-On--December 8, 2005, you were notified —that tht--i,4ass,,chwetts' -Department of
Environmental Protection, Bureau of Waste Site Cleanup (the Department) had begun to audit
response actions conducted to address the release of petroleum products at the location identified
above. In particular, the audit focused on the Class A-2 Response Action Outcome (RAO) filed
for the site. This notice informs you of the results of the Department's audit.
DETERMINATION
The Department has determined tltat there were no significant violations of applicable
requirements of the Massachusetts Contingency Plan (MCP). 'Therefore, no additional actions
are necessary. The Audit Memorandum (Attachment A) describes the activities Department
personnel performed during the audit and summarizes relevant site information.

AUDIT MEMORANDUM
The audit consisted of the following activities:
• AD examination of documents within the Department's records.
• Preparation of a Notice of Audit dated December 8, 2005.
• A site inspection on December 13, 2005,
• Preparation of this Notice of Audit Findings.
II. SITE SUMMARY
Audit Memorandum
The site, which is a residential property, is located in the eastern portion of Mansfield,
approximately 600 feet southeast of the intersection of Fast Street and RT 106, Presently, and at
the time of the release, the on -site dwelling was occupied. The dwelling is abutted by a lower
lying wetland area and a tributary of the Canoe River to the east and south, Fast Street to the west
and a residential property to the north. The site is located in a Zone 11, an Area of Critical
Environmental Concern (ACEC), and a medium yield Potentially Productive Aquifer (PPA). No
inforuratiorr-concerning the -presence -of private-or-publie-drinking water -supply -wells was
discovered during the completion of assessment activities.
The subject release occurred on April 8, 1998 due to a delivery of 125 gallons of heating fuel
that was inadvertently pumped into an on -site irrigation well via the Above Ground Storage Tank
(AST) fill receptacle. The irrigation well was located in the back yard, near the Canoe River
tributary stream located on the eastern property boundary and extended to approximately
eighteen (18) feet Below Ground Surface (BGS)- The AST is located in the basement of the
dwelling. Reportedly, the release impacted the irrigation well with little significant surficial
migration.. The Department was notified and a Notice of Responsibility (NOR) was subsequently
issued to United Oil Incorporated (United). Release Tracking Number (RTN) 4-13785 was
assigned by the Department. Immediate Response Actions (IRAs) verbally approved by the
Department at the time of notification included excavation of up to one -hundred (106) cubic
yards of contaminated soil, evacuation of up to 3,000 gallons of oily groundwater from the
former iriigation well, and assessment activities to include installation of groundwater
monitoring wells.
During April, 1998, Cyn Environmental Services (Cyn) personnel supervised the completion
of remedial actions and assessment at the subject property. Removal activities completed
included soil removal f m. the former irrigation well area, removal of the irrigation well and
collection and subsequent disposal of oily groundwater from. the excavation. A four -inch
diameter recovery well was later installed in the excavation to facilitate further recovery efforts.
A total of approximately 2,700 gallons of oily groundwater was historically collected and
disposed of from the excavation and subsequent development of the recovery well during
groundwater sampling evolutions.. A total of 65 tons of petroleum product impacted soil was

disposed of via transportation to the Aggregate Industries recycling facility in Stoughton, MA.
1301 documentation was reportedly included in a submitted IRA Modification Plan.
During April, 1998, assessment activities conducted at the site included the advancement
of hollow stem auger (HSA) soil borings with the installation of two-inch diameter groundwater
monitoring wells. In addition, Geo-probe soil borings were advanced in the immediate vicinity
of the release to collect soil samples in effort to determine the extent of the release in sub -surface
soils. Selected soil samples collected from the Geo-probe borings were subsequently analyzed
for the presence of Extractable Petroleum Hydrocarbons (EPH) including target Polynuclear
Aromatic Hydrocarbons (PAIIs) and Volatile Petroleum Hydrocarbon (VPI•I) with targeted
Volatile Organic Compounds (VOCs). Geo-probe soil. analytical results were below appropriate
Method I Standards. Sampling and subsequent laboratory analysis of the installed groundwater
monitoring wells revealed detected concentrations of various EPH and VPH fractions that
exceeded applicable Method I groundwater standards. In addition, sampling and subsequent
EPH, VPH, PAH and VOC analyses of stream water and stream bank sediments was also
conducted. Results of the stream and sediment sampling did not reveal a condition of concern
attributable to the subject release. The determined on -site groundwater flow direction is south,
cross gradient from the abutting southwards flowing stream. These results were summarized in a
Phase I / Tier IC Classification submittal received by the Department during April, 1999.
During August 1999, on behalf of United, Cyn prepared and submitted a modified IRA Plan,
..was-srtbmitted-in-orderto-seek-Department-approval-to-conduct additional sub -surface
assessment. The Plan also requested the use of an insitu treatment application of a chemical
neutralization compound, to be followed by multiple groundwater sampling evolutions.
The Department completed review of the Tier IC submittal and issued the Tier IC Permit
on October 19, 1999. As part of the Permit, the Department laid out additional required response
actions to be performed. These actions- included indoor air sampling of the on -site residential
dwelling and the sampling of the sump located in the basement of the dwelling. Results of these
assessment efforts subsequently submitted to the Department did not reveal a condition of
concern attributable to the subject release.
On August 3, 2000, Cya personnel supervised the installation of eight (8) remedial
additive injection points via Geo-probe borings. The remedial additive was subsequently applied
in these injection points during August 2000 and October 2001. Reportedly, based on low
analytical results from surrounding monitoring wells and high detected concentrations of FPH
and VPH fractions from MW-2, which is in close proximity to the former irrigation well, it was
determined that a last application Of remedial additive would be directly applied to MW-2. This
application occurred during May, 2002. In effort to monitor the effectiveness of the remedial
additive applications, a post application quarterly groundwater sampling and analysis program
was conducted between May, 2002 and April, 2003. All groundwater analysis results were non -
detect for all analyses.
The Class A-2 Response Action Outcome (RAO) was submitted to the Department on June
18, 2003. The RAO, which includes a Method 1 Risk Characterization, documented that a
Condition of No Significant Risk exists at the site and a Permanent Solution has been achieved.

III, RISK CHARACTERIZATION
The Method I Risk Characterization completed for the subject release included petroleum
products in soil and groundwater as Contaminants of Concern (COC)_ Impacted soils at the site
were categorized as S-1. Due to a shallow depth to water and the fact that the site is located in a
Zone IT of a municipal water supply, groundwater was categorized as OW-1, CrW-2 and OW-3.
Developed EPCs for soil did not exceed Method 1 standards. Quarterly post groundwater
treatment analytical results were below applicable Method 1 standards.
IV. SITE INSPECTION
On December 13, 2005, Bob Murphy of the Department, accompanied by Mr. Cauldwell and
Mr. Philip McBain, the Licensed Site Professional -of -Record, conducted a visual walkthrough of
the site. The noted stream appeared free of any petroleum product. No olfactory or visual evidence
of .impact was observed in the dwelling basement. No othei observations of concern were noted
during the inspection-
V. DETERMINATIONS
On the basis of the activities performed during the audit and in reliance upon the accuracy ol'that
information, the Department makes the following Determination:
No violations were identified.
