Boelitz DEP - SOUTHEAST REGION Main Street (Rte 105) Lakeville Hospital Lakeville, MA 02346 (508) 946-2862 RE: Hatheway & Patterson (DEP Site Number 4-0571) Dear Mr. Boelitz: As requested, Penny Engineering has revised our private well survey report to contain_ a list of all theresidences contacted:-) The survey was conducted iat residences downgradient of Me Hatheway & Patterson facility as shown in Figure 1`The survey was conducted during September and October by delivering 20 questionnaires (see attached sample questionnaire) to all residences that abut the Rumford River or the Rumford River Backwash as listed in Table 2. On November 4, 1992, after submitting our October 27, 1992 report, we received a late questionnaire, which brings the total number of respondents to 13. All 13 respondents reported that they are connected to town water and only one had a private well (see Table 1). Reportedly, the private well was installed in 1988 with the intention of using it to water the lawn. It has never been used. A copy of the late questionnaire is enclosed.
