Final Inspection

The Department of Environmental Protection (Department), in response to a request, had one of its
engineers inspect the newly constricted Dustin/Prescott Wells Water Treatment Facility. The
results of an inspection conducted on August 11, 2005, indicate that the facility was constructed in
accordance with the plans approved by the Department in a communication dated October 6, 2003.
The Department has classified the new treatment facility at a Class II-T rating.
The Department hereby approves the new/modified pumping and treatment works for use subject to
any and all provisions previously identified.
Be advised that the identification number for the treatment works will be designated as Treatment
Plant ID #4167000-08T.
Attached is a revised copy of a water quality sampling schedule that includes the new well grouping
that is now being directed to the subject treatment facility.
In addition, please review the document and notify this office if any changes may be warranted. If
the new sampling schedule is acceptable to you, please disregard your previous sampling schedule
and provide a copy of this revised schedule to your laboratory.
