completed. Each sample container will be labeled with the following information: • Project name and number • Sample location • Collection time and date • Analysis to be performed • Sample identification • Name of sampler • Preservative (if applicable) An example of a Chain -of -Custody form is attached. The original Chain -of -Custody will accompany the samples until the project is complete. Original Chains -of -Custody will remain in the permanent project file. Copies of the Chains -of -Custody will also remain with the project manager, the laboratory manager and attached to the laboratory analytical reports. Field screening samples will be labeled in the field by the field scientist. Field screening results will be recorded in a log book by the field geologist and copies of the field notes will be maintained as part of the project permanent file. Field notes will include the following: • Project name and address • Date and time sample was collected • Sample location identification • Field screening results • Site conditions at time of sample Data Management and Documentation (use multiple pages if needed) All field activities will be recorded by field personnel in permanently bound notebooks with individually numbered pages that are maintained for all field activities. Field notes will be copied daily upon completion of field activities and will be maintained in the permanent site file. Field logbooks will be archived at Resource Controls, 474 Broadway, Pawtucket, Rhode Island and/or the Mansfield Field Office located at 374 North Main Street in Mansfield, Massachusetts. All entries in the notebooks are made with permanent ink, and corrections are made using a single line through the mistake with the initials and date of the individual who made them. Entries include sampling location, time, date, weather conditions, subcontractors and equipment on -site, site visitors, work to be conducted, parameters to be measured, and any problems encountered during field activities. Analytical samples are recorded in a permanently bound laboratory notebook, specific for each instrument. A brief narrative is included at the end of each analytical run that includes any problems encountered with the standards, samples, blanks or QC samples. Some of the automated analytical equipment (GC and GC/MS) have computer generated analytical runs and any problems associated with the analytical runs are flagged and noted on the computer reports. If any corrective action is taken it will be noted in narrative in the instrument notebook (see the laboratory QAP for more detail). The following deliverables will be provided by the laboratory: • Sample data results sheets similar in format to a CLP Form I for all field sampling including PE sample results, equipment/rinsate blanks and trip blanks. • Surrogate recoveries and acceptance limits. • Matrix spike/matrix spike duplicate results and acceptance limits. • Duplicate sample results. • Method/reagent blank results. • Calibration standards/Reference Standards/LFB reports. • A summary or narrative of any deviations from the QC criteria as well as observations about the samples (i.e. missed holding times, trip blank contamination, and reference standards or check standards outside of criteria). The following deliverables will not be required, but must be maintained by the laboratory and may be requested at a later date: All raw data including chromatograms. Copies of Instrument logbooks. Copies of internal chains -of -custody. All reports are generated in hard copy form. A computer with sample results can also be provided. Title: QAPjP Revision Number: 0 Site Name: Hatheway & Patterson Facility Revision Date: 1/4/99 Site Location: County Street, Mansfield, MA Page: 35 of 39 Appendix O 10. Assessment and Response Actions (use multiple pages if needed) The project manager will be responsible for scheduling an audit of the field sampling team on the first day of sampling. This systems audit will ensure that the field sampling team is following documented procedures and that the sample collection is proceeding according to the QAPjP and will meet the project objectives. Copies of the report will be sent to the site project manager, the EPA project manager and maintained in the site files. Any deficiencies found during the field audit or problems encountered during this investigation require corrective action. The site project manager will be notified immediately if any problems are encountered in the field. Minor adjustments to the sampling design, such as moving a soil boring several feet to avoid an obstruction, may proceed with notification of the project manager and documentation in the field notebook. Major corrective actions, such as encountering buried drums, require immediate notification of the EPA project manager prior to proceeding with the drilling. The laboratory must maintain its certification with the State of Massachusetts throughout the course of the project. Analytical QA/QC will rely upon system audits, performance evaluation audits, and internal audits currently in place at the laboratory that will be analyzing samples collected from the Hatheway and Patterson Facility and the Clemmey Tank Site. A system audit is an on -site review of the laboratory's quality assurance practices and operating procedures. The laboratory has agreed to audits by various government agencies and clients when requested. Performance audits provide a systematic check of the quality of the laboratory data and of the measurement systems. The laboratory participates in blind laboratory performance evaluations for the EPA as part of the water supply and water pollution programs and for the State of New York, running their proficiency sample twice a year. The laboratory participates in the American Industrial Hygiene Association's Environmental Lead Proficiency Analytical Testing program for the analysis of lead in paint, soil and dust wipes. Internal audits are performed on a quarterly basis or as needed in the event an out -of -control situation is discovered. NCR reports are initiated and completed as a result of any audit findings. Follow up of out -of -control situations is performed until the analyses is determined to be in control. Internal audits are kept on file by the QA/QC Manager. Analyte concentrations will be checked against contaminant levels in equipment/rinsate blanks and method/reagent blanks to identify possible sources of false positives. A review of all QC analyses will be performed to assess the data quality. Any encountered deviations not documented in the narrative will be rectified with the laboratory. All documented QA/QC system audits, performance evaluation audits and internal audits per specifically for the Hatheway and Patterson Facility and Clemmey Tank Site project will be included as attachments to the final analytical reports Title: QAPjP Revision Number: 0 Site Name: Hatheway & Patterson Facility Revision Date: 1/4/99 Site Location: County Street, Mansfield, MA Page: 38 of 39 Appendix R 13. Data Usability (use multiple pages if needed) Environmental assessment activities at the Hatheway and Patterson Facility and the Clemmey Tank Site are being conducted as part of the "Brownfields Economic Redevelopment Initiative" that is coordinated by the EPA. The purpose of conducting a Site Assessment of the Hatheway and Patterson Facility and the Clemmey Tank Site is to define the vertical and horizontal distribution of contamination in sediments, soil, overburden and bedrock groundwater. Field screening and laboratory analytical data will be collected from the various media to quantify concentrations of pentachlorophenol (PCP), fluorochrome-aresenate-phenol (FCAP), fuel oil (PAH), and creosote. Site assessment methods are further described in Appendix D. The data validation and usability process outlined in this section will be conducted to ensure the objectives of the assessment are met and that all aspects of sample collection and analysis are competent and reliable. To ensure the proper sample collection procedures are being adhered to during assessment activities, the project manager will schedule an audit of the field sampling team on the first day of sampling. This systems audit will ensure that the field sampling team is following documented procedures, that the sampling collection is proceeding according to the QAPjP, and will meet the project objectives. Any deficiencies found during the field audit or problems encountered during this investigation require corrective action. The site project manager will be notified immediately if any problems are encountered in the field. Minor adjustments to the sampling design, such as moving a soil boring several feet to avoid an obstruction, may proceed without notification of the project manager and documentation in the field notebook. Major corrective actions, such as encountering buried drums, will require immediate notification of the project manager prior to proceeding with the drilling. The final Site Assessment Report will include sections summarizing the methods and results of data validation and usability conducted by Resource Controls. Data validation will be performed to ensure laboratory analyses were conducted pursuant to prescribed analytical procedures included and/or referenced in the QAPjP. The data validation procedure will include checking the laboratory data for completeness. Resource Controls will check the data package to ensure that all the required documents and forms are included and in order. Performance evaluation sample results (i.e. trip blanks, field blanks and duplicates) will be evaluated to assess potential usability issues. Title: QAPjP Revision Number: 0 Site Name: Hatheway & Patterson Facility Revision Date: 1/4/99 Site Location: County Street, Mansfield, MA Page: 39 of 39 The second step in the data validation process will consist of evaluating the results of all quality control checks and analytical procedures documented by the laboratory. Items such as holding times, sample preservation techniques, QC sample results, etc., will be assessed. Laboratory and instrument performance will be checked by reviewing items such as laboratory blank contamination and instrument calibration. Potential problems originating from field sampling work will be assessed by examining field duplicates, equipment blank, and trip blank results. The third step in the data validation process will be examination of the laboratory analytical results to verify the accuracy of the results. The data will be examined to check for calculation, compound identification, and/or transcription errors. The purpose of the data validation is to assess and summarize the analytical quality and defensibility, document all factors contributing to "analytical error" that ultimately affect data usability, and provide information concerning the effectiveness of analytical methods and effectiveness. Upon completion of data validation, data usability will be summarized in a separate section of the Site Assessment Report. Data usability will summarize potential usability issues that were observed in the data validation process. This includes potential impact of "measurement error" on the application of the results on the final assessment of the site and whether the "measurement error" if any, will have a major or minor impact on data usability. Included in the data usability section will be a summary that identifies each quality control parameter that did not meet criteria. If possible, the cause for error, field sampling error or laboratory error, will be identified and described. The summary will identify the affected samples, the analytical or sample collection problem (if identifiable), and the recommended response for use of the data in the site assessment. Upon conclusion of assessment activities, field screening and laboratory analytical results will be summarized in a Site Assessment Report. In the report, the laboratory analytical results will be presented in tabulated format and compared with MCP Method 1 soil and groundwater standards. The data will also be used to characterize the risk of harm to health, public welfare and the environment based on the MCP Methods for Characterization of Risk of Harm Section 40.0940.
