On July 20, 2006, a representative of MassDEP conducted a single -media inspection of the Hub
Folding Box Company facility located at 774 Norfolk Street, Mansfield, Massachusetts_
The purpose of the inspection was to determine the status of Hub Folding Box Company relative to
compliance with the Massachusetts Air Pollution Control Regulations, as contained in 310 CMR 7.00,
adopted under the provisions of Sections 142A-E and 1421 of Chapter 111 and Sections 4 and 6 of Chapter
21 C of the Massachusetts General Laws, as applicable.,
At the time of the inspection, there were no violations observed relative to the Air Pollution Control
Regulations as cited above.
Be advised that it is the facility's responsibility to maintain current awareness of, and compliance
with, the above -referenced environmental laws and regulations of the Commonwealth.

Dear Mr. Flavin:
On November 8, 2007, a representative of the Department of Environmental Protection
(MassDEP) conducted an multi -media inspection of the Hub Folding Box Company facility located
at 774 Norfolk Street, Mansfield, Massachusetts.
The purpose of the inspection was to determine the status of Hub Folding Box Company
relative to compliance with the Massachusetts Hazardous Waste Regulations, as contained in 310
CMR 30.000 adopted under the provisions of Sections 4, 61 and 9 of Chapter 21C of the
Massachusetts General Laws as applicable, the Massachusetts Air Pollution Control Regulations,
as contained in 310 CMR 7.00, adopted under the provisions of Sections 142A-E and 142J of
Chapter 111 and Sections 4 and 6 of Chapter 21 C of the Massachusetts General Laws as applicable
and the Massachusetts Division of Water Pollution Control Regulations, as contained in 314
CMR 1.00-12.00, adopted under the provisions of Chapter 21 of the Massachusetts General Laws
as applicable.
At the time of the inspection, there were no violations observed relative to the Air Pollution
Control Regulations, Hazardous Waste Regulations and the Water Pollution Control Regulations,
as cited above.
