Dear Jane,
Per our e-mail conversation earlier this week, I am submitting documents detailing my schooling
and a $10 fee by check to request a license to practice massage in the Town of Mansfield.
I am currently licensed in Rhode Island and I am also Nationally Certified and have my First
Aid/CPR certification. I completed my schooling prior to getting married, which is why you will
see two different last names.
You may send documents to my address above, and if you need any other supporting
documentation please contact me.
