2. Extension of Notice of Intent/Reconstruction of Maple Street: Richard C. Alves, Town of
Mansfield Engineering Division, Maple Street, within Town Right -of -Way, Map 30, 38 & 39 —
Utility upgrades to include improvements to the existing water main and install electric conduit
and manholes on Maple Street from the intersection of Bird Road westerly to Cate Springs.
Order was amended in April 2016 and was extended to January 13, 2020. New extension date
will be January 13, 2023. DEP SE 211-0918
3. Request for Certificate of Compliance: Andres Gazzolo, Trustee of Gazzolo Realty Trust & Elly
Realty Trust, 30 Reservoir Street, Map 36, Parcel 1. Nextel Communications - Proposed
monopole on an existing pad, upgrade the existing site utilities and the placement of a shelter
within the proposed compound within the 100 ft. buffer of bordering vegetated wetlands-DEP
SE 211-0787
4. Request for Certificate of Compliance: John Walsh, 54 Willow Street, Map 28, Parcels 11, 12,
118, 150. Construction of subdivision roadway with associated drainage, water and utilities
within 100 feet of Bordering Vegetated Wetlands and Inland Bank. DEP SE 211-0931
