Box 177 Release of Hazardous Material, Mansfield, Massachusetts 02048 15_.,County Street, Mansfield, MA Approval,_. Phase II Scope of Work'_) M. G. L. Chapter 21E ATTENTION: William E. Haynes, President Gentlemen: The Department of Environmental Quality Engineering, Division of Hazardous Waste, has reviewed an October 1988, Phase II site assessment scope of work on the above -referenced site titled "Hathaway and Patterson Phase II Site Investigation," prepared on your behalf by Keystone Environmental Resources, Inc. The Department approves this scope of work with the following provisions: 1. All work as outlined in the scope of work, the site assessment report, the risk assessment, and the evaluation of remediation alternatives (page 3-8) will be completed and submitted to the Department within seventy-five (75) days or your receipt of this letter; 2. Cross section(s) showing the overburden thickness and lithologies (based upon detailed sample descriptions) including -layering, the bedrock, and the horizontal and vertical (if any) components of ground water flow shall be included with the site investigation report; 3. Plume maps shall be drawn with the relative contaminant concentrations, based upon the sampling results, contoured; and 1 4. While MW-10 should show whether or not contaminants are entering the wetland and Rumford river backwash areas to the south of the site, this southernmost monitoring well location is about 560 feet southwest of MW-6, which had high levels of xylenes and PAH's, and ma not provide enough data to delineate the plume south of MW-6 y and the bedrock outcrop. Therefore, you shall install an additional monitoring well between MW-7 and MW-9 or submit supporting information to justify the exclusion. -2- This site is now subject to the Massachusetts Contingency Plan (MCP) 310 CMR 40.000 which took effect on October 3, 1988. The MCP defines the purpose, scope, and requisite activities in successive phases of investigation and remedial action. All further work at this site, including the Phase II Comprehensive Site Assessment (310 CMR 40.545(3)) being done according to this scope of work and the Phase III Development of Remedial Response Alternatives and Final Remedial Response Plan (310 CMR 40.546) will have to be completed according to the guidelines set in the MCP and the Draft Interim Guidance Document for doing risk assessments, both available at the State House Bookstore in Boston. Please keep the Department informed as to the dates and times for all work scheduled and in progress, so that site visits can be arranged as needed. If you should have any questions regarding this matter, please contact Patricia Billingsley of this Department. In any correspondence to this office, please refer to DEQE case number 4-571.
