I have enclosed a draft copy of the questionnaire we are proposing to use for the well
Survey. Will you please review the questionnaire and call me with your comments?
I have also enclosed a draft vicinity map. We are proposing to determine if any of the
house lots abutting the Rumford River have drinking water wells. Our survey will include
the homes along Highland Avenue and Morrow Street. It will also include a limited
number of homes along Chauncy Street.
Thank you for your assistance and your offer to help us with any uncooperative home
owners. Once the survey has been completed, we will provide you with a copy of our
findings.

of Health, the DEP and the Hathaway & Patterson Company of 15 County Street are conducting a
survey to identify any private water supply wells in the area between Chauncy Street and County
Street. We are performing the survey as part of an environmental investigation. Please complete
