Box 177 Mansfield, Massachusetts 02048 ATTENTION: William E. Haynes President Gentlemen: January 22, 1991 RE: MANSFIELD--WSC/SA-4-0571 Hatheway & Patterson 15 County Road, Approval of Short Term Measure, M.G.L. c. 21E, 310 CMR 40.000 The Department of Environmental Protection, Bureau of Waste Site Cleanup (the "Department"), has reviewed a design for a Short Term Measure (the "STM Design") submitted on your behalf (as used in this letter "you" and "yours" refer to Hatheway & Patterson Co., Inc.) by Keystone Environmental Resources, Inc. on January 1, 1991, titled, m DESIGN REPORT FOR SHORT-TERM MEASURE. S The STM Design includes a schedule and a plan to install, operate and monitor an active product recovery system along the east bank of the Rumford River. The Department approves the STM Design, with the following provisions: 1. The STM will comply with 310 CMR 40.542. 2. You shall meet the requirements relative to Short Term Measures listed on pages one (1) to three (3) of the Department's letter of September 6, 1990. - 3. You will comply with provision k. on page three (3) of the Department's letter of September 6, 1990, relative to monitoring wells that have been destroyed, damaged or improperly completed. This provision requires you to replace monitoring well MW-8A which was reported broken in your STM Design and any other wells that have been destroyed or damaged. The Department requires you Recycled Paper -2- undertake this task and submit completion logs no later than March 29, 1991. 4. The hydrogeology compiled to date in the vicinity of the recovery system is incomplete and in some respects inconsistent, suggesting surveying or gauging inaccuracies. Surveying vertical closure must be within 1/100th of a foot, and gauging must achieve the same level of accuracy. Piezometric contours must honor the gauging data. In reporting piezometric head and surface water elevations, well head elevations, depth to water and water elevations must be tabulated. Prior to undertaking the Short Term Measure, you will survey and gauge monitoring wells P-3, P-4, P-5, MW-2, MW-3, MW-5A, MW-6, MW- 7A, NW-10 and MW-12, and the upstream and downstream Rumford River gauging stations, all to the same sea -level datum. The results shall be tabulated and an areal flow net shall be compiled that encompasses the area occupied by these wells and by the active recovery system. The anticipated zone of influence of the product recovery system shall be outlined on the flow net. You will submit these results to the Department no later than March 29, 1991. 5. on completion of the installation of the product recovery system, you shall survey the head of the two observation sumps and of the product recovery sump to sea -level. You will test the product recovery system and gauge the system sumps and surrounding monitoring wells over a range of discharge rates to determine the rate sufficient to capture all the product while minimizing recharge from the Rumford River. You will document this determination with a flow net of the recharge area of the activated recovery system. 6. You will submit a report on the Short Term Measure as -built within sixty (60) days of the activation of the product recovery system. 7. Installation of the product recovery system will begin no later than April 15, 1991, and the recovery system will be activated no later than May 31, 1991. The Department requires a written response within fourteen (14) days of receipt of this letter indicating whether you intend to take the actions outlined above. You should be aware that the implementation of a Short Term Measure does not relieve you from undertaking a full Phase II_ - Comprehensive Site Assessment. In addition, you may have to prepare a Phase III - Development of Remedial Response Alternatives and a Final Remedial Response Plan and you may have to undertake a Phase IV - Implementation of the Approved Remedial Response Alternative, if required under 310 CMR 40.000. Please be advised that you must comply with all applicable Federal, State and local permits throughout the implementation of the STM. -3- You have been previously notified of your liability in the Notice of Responsibility letter dated August 16, 1988. Your cooperation in this matter is appreciated. Should you have any further questions regarding site WSC/SA-4-0571, please contact Marcel V. Boelitz of this office at (508) 947-2868.
